While the number of people waiting heart transplantation increases, the number of organ donors decreases. This shrinking donor pool has prompted reassessment of donor selection for heart transplantation. Bench repair of a donor aortic valve was performed before minimal access orthotopic heart transplantation. Aortic insufficiency in the structurally normal tricuspid aortic valve was due to annular dilatation and was corrected with subcommissural annular plication. The postoperative period was uneventful. Follow-up at 4.5 years showed good results and no evidence of aortic regurgitation.

Three days following bilateral adrenalectomy the following findings were revealed in the hypothalamus-hypophysis and circum-ventricular organs of Balb mice: considerable enlargement of neuronal perikarya and nuclei in the nucleus arcuatus of the hypothalamus. Amitotic divisions were frequent in neurons and glial cells. Mitoses were also sporadically encountered. The pars distalis of the hypophysis showed typical post-adrenalectomy changes. In addition, enlargement of the so-called pars rostralis was observed. This part and the pars intermedia showed signs of enhanced secretion. The subfornical

Using an immunoperoxidase technique at the light and electron microscope levels, the localization of somatostatin and luteinizing hormone-releasing hormone (LH-RH) has been performed in the brain, as well as the pancreas and stomach. The following generalizations may be drawn on the localization of LH-RH and somatostatin. (a) LH-RH and somatostatin are contained in axons and nerve endings of the median eminence and organum vasculosum of the lamina terminalis. (b) LH-RH and somatostatin are present in different nerve endings. (c) In the subcommissural organ, subfornical organ and area postrema,

OBJECTIVE: The pineal region is a rare intracranial site for metastasis. We report three patients initially considered to have metastatic papillary adenocarcinoma to the pineal region. On review, these papillary, keratin-positive neoplasms meet the criteria for papillary tumor of the pineal region (PTPR).
CLINICAL PRESENTATION: These neoplasms occurred in three women (age range, 37-55 yr). Imaging studies demonstrated well-circumscribed lesions in the pineal region. All patients presented with obstructive hydrocephalus and symptoms attributable to hydrocephalus and tectal compression.

BACKGROUND: Dab2, one of two mammalian orthologs of Drosophila Disabled, has been shown to be involved in cell positioning and formation of visceral endoderm during mouse embryogenesis, but its role in neuronal development is not yet fully understood. In this report, we have examined the localization of the Dab2 protein in the mouse embryonic central nervous system (CNS) at different developmental stages.
RESULTS: Dab2 protein was transiently expressed in rhombomeres 5 and 6 of the developing hindbrain between E8.5 and E11.5, and in the floor plate of the neural tube from E9.5 to E12.5, follo

Presented is a patient with papillary tumor of the pineal region (PTPR), an uncommon and recently recognized neoplasm. As its name implies, PTPR does not arise from the pineal gland itself. The cell of origin is thought to be the specialized ependymocytes of the subcommissural organ. Primary tumors of the pineal region include pineal parenchymal neoplasms, germ cell neoplasms, and tumors arising from adjacent structures, including meningiomas, astrocytomas, and ependymomas. Like other masses in this location, PTPR often leads to obstructive hydrocephalus. Due to the relative paucity of reporte

Secretory neurons capable of elaborating neuropeptides and biogenic amines are an integral component of nervous systems. This apparatus is more extended than assumed during an earlier period of investigations. It is involved in short- and long-range communication by means of paracrine, transmitter-like, modulatory and neurohormonal types of messages. This finely adjusted activity of secretory neurons serves the control of a variety of important biological functions. Secretory pinealocytes are derivatives of pineal photoreceptors, primary sensory cells of neuronal character. In contrast to thes

The circumventricular organs (CVOs) are specialised neuroepithelial structures found in the midline of the brain, grouped around the third and fourth ventricles. They mediate the communication between the brain and the periphery by performing sensory and secretory roles, facilitated by increased vascularisation and the absence of a blood-brain barrier. Surprisingly little is known about the origins of the CVOs (both developmental and evolutionary), but their functional and organisational similarities raise the question of the extent of their relationship. Here, I review our current knowledge o

A papillary tumor of the pineal region (PTPR) is a rare tumor of neuroepithelial origin formed from specialized ependymocytes of the subcommissural organ located in the lining of the posterior commissure, not the pineal gland itself. Patients with this type of tumor generally present with nonspecific symptoms secondary to obstructive hydrocephalus such as headache and vision changes. The mean age of patient presentation is 31, with a slight predominance in females. This type of tumor has a high rate of recurrence (56%) following surgical resection. This case study describes the presentation of

