Stargardt disease: towards developing a model to predict phenotype.

Stargardt disease is an ABCA4-associated retinopathy, which generally follows an autosomal recessive inheritance pattern and is a frequent cause of macular degeneration in childhood. ABCA4 displays significant allelic heterogeneity whereby different mutations can cause retinal diseases with varying severity and age of onset. A genotype-phenotype model has been proposed linking ABCA4 mutations, purported ABCA4 functional protein activity and severity of disease, as measured by degree of visual loss and the age of onset. It has, however, been difficult to verify this model statistically in obser

Genotype-phenotype correlation in Italian families with Stargardt disease.

Autosomal recessive Stargardt disease (STGD) has been associated with substantial genetic and phenotypic heterogeneity. By systematic clinical analyses of STGD patients with complete genetic data (i.e. identified mutations on both alleles of the ABCA4 gene), we set out to determine phenotypic subtypes and to correlate these with specific ABCA4 alleles. Twenty-eight patients from 18 families with STGD/fundus flavimaculatus were investigated. All patients were submitted to complete ophthalmologic examination, electrophysiology, fluorescein angiography and ABCA4 gene chip analysis. Two main clini

Choroidal Patterns in Stargardt Disease: Correlations with Visual Acuity and Disease Progression.

BACKGROUND: To identify different choroidal patterns in Stargardt disease (STGD) and to assess their clinical correlates. METHODS: 100 STGD eyes (29 males; mean age 42.6 ± 16.5 years) and 100 control eyes (29 males; mean age 43.2 ± 8.5 years) were included. Optical coherence tomography (OCT) and OCT angiography (OCTA) images were obtained. Four different choroidal patterns, quantitative OCT and OCTA parameters were assessed and statistically analyzed. The main outcome was the correlation between each choroidal pattern and anatomical and functional retinal status. Furthermore, we assessed stru

Bisretinoids as a Source of Early Photoreceptor Pathology in Stargardt Disease.

BACKGROUND: Stargardt disease (STGD1) due to biallelic mutations in the ABCA4 gene is the most frequent single-gene retinal disease with a genetic prevalence of about 1 in 7,000. Pathology in STGD1 is due to dysfunction of a retina-specific vitamin A transporter which causes accumulation of cytotoxic vitamin A byproducts known as bisretinoids. Bisretinoids produce a distinct autofluorescent emission within affected cells that is seen to precede atrophy of the retinal pigment epithelium (RPE), photoreceptor cell death, and vision loss. This sequence of pathogenesis, and the fact that formation

Stargardt's disease presenting with bilateral central ring scotoma.

Ring scotoma is an annular field defect centered on fixation. Age-related macular degeneration in the elderly and hydroxychloroquine toxicity in younger patients are usual causes of central ring scotoma. We report bilateral ring scotoma as the presentation of adult onset Stargardt's disease. Central ring scotoma has a precise localizing value, to a lesion involving macula.

Stargardt's disease is not allelic to the genes for neuronal ceroid lipofuscinoses.

Stargardt's disease is an autosomal recessive condition characterised by a rapid and bilateral loss of central vision at around 7 to 12 years, with typical changes in the macular and perimacular region. It is one of the most frequent causes of macular degeneration in childhood and accounts for 7% of all retinal dystrophies. Considering that inclusions of lipofuscin-like substances are observed in retinal pigmentary cells of patients with Stargardt's disease on the one hand, and that the early symptoms of neuronal ceroid lipofuscinosis (CLN3) are suggestive of Stargardt's disease on the other h

[Retinal angiomatosis in association with Stargardt's disease. A case report].

We present what we believe to be the first reported association between retinal angiomatosis and Stardgardt's disease (Gass type 3). We have had the opportunity of following photographically a patient for the last eleven years. The earliest photos showed the macular problem whilst the latter shots showed the presence of a capillary angioma involving the temporal part of the optic disc and the adjacent retina of the left eye. We believe that this tumour arose in previously normal tissue. Examination and investigation of the patient revealed a peripapillary choroidal rupture at the site of the a

Nutritional supplements: current evidence for retinitis pigmentosa and Stargardt disease.

PURPOSE OF REVIEW: Inherited retinal diseases (IRDs) are genetically and phenotypically heterogeneous disorders that cause progressive vision loss and lack broadly effective disease-modifying therapies. Increasing evidence implicates metabolic stress, oxidative injury, and photoreceptor-retinal pigment epithelium (RPE) dysfunction in IRD pathophysiology. This review evaluates the evidence and limitations surrounding nutritional and metabolic interventions for retinitis pigmentosa and Stargardt disease.A narrative review of preclinical studies, randomized controlled trials, and contemporary gen

Inferred retinal sensitivity in recessive Stargardt disease using machine learning.

Spatially-resolved retinal function can be measured by psychophysical testing like fundus-controlled perimetry (FCP or 'microperimetry'). It may serve as a performance outcome measure in emerging interventional clinical trials for macular diseases as requested by regulatory agencies. As FCP constitute laborious examinations, we have evaluated a machine-learning-based approach to predict spatially-resolved retinal function ('inferred sensitivity') based on microstructural imaging (obtained by spectral domain optical coherence tomography) and patient data in recessive Stargardt disease. Using ne

Recessive multiple epiphyseal dysplasia and Stargardt disease in two sisters.

BACKGROUND: The rapid spread of genome-wide next-generation sequencing in the molecular diagnosis of rare genetic disorders has produced increasing evidence of multilocus genomic variations in cases with a previously well-characterized molecular diagnosis. Here, we describe two patients with a rare combination of skeletal abnormalities and retinal dystrophy caused by variants in the SLC26A2 and ABCA4 genes, respectively, in a family with parental consanguinity. METHODS: Next-generation sequencing and Sanger sequencing were performed to obtain a molecular diagnosis for the retinal and skeletal
