Guidelines for diagnosis and management of aphthous stomatitis.

Aphthous ulcers are the most common oral mucosal lesions in the general population. These often are recurrent and periodic lesions that cause clinically significant morbidity. Many suggestions have been proposed but the etiology of recurrent aphthous stomatitis (RAS) is unknown. Several precipitating factors for aphthous ulcers appear to operate in subjects with genetic predisposition. An autoimmune or hypersensitivity mechanism is widely considered possible. Sometimes aphthous ulcers can be the sign of systemic diseases, so it is essential to establish a correct diagnosis to determine suitabl

Aphthous stomatitis with involvement of attached gingiva.

Involvement of the attached gingiva by aphthous stomatitis is considered uncommon. This report presents two cases in which patients with aphthous stomatitis developed ulcers on multiple mucosal surfaces, including the attached gingiva. The gingival ulcers began on the marginal gingiva and enlarged to form V- or U-shaped ulcers on the attached gingiva. Other disorders producing gingival ulceration were excluded on the basis of history, clinical features, bacterial culture, viral culture, and biopsy with routine microscopic study as well as direct immunofluorescence.

Use of levamisole in recurrent aphthous stomatitis.

A review of the use of levamisole in the treatment of recurrent aphthous stomatitis is presented. While various uncontrolled clinical trials have demonstrated its effectiveness in increasing the time between episodes and shortening the duration of the ulceration, a series of double-blind trials have given conflicting results that have not entirely confirmed the open trials. Side effects, of which dysgeusia, hyperosmia and nausea are the most frequent, and the administration of the drug are also discussed.

Management of recurrent aphthous stomatitis. A review.

The management of recurrent aphthous ulceration is discussed. It appears that the treatment of choice in mild to moderately severe aphthous stomatitis is repeated application of topical tetracycline hydrochloride. Viscous lidocaine (2 percent) provides symptomatic relief and is therefore a useful adjunct to therapy. Cyclical estrogens appear to be helpful in recurrent premenstrual aphthosis. Immune-regulating agents such as levamisole hold promise for the future.

Aspects of the fine anatomy of aphthous stomatitis.

Samples of human aphthous stomatitis were studied under the electron microscope. The epithelial cells bordering the ulcer present multivesicular bodies, pinocytotic vesicles, primary lysosomes, digestive vacuoles, and autophagic vacuoles. The golgicomplex is noticeably increased; free ribosomes and rough endoplasmic reticulum are abundant. Crystalline structures are close to the nucleus. One or two paranuclear bodies are in most of the nuclei. A frank cell injury is evident, causing a major activity of the digestive cell system; the origin and formation of lysosomes through r.e.r. and the Golg

Recurrent aphthous stomatitis: genetic aspects of etiology.

Recurrent aphthous stomatitis (RAS; recurrent aphthous ulcers - RAU; canker sores) is a chronic inflammatory, ulcerative condition of the oral mucosa. Its prevalence in the general population ranges between 5% and 20%, depending on the method and group studied. The etiopathogenesis of the disease is considered to be multifactorial, but remains still not fully understood. In patients with RAS, an enhanced immunologic response occurs to some trigger factors that may include: mechanical injury, stress or bacterial and viral antigens. Higher prevalence of aphthae in relatives may also indicate the

Thyroid autoimmunity associated with recurrent aphthous stomatitis.

BACKGROUND: Recurrent aphthous stomatitis (RAS) is an autoimmune disorder characterized by the periodic appearance of aphthous lesions on the oral mucosa. TH1 cytokines plays a key role in the aetiopathogenesis. Autoimmune thyroid disease (ATD) is the most common autoimmune disease and is frequently accompanied by various other autoimmune diseases. OBJECTIVE: To investigate the frequency of ATD which has not been studied in the patients with RAS. METHODS: Ninety patients and 30 healthy volunteers were included into the study. The serum samples were assayed for thyroid stimulant hormone (TSH)

Polymorphonuclear neutrophil function in recurrent aphthous stomatitis.