Intermediate filament distribution was studied in children and adult human pineal glands by using immunocytochemistry and immunoblotting. The demonstration of cytokeratin positive cell clusters in the infant pineal gland may be related to ependymal cells arising from the subcommissural organ. The SCO ependymal cells and the ependymal cells in the pineal parenchyma express the neuro-ectoderm specific individual cytokeratins no. 8 and 18. Cytokeratin positive ependymocytes additionally exhibited vimentin and were serotonin negative as revealed by double immunofluorescence labelling. By contrast,

Of the posterior third ventricular tumors, a papillary tumor of the pineal gland is a rare entity that originates from specialized ependymoma of the subcommissural organ. In this video narration, we present a case of a 33-year-old male with headaches and recent cognitive decline due to a posterior third ventricular lesion. The patient underwent a posterior interhemispheric approach, and a gross-total decompression was achieved with no signs of recurrence in a 2-year follow-up period. With this case we highlight the microsurgical technique employed for decompressing tumors of the posterior thir

Papillary tumor of the pineal region has recently been proposed as a new, distinct clinicopathological entity. On the basis of the immunophenotypic and ultrastructural properties of these lesions, origin from specialized ependymocytes of the subcommissural organ was postulated. We present the third publication on a papillary pineal tumor and describe the morphological, immunohistochemical and ultrastructural features of this neoplasm. The patient was a young woman who presented with signs of raised intracranial pressure and Parinaud syndrome. Magnetic resonance imaging revealed a neoplastic le

OBJECTIVE: The mesocoelic recess (MCR) is found in the brain of human embryos and fetuses. The mesocoelic recess seems to be functionally related to the subcommissural organ that is one of neurosecretory organs involved in osmoregulation on the basis of data from other species. Subsequently, recent speculation as to the importance of the subcommissural organ in the development of congenital hydrocephalus has been raised. Yet unlike other mammals, MCR is known to be a vestigial structure in the adult human brain. Here, we performed the in vivo imaging identification of this space to investigate

The surface specializations of the wall of the third cerebral ventricle of Rana temporaria were investigated with the scanning electron microscope. These specializations can be divided into three types: cilia, large bulbous protrusions, and microvillus-like protrusions. Most parts of the ventricular surface are densely ciliated. In contrast, other regions are either scantily ciliated or devoid of cilia. Four areas of the ventricular surface are studded with numerous large bulbous protrusions. These large protrusions can be divided into two types: One type consists of intraventricular end bulbs

Pineal region tumors are heterogeneous lesions and include mainly pineal parenchymal tumors (PPTs), papillary tumors of the pineal region (PTPRs) and germ cell tumors (GCTs). This article describes the cystic pineal gland compared with normal tissue and histopathological features of the most frequent pineal region tumors. PPTs are subdivided into pineocytoma (grade I), pineoblastoma (grade IV) and tumors with intermediate differentiation (PPTIDs; grades II-III). A grading system based on the number of mitoses and neurofilament protein expression distinguishes low- from high-grade PPTID. PTPR i

1. The circumventricular organs (CVO) are structures that permit polypeptide hypothalamic hormones to leave the brain without disrupting the blood-brain barrier (BBB) and permit substances that do not cross the BBB to trigger changes in brain function. 2. In mammals, CVO include only the median eminence and adjacent neurohypophysis, organum vasculosum lamina terminalis, subfornical organ and the area postrema. 3. The CVO are characterized by their small size, high permeability and fenestrated capillaries. The subcommissural organ is not highly permeable and does not have fenestrated capillarie

The occurrence of about 17 circumventricular organs is described in 31 species belonging to various groups of vertebrates from cyclostomes to mammals and the phylogeny of each organ is discussed briefly. The neurohypophysis (median eminence and neural lobe) and the subcommissural organ are observed in all species studied. The pineal is also ubiquitous except for the hagfish (Eptatretus) and the caiman (Caiman). These four circumventricular organs are phylogenetically the oldest organs. The subfornical organ and the area postrema may be phylogenetically the youngest organs, although the subforn

Papillary tumor of the pineal region (PTPR) is a recently described neoplasm. Several studies have been published on this tumor, but its pathological features and the appropriate treatment remain unclear. PTPR is reported to originate from ependymal cells in the subcommissural organ. Isocitrate dehydrogenase 1 and 2 (IDH1/2) mutations have been a focus area in glioma research as promising predictors. We report a case of PTPR that was characterized by local recurrence, although subtotal removal and radiotherapy seemed effective for many months. Histological examination showed ependymal features
