The effect of diazepam (10 mg i.m. X 7 days) on crossed and uncrossed acoustic stapedius reflex parameters (threshold, amplitude and duration) has been studied in 7 normal hearing subjects. The results showed that diazepam has a not clinically significant damping effect on the threshold, the amplitude and, very often, the duration of both ipsilateral and contralateral acoustic stapedius reflexes.

Ipsilateral stapedius reflex testing has been evaluated in 166 patients. It proves of value in defining the state of an ear (1) opposite to a unilateral conductive loss; (2) the less hard-of-hearing ear in the presence of bilateral asymmetrical sensorineural hearing losses where the worse ear has sufficiently severe loss to prevent a contralateral reflex from being elicited; (3) in patients with absent contralateral acoustic reflexes at 500, 1000, and 2000 Hz; and (4) in suspected central lesions. Experiments were performed in 18 patients with peripheral facial palsy (Bell's palsy) including unilateral stapedius muscle paralysis. After exposure to narrow band noise centered at 0.5 kHz temporary threshold shift at 0.75 kHz was significantly higher in the ear with paralysis than in the normal ear. The growth function of the contralateral stapedius reflex elicited by analog electrostimulation via cochlear implant was investigated in 9 patients and compared to reflex functions obtained for acoustic stimulation in 3 normal-hearing subjects. The reflex thresholds and initial latency times of the stapedial reflex of 50 patients with normal hearing and cochlear hearing loss were examined using Madson ZO 73 equipment. The following results were obtained: 1. The initial latency times of the stapedius reflex diminish proportional to the supra-threshold loudness. 2. No significant difference of the latency times was found between 0.5, 1, 2 and 4 kHz. 3. The initial latency times of patients with positive recruitment (cochlear hearing loss) do not differ from those of patients with normal hearing.

The on-off effect of the acoustic stapedius reflex was found in association with several kinds of ear disease: otosclerotic stapes fixation, incudostapedial dislocation and sensorineural hearing loss. One fifth (8/40) of the patient group with an on-off effect and sensorineural hearing loss showed the typical symptoms of Ménière's syndrome.

The depression of the stapedius reflex after a whole workday in industrial noise was studied in 11 normal-hearing employees at a shipbuilding yard. The reflex activity was assessed with simultaneous bilateral recordings of impedance change induced by stimulation with 2 000 and 500 Hz tones. One ear was exposed for approximately 7 hours in the plant, the other one was protected. The dose was measured individually at 89--103 dB(A) equivalent level. Generally, a bilateral symmetry of the acoustic stapedius reflex is assumed. But a homolaterally higher sensitivity has been described. We examined 71 patients with complete unilateral deafness. With a TDH 39 headphone, stimuli of intensities up to 133 dB were conveyed to the deaf ear. A fairly accurate estimate of the normal homolateral threshold is obtained by correcting for the cross-hearing loss and by mathematical consideration of the relative probabilities of the positive and negative cases. Following surgical implantation of a cochlear implant, the external processor must be programmed individually, which is typically achieved by behavioral measurements, with the user indicating the perceived loudness between the threshold and the maximum comfort levels (MCLs). However, the stapedius reflex measurement could be used instead, as the basis for programming the fitting maps. The electrically evoked stapedius reflex threshold is known to have a high correlation with the MCLs and thus is used as an objective tool to determine the MCL, especially in children and non-cooperative patients