Polymorphonuclear neutrophils (PMN) are the predominant cell type in the established lesion of recurrent aphthous stomatitis (RAS). The function of these cells was studied in 15 patients with RAS and 15 age and sex matched controls. Production of oxygen radicals was assessed using cytochrome-c and nitroblue tetrazolium dye (NBT) reduction assays. Oxidative metabolism was found to be similar for experimental and control groups in both stimulated and non-stimulated assays. Adherence to glass surfaces by PMN was also measured. Adherence of PMN, both from patients and controls, was significantly r

Azithromycin-induced Aphthous Stomatitis: A Case Series.

BACKGROUND: Azithromycin is one of the most popular antibiotics in current clinical practice. This medication generally considered to be safe and well-tolerated in different demographic populations. Like any other drug, azithromycin use is not without risk and adverse effects. In recent years, cardiovascular accidents have been announced as its major and most important side effect. But azithromycin use can be accompanied with less recognized complications which are significantly discomforting. In this article, we presented a neglected adverse effect of azithromycin in medical literature which

Laryngeal manifestations in atypical recurrent aphthous stomatitis.

UNLABELLED: Recurrent aphthoid stomatitis is characteristically observed in children and adolescents in the form of painful relapsing ulcers in the oral mucosa unaccompanied by evidences of systemic disease. The ulcers appear every one or two weeks for at least one entire year. Some patients suspected for recurrent aphthoid stomatitis develop lesions in atypical sites - mainly in the larynx - concurrently to the ones found in the oral mucosa. AIM: this study aims to describe a series of recurrent aphthoid stomatitis patients with atypical laryngeal injuries. STUDY DESIGN: this is a case seri

Recurrent aphthous stomatitis: etiology, diagnosis, and treatment.

Recurrent aphthous stomatitis is a very common condition seen frequently by dentists in practice. Although the majority of cases are benign and resolve in less than two weeks, these ulcerations may be indicative of underlying systemic diseases ranging from vitamin deficiency to autoimmunity. Awareness of these correlations can help the dentist make or aid the diagnosis of potentially serious conditions. There are treatments of which the practitioner should be aware that may ameliorate the pain of the ulcerations or shorten their duration. The diagnosis and management of this important conditio

Role of biologics in refractory recurrent aphthous stomatitis.

Refractory recurrent aphthous stomatitis (RRAS) manifests as severe ulcerative lesions of the oral mucosa with poor healing and a poor response to conventional therapy, with or without systemic diseases. Its treatment remains a clinical challenge owing to the lack of effective therapies. Recently, biologics have emerged as promising targeted therapies for RRAS. The biologics targeting specific inflammatory pathways involved in the pathogenesis of RRAS, including tumor necrosis factor-alpha inhibitors and interleukin inhibitors, offer a more precise and promising therapeutic approach for RRAS.

Treatment of recurrent aphthous stomatitis. A literature review.

Recurrent aphthous stomatitis (RAS) is the most common chronic disease of the oral cavity, affecting 5-25% of the population. The underlying etiology remains unclear, and no curative treatment is available. The present review examines the existing treatments for RAS with the purpose of answering a number of questions: How should these patients be treated in the dental clinic? What topical drugs are available and when should they be used? What systemic drugs are available and when should they be used? A literature search was made of the PubMed, Cochrane and Scopus databases, limited to articles

Comorbidity of recurrent aphthous stomatitis and polyps ventriculi.

As it is known, many diseases of gastric system cause changes in the oral cavity, with either pathological findings or subjective impressions. When these changes are of pathological nature, the most common finding is recurrent aphthous stomatitis on the tongue, which emerges as a consequence of gastric diseases. Recurrent aphthous stomatitis is a disorder characterised by recurrent ulcerations limited to the oral mucosa, without any other signs of diseases. According to their clinical form, they may be big, small and hyperform. Etiology of recurrent aphthae is genetic predisposition, systemic

Celiac disease in patients having recurrent aphthous stomatitis.

BACKGROUND/AIMS: Celiac disease is a condition related to the small intestine's intolerance to gluten. In epidemiologic studies the prevalence is highly variable. The diagnosis can be difficult due to the wide spectrum of signs and symptoms. As the risk for intestinal lymphoma is higher in these patients, early diagnosis has its privileges. The higher prevalence of recurrent aphthous stomatitis in celiac disease led us to investigate the celiac disease prevalence in patients with recurrent aphthous stomatitis, which might assist in diagnosis of asymptomatic celiac disease patients.
