BACKGROUND: Gastric antral vascular ectasia (GAVE) is a source of GI bleeding. Several associated conditions have been recognized. However, there is little information on the occurrence of ectatic lesions in regions of the stomach other than the antrum, that is, the cardia. Patients in whom a diagnosis of GAVE was made from 1990 to 2000 are presented with an emphasis on the frequency of contemporaneous ectatic lesions in the gastric cardia.

INTRODUCTION: During a 5-year time frame, five cases of symptomatic gastric volvulus were diagnosed and treated in our department. Four presented with the acute form of gastric volvulus and underwent emergency surgery. The fifth suffered the chronic variant of the syndrome and was benefited by nonoperative management. The cases serve to remind emergency physicians of the spectrum of gastric volvulus. The report supplements the sparse clinical description in the emergency literature.

Based on own experience and on published data we report about indications and efficacy of endoscopic ultrasonography (EUS) in esophageal and gastric diseases. At the present time the following conclusions can be drawn: Submucous tumors can be clearly demonstrated and unequivocally distinguished from extramural compressions. The records of all upper digestive endoscopies performed in one year in the Clinical Measurement Department of a teaching hospital serving a population of 250 000 were reviewed; 25% of patients had oesophagitis, which was usually associated with other significant findings such as peptic ulcer and gastroduodenal erosions. Only about 1 in 6 patients with oesophagitis had an endoscopically identified hiatus hernia. Oesophagitis was associated with gastric acid hypersecretion in both men and women.

Examining the gastroduodenal mucosa (GDM) in 61 patients with chronic obstructive pulmonary diseases (COPD) showed changes in the areas studied in all cases. GDM atrophy increased with the severity of COPD. Helicobacter pylori are essential in developing gastric pathology in patients with mild bronchopulmonary disease. The severity of atrophic gastritis increases with the longer duration of COPD and with more severity of hypoxia and bronchial obstruction.

The incidence and distribution of chronic gastritis, chronicatrophic gastritis and epithelial dysplasia I-III have been investigated in 50 resected stomachs of patients suffering from duodenal ulcer, gastric ulcer, early or advanced gastric cancer. Only in gastric cancer epithelial dysplasia III has been frequently observed, particularly in the neighbourhood of gastric cancer. Distribution of chronic-atrophic gastritis was similar to the distribution of dysplasia I and II. These mucosal lesions were detectable with the same frequency in patients with or without gastric cancer.

There was installed a significant positive relationship between body mass index (BMI) and the velocity of cicatrization of ulcer in patients with ulcer disease (PUD) with trophological violations. The method of C. pylori diagnosis proposed by the authors proved more informative that histological detection of the infection. The technique employs evaluation of urease activity in gastric contents. Upon comparison of the two diagnostic modalities in 30 patients with chronic gastroduodenitis and peptic ulcer there was no definite correlation in their results. It was noted that C. pylori invasion produced negligible characteristic symptoms in gastroduodenal affections.
