Attenuating Cardiac Sympathetic Tone via Cryoneurolytic Stellate Ganglion Ablation.

BACKGROUND: Stellate ganglion blockade provides temporary control of refractory ventricular arrhythmias by reducing sympathetic flow to the heart. However, current percutaneous approaches are limited by short duration of effect, creating a need for longer-lasting sympathetic modulation without more invasive permanent sympathectomy. OBJECTIVE: To determine the efficacy and durability of stellate ganglion cryoneurolytic ablation in attenuating evoked cardiac sympathetic responses. METHODS: We conducted acute cryoablation studies in 6 Yorkshire pigs and chronic studies in 10 Yucatan minipigs (7…

Management of Rapid Atrial Fibrillation Using Stellate Ganglion Blockade.

BACKGROUND: Stellate ganglion blockade has been demonstrated as an efficacious treatment for refractory ventricular tachyarrhythmias. There are no published reports of the use of this technique in management of rapid atrial arrhythmia. CASE SUMMARY: A 73-year-old woman presented to our institution in rapid atrial fibrillation and cardiogenic shock. She was intubated for airway protection and underwent successful electrical cardioversion. However, atrial fibrillation returned multiple times with weaning of sedation for extubation. Stellate ganglion infusion catheter was placed with infusion of…

Stellate ganglion block for treating post-COVID-19 parosmia.

BACKGROUND: Post-COVID parosmia may be due to dysautonomia and sympathetic hyperresponsiveness, which can be attenuated by stellate ganglion block (SGB). This study evaluates SGB as a treatment for post-COVID olfactory dysfunction (OD). METHODS: Retrospective case series with prospective data of patients with post-COVID OD undergoing unilateral (UL) or bilateral (BL) SGB. Patients completed Brief Smell Identification Tests (BSIT) (12 points maximum) and post-procedure surveys including parosmia severity scores on a scale of 1 (absent) to 10 (severe). Scores were compared from before treatment…

Establishment of ultrasound-guided stellate ganglion block in rats.

BACKGROUND: A novel protocol for accurate stellate ganglion block under ultrasound guidance was designed in rats. This technique raises the success rate of stellate ganglion block and reduces the incidence of brachial plexus and vagus nerve block. METHODS: Fifty-six Sprague-Dawley were randomly divided into an ultrasound-guided group (n = 28) and a blind technique group (n = 28). The rats in the blind technique group were injected with 1.5% lidocaine mixed with methylene blue after signs of brachial plexus stimulation were elicited. The lateral side of the cephalic brachial vein was located u…

How does stellate ganglion block alleviate immunologically-linked disorders?

BACKGROUND: The stellate ganglion is an autonomic nervous ganglion, formed by the fusion of the inferior cervical sympathetic ganglion and the first thoracic sympathetic ganglion, which is present in about 80% of people. It is anterior to the neck of the first rib and contains neurons that supply sympathetic innervation to the head and neck. Injection of local anesthetics near the stellate ganglion (stellate ganglion block; SGB) has been used for multiple clinical indications including sympathetic-mediated pain and vascular insufficiency syndromes of the upper extremity. In addition, reports o…

Facial pain: a possible therapy with stellate ganglion block.

OBJECTIVE: The goal of the present study is to verify the efficacy of stellate ganglion block (SGB) in the treatment of facial pain that can be found in different pathological syndromes, and also to examine whether the efficacy is dependent upon when this therapy is administered. PATIENTS: Fifty patients (divided into two randomized groups) with facial pain caused by traumas, iatrogenic issues, herpes zoster, or neurological pathologies participated in this study. DESIGN AND INTERVENTIONS: The first group (N = 25) was treated with SGB produced by 10 administrations of 10 mg of levobupivacain…

Ultrasound-guided stellate ganglion block successfully prevented esophageal puncture.

