Ligamentous injuries about the ankle and subtalar joints.

Ligamentous injuries at the ankle and subtalar joint range from simple sprains to severe talar dislocations. While lateral ankle sprains are among the most frequently encountered injuries and do not pose a greater diagnostic problem, the surgeon must be suspicious not to overlook associated ligamentous injuries at the subtalar and midtarsal level that may result in chronic painful conditions. Syndesmotic instabilities with or without ankle fractures must be assessed carefully and treated properly, since these are prearthrotic conditions. In the treatment of chronic ankle or subtalar instabilit

Arthrography of the foot and ankle. Ankle and subtalar joint.

Arthrography is the intra-articular injection of contrast media. This article reviews the normal and pathologic findings of standard arthrography and MR imaging arthrography of the ankle and subtalar joint. Standard arthrography is used primarily after acute ankle sprains, whereas MR imaging arthrography is used for staging and detecting osteochondritis dissecans of the talus, anterolateral soft tissue impingement, and chronic lateral ankle instability.

Effect of simulated joint instability and bracing on ankle and subtalar joint flexibility.

It is clinically challenging to distinguish between ankle and subtalar joints instability in vivo. Understanding the changes in load-displacement at the ankle and subtalar joints after ligament injuries may detect specific changes in joint characteristics that cannot be detected by investigating changes in range of motion alone. The effect of restricting joints end range of motion with ankle braces was already established, but little is known about the effect of an ankle brace on the flexibility of the injured ankle and subtalar joints. Therefore, the purposes of this study were to (1) underst

Intrinsic subtalar ligaments are primary varus stabilizers of the subtalar joint: A 3D cadaveric motion analysis.

BACKGROUND: The respective roles of subtalar joint intrinsic ligament (cervical ligament [CL], interosseous talocalcaneal ligament [ITCL]) versus the extrinsic calcaneofibular ligament (CFL) are unclear. This study aimed to use 3D opto-electronic analysis to compare their respective contributions to subtalar joint stability. METHODS: In this cadaveric comparative study, twenty feet were split into intrinsic or extrinsic groups and underwent sequential sectioning: (1) intact, (2) anterior talo-fibular ligament (ATFL) cut, (3) CFL or CL+ITCL cut, (4) all ligaments cut. At each stage, manual ant

Classification of the subtalar articular surface and its matching situation: an anatomical study on Chinese subtalar joint.

PURPOSE: The subtalar joint (STJ) is complex in anatomy and function. The purpose of this study is to classify the articular surface of the calcaneus in a sample Chinese population and discuss the relationship between its matching situation and the stability of STJ. METHODS: 328 patients with 445 STJs were measured and classified using CT three-dimensional reconstruction. The calcaneal facets were classified according to the morphological characteristics. According to the number, shape, and fusion of the calcaneus and talus facets, the matching situation was determined. The parameters of meas

Management of the Subtalar Joint Following Calcaneal Fracture Malunion.

Subtalar joint arthrosis is common following intra-articular calcaneus fractures. The appropriate management of pain secondary to posttraumatic arthritis depends on the status of the remaining posterior facet articular cartilage, the magnitude of any residual joint displacement and distortions in the overall morphology of the calcaneus. In select circumstances, joint-preserving surgical techniques may be considered including lateral wall exostectomy, far lateral posterior facet joint debridement, and intra-articular osteotomies. When the subtalar joint is not salvageable, some form of arthrode

Rates of subtalar joint osteoarthritis after modified Chrisman-snook.

Ankle sprains commonly damage the lateral compartment of the tibiotalar and subtalar joint. The presence of functional or mechanical instability leads to long term dysfunction of patients due to ankle laxity. The gold standard surgical procedure for chronic ankle instability is repair of the anterior talofibular and/or calcaneofibular ligaments with imbrication of the inferior extensor retinaculum. The senior author's indications for a modified Brostrom versus a modified Chrisman-Snook (CS) have evolved over the past decade. A potential complication with any technique that crosses the subtalar

A Mouse Model of Ankle-Subtalar Complex Joint Instability.

Ankle sprains are perhaps the most common sports injuries in daily life, often resulting in instability of the ankle-subtalar complex joint (ASCJ), and can eventually lead to post-traumatic osteoarthritis (PTOA) in the long term. However, due to the complexity of the injury mechanism and the clinical manifestations, such as ecchymosis, hematoma, or tenderness in the lateral foot, there is no clinical consensus on diagnosing and treating ASCJ instability. Since the musculoskeletal structure of the bones and ligaments of the mouse hindfoot is comparable to that of humans, an animal model of ASCJ

Surgical Treatment and Management for Chronic Dislocated Subtalar Joint.

Lateral dislocation of the subtalar joint is a relatively uncommon pathology. It has previously been described in the literature secondary to acute trauma. This form of dislocation can also be acquired and seen in severe long-standing cases of flatfoot and Charcot neuroarthropathy. This study aims to describe this "sidecar" deformity, etiologies of the deformity, and the surgical options for correction. This study was performed by reviewing medical records of a single foot and ankle surgeon for patients who met inclusion criteria and underwent surgical correction. The study period was from Oct

True posterior dislocation of subtalar joint: a case report.

Posterior dislocation subtalar joint is rare and usually results from a high-velocity injury. Previous reports of all complete posterior dislocations of the subtalar joint have had some degree of a rotational component. We report a case of a true complete posterior dislocation of subtalar joint with no rotation.

Talocrural and subtalar joint instability after lateral ankle sprain.

PURPOSE: Recurrence of lateral ankle sprain (LAS) is common among recreational and competitive athletes. Talocrural (TC) joint laxity has traditionally been seen as the cause of mechanical instability after LAS. The purpose of this study was to examine the use of stress fluoroscopy and physical examination in the assessment of TC and subtalar (ST) instability in subjects with and without a history of LAS. METHODS: Twelve subjects with a history of unilateral LAS and eight healthy controls were examined by two blinded examiners. The first examiner performed physical examination on each ankle b

The Effect of the Mobile Subtalar Joint on Calcaneal and Supramalleolar Osteotomies.

BACKGROUND: The mobile subtalar joint (STJ) may compensate for supra- and inframalleolar deformities and counteract the effect of realigning calcaneal or distal tibial osteotomies. The purpose of this study was to evaluate the compensatory effect of the mobile STJ after supramalleolar osteotomy (SMOT) and calcaneus osteotomy (COT) and whether the extent of the compensation correlates with STJ shape and orientation. METHODS: In 10 human lower leg cadavers without evidence of deformity or prior trauma 700 Newton load were applied as a simulated standing pose. The center of force (COF) migration

Arthroscopic Evaluation of the Subtalar Joint: A Review and Survey of Pathology.

BACKGROUND: A survey of pathology present in the subtalar joint by means of subtalar arthroscopy with anterolateral and middle portals has not been extensively explored in the literature. The focus of this study was to identify pathology in the subtalar joint with subtalar joint arthroscopy using this approach. We also compared these arthroscopic findings with those reported in the literature. METHODS: We performed a retrospective review of 49 consecutive patients who had undergone 53 subtalar arthroscopic procedures. Data were obtained from intraoperative arthroscopic findings that were docu

Arthroereisis of the subtalar joint: an uncommon complication and literature review.

Arthroereisis is an operation to prevent abnormal pronation in the subtalar joint in children and adults with symptomatic flexible flatfeet. In the present report, we describe an uncommon late complication of a former variation of this procedure, namely the case of an adult male who experienced migration of a xenogeneic bone graft that had been implanted 55 years earlier. A thorough review of the existing data was also undertaken to better understand the complications of this procedure.

Anatomic basis for a minimally invasive approach to the subtalar joint.

INTRODUCTION: An isolated arthrodesis of the talocalcaneonavicular joint is a common indication in cases of pain and post-traumatic arthroses. MATERIALS AND METHODS: Because of the high infection rate after surgery with the lateral incision, the authors decided to evaluate an alternative, minimally invasive procedure. Joint destruction was carried out via a posterolateral access after the insertion of two guidewires. For an evaluation of the risk for vessel and nerve structures, 102 ankle joint specimens preserved in formalin/alcohol were examined. Additionally, the minimally invasive access

[Subtalar arthrodeses and arthrodeses of the ankle joint (author's transl)].

We present a check-up study of five arthrodeses of the ankle joint and five subtalar arthrodeses. The arthrodeses of the ankle joint were indicated because of hard pains after bimalleolar fractures and fractures of the pilon tibial. Subtalar arthrodeses were indicated in posttraumatic arthritis of the calcaneus after intraarticular fractures of the calcaneus. Resection-compression-arthrodesis following Charnley was used for the ankle joint, subtalar arthrodesis was done by the technic of using a corticocancellous bone graft from the iliac crest.