Stellate ganglion block is utilized in the diagnosis and management of various vascular disorders and sympathetically mediated pain in the upper extremity, head and neck. The cervical sympathetic chain is composed of superior, middle, intermediate, and inferior cervical ganglia. However, in approximately 80% of the population, the inferior cervical ganglion is fused with the first thoracic ganglion, forming the stellate ganglion also known as cervicothoracic ganglion. The stellate ganglion lies medial to the scalene muscles, lateral to the longus coli muscle, esophagus and trachea along with t…

Comparison between neurotropin and mepivacaine for stellate ganglion injection.

Neurotropin, a nonproteinaceous extract from the inflamed skin of rabbits inoculated with vaccinia virus, is reported to decrease pain effectively when used for stellate ganglion (SG) injection. We compared the effects of neurotropin SG injection with those of mepivacaine on pain relief, as well as comparing the side effects. One hundred and eighty-eight SG injections in 15 patients (5 with postherpetic neuralgia and 10 with sudden deafness) were performed either with 1% mepivacaine 6 ml or with neurotropin 3 ml combined with saline 3 ml in turn. Fifteen min before and after the injection, the…

[Stellate ganglion radiofrequency neurolysis under CT guidance. Preliminary study].

Sympathetically maintained pain syndrome of the upper limb is difficult to treat even with high doses of specific medication. Stellate ganglion block by in situ injection of a local anesthetic is an efficient and accepted method for diagnosis and treatment. The sedative effect is however transitory linked to the short effect of the drug. CT guidance, displaying an excellent contrast between soft tissues, bones, vessels and nerves, is a well suited and safe mean of guidance. Seven patients suffering from reflex sympathetic dystrophy were treated by stellate ganglion radiofrequency (RF) neurolys…

Stellate ganglion drives sympathetic regulation of cochlear blood flow.

The functional properties of the sympathetic fibers innervating the cochlea are not well understood. Adrenergic fibers supplying lateral wal … le in control of cochlear blood flow (CBF), we examined the effect of electrical stimulation of the stellate ganglion (ESS) and transection of postganglionic fibers from the stellate on CBF measured by laser Doppler flowmetry and on systemic blood pressure (BP) in the guinea pig. ESS produced a 20-35% increase in BP and 10-15% decrease in CBF. The decrease in CBF p …

Stellate ganglion block in perioperative practice: a narrative review.

The stellate ganglion block, also known as the cervicothoracic ganglion block, has traditionally been utilised by chronic pain physicians for managing sympathetic-mediated pathologies, such as chronic regional pain syndrome and hyperhidrosis. However, recent evidence suggests that the SGB, by inhibiting sympathetic outflow, modulating vasodilation and regional perfusion, and attenuating stress and inflammation responses, might serve as a safe and minimally invasive intervention for preventing or treating common conditions observed in surgical patients. Its potential applications include preven…

Anti-arrhythmic targeting of sympathetic stellate ganglion P2X3 receptors.

Sympathetic hyperactivity associated with many primary cardiovascular diseases is pro-arrhythmic and a key contributor to sudden cardiac death. Surgically removing the sympathetic stellate ganglion has demonstrated therapeutic potential in minimising cardiac arrhythmia, but there are off-target risks prompting the need for more site-specific non-invasive approaches. Here we hypothesised that P2X3 purinergic receptors (P2X3R) may be a therapeutic target and play a role in the excessive sympathetic drive to the heart in cardiovascular disease. We investigated the role of P2X3R in the sympathetic…

Intractable Hiccups Treated With Stellate Ganglion Block: Case Report.

BACKGROUND: A hiccup is a common well-documented physiologic process that occurs due to a sudden involuntary contraction of the diaphragm and intercostal muscles, with subsequent rapid closure of the glottis. While acute hiccups, lasting < 48 hours, are experienced by almost all individuals at some point in their life, prolonged hiccups lasting > 48 hours, or intractable hiccups lasting > 1 month, are quite rare and may significantly impact a person's quality of life. We would like to share a report of a stellate ganglion (SG) block relieving the discomfort of a patient suffering from idiopath…
