A shrinking service: can it mislead people into thinking there is less disease?

This study looks at the impact of reduced clinical services on infection rates in a small suburban genitourinary medicine clinic, and compares the rates with the national trends. Data were gathered from the KC60 reports and compared with national data on the Health Protection Association Website. Between 1996 and 2001, waiting time for appointments increased along with the number of patients not attending appointments. There was an apparent drop in the number of cases of chlamydial infection in contrast to a national increase; however, when the figures were adjusted for the fall in clinic size

Understanding complexity in psychological services: A modified Delphi study.

RATIONALE, AIMS AND OBJECTIVES: The concept of patient or case complexity is relevant-and widely used-at all levels and stages of mental health service provision, but there have been few methodologically robust attempts to define this term. This study aimed to establish a consensus on factors contributing to patient complexity in adult psychological services using Delphi methodology. METHOD: Applied psychologists in a single urban/suburban UK National Health Service setting took part in a three-round modified Delphi study. Twenty-eight respondents in Round 1 gave qualitative data on factors t

The pathways to primary mental health care in high-density suburbs in Harare, Zimbabwe.

The pathways to care for mental illness are diverse and are dependent on sociocultural and economic factors. The objective of this study was to describe the pathways to primary care for patients with common mental disorders in Harare. One hundred and nine consecutive patients with conspicuous psychiatric morbidity identified by general nurses in three primary health care clinics (PHC) and by four traditional medical practitioners were interviewed with the Pathways to Care Schedule. Other than those patients with an acute illness, most patients consulted more than one care provider; three-quart

The impact of social isolation policies during COVID-19 on suburban mothers and pregnant women with opioid use disorder.

The aim of this qualitative research paper was to identify how social isolation policies implemented during the COVID-19 pandemic impacted the lives of pregnant women and mothers with opioid use disorder. Between 2020 and 2022, in-depth interviews were conducted with 37 mothers and pregnant women living in the suburbs of Newark, NJ, and New Haven, CT. Participants had to be at least 18 years old, misused or abused opioids in the last 12 months while residing in a suburban location, and pregnant or caring for children aged 12 and younger. We used syndemics theory to guide our grounded theory an

Causes of maternal mortality in a semi-urban Nigerian setting.

Focus group discussions with people in Ekpoma, Nigeria, revealed them to be quite knowledgeable about haemorrhage in pregnancy and delivery. However, because of their inability to recognize early warning signs they continued traditional treatment even when clear evidence of danger existed. Furthermore, they tended not to seek help in clinics and hospitals because of sociocultural conditioning and a negative perception of the quality of care available. There were shortages of materials and adequately trained and committed personnel in the modern health institutions serving the community. An out

Beyond capacity limits: can social cohesion offset the impact of service constraints on youth mental health?

BACKGROUND: Rates of youth anxiety, depression, and self-harm have increased substantially in recent years. Expansion of clinical service capacity is constrained by workforce shortages and system fragmentation, and even substantial investment may not achieve the scale of growth required to address unmet need. Preventive strategies - such as strengthening social cohesion - are therefore essential to alleviate mounting pressures on the mental health system, yet their potential to compensate for these constraints remains unquantified. METHODS: This study employed a system dynamics model to explo

Delivery of health and social programs for temporary and precarious status (TAPS) migrants in Ontario, Canada: A systematic mapping review.

BACKGROUND: In recent years, Canada has accepted more temporary migrants to fulfill labour shortages and support economic productivity. These temporary and precarious status (TAPS) migrants, including temporary foreign workers, international students, refugee claimants, and non-status individuals, work in various sectors to fill gaps in the labour force. However, the precarity and uncertainty experienced by TAPS migrants create challenges around their access to health and social services. In response to shifting migration policies that may exacerbate these challenges, this study aims to identi

Evaluating Sepsis Bundle Compliance as a Predictor for Patient Outcomes at a Community Hospital: A Retrospective Study.

BACKGROUND: Clinicians are encouraged to use the Centers for Medicare & Medicaid Services early management bundle for severe sepsis and septic shock (SEP-1); however, it is unclear whether this process measure improves patient outcomes. PURPOSE: The purpose of this study was to evaluate whether compliance with the SEP-1 bundle is a predictor of hospital mortality, length of stay (LOS), and intensive care unit LOS at a suburban community hospital. METHODS: A retrospective observational study was conducted. RESULTS: A total of 577 patients were included in the analysis. Compliance with the SE

Staying Out of the Way: Perceptions of Digital Non-Emergency Medical Transportation Services, Barriers, and Access to Care Among Young Black Male Survivors of Firearm Violence.

With a reduction in primary barriers to healthcare access as a result of the Affordable Care Act, there is an increased need to address secondary barriers faced by low-income young Black male survivors of violent injury. While transportation is often characterized as a barrier for individuals with chronic disease and disability, it also acts as a significant barrier in accessing cognitive behavioral therapy and mentoring services through hospital-based violence intervention programs (HVIPs). These services address the traumatic stress associated with surviving gun violence. Although there are

A University and Community Partnership to Address Diaper Insecurity Among Infants in Suburban Communities Through Home Visiting Services.

CONTEXT: Despite overall affluence in a suburban county, diaper insecurity affects many families in low-income communities where poverty rates are 3 times the county average. Existing federal assistance programs do not cover diapers, and transportation barriers in this car-dependent suburb limit access to traditional diaper bank models. OBJECTIVE: To describe and evaluate a university-community-federally qualified health center (FQHC) partnership model for addressing diaper insecurity among Medicaid-eligible families with infants and toddlers through community health worker (CHW) home visitin

Buprenorphine provider preferences to expanding harm reduction services in rural and suburban primary care settings: a short report.

INTRODUCTION: The opioid overdose crisis continues to worsen in suburban and rural regions. Expanding harm reduction approaches in primary care offers a patient-centered approach to reduce opioid overdoses. METHODS: We conducted a semi-structured survey among 26 buprenorphine prescribers to assess acceptability of office-based harm reduction strategies for people who use opioids (PWUOs) and expanding partnerships with community stakeholders to ensure low-threshold access to buprenorphine. RESULTS: Respondents were mostly very or somewhat supportive of providing HIV testing (96.2%), informati

Jails, prisons, and the health of urban populations: a review of the impact of the correctional system on community health.

This review examined the interactions between the correctional system and the health of urban populations. Cities have more poor people, more people of color, and higher crime rates than suburban and rural areas; thus, urban populations are overrepresented in the nation's jails and prisons. As a result, US incarceration policies and programs have a disproportionate impact on urban communities, especially black and Latino ones. Health conditions that are overrepresented in incarcerated populations include substance abuse, human immunodeficiency virus (HIV) and other infectious diseases, perpetr

The impact of Shanghai's community nursing center policy on chronic wound care practice: a qualitative study.

OBJECT: This qualitative study explores how the community nursing center policy reshapes chronic wound care. It captures frontline nurses’ experiences to identify strengths and barriers, providing evidence for optimizing and scaling community nursing center models. BACKGROUND: Global aging and lifestyle changes have intensified the demand for chronic wound care, a service predominantly delivered in primary care settings. To strengthen community capacity, Shanghai has been establishing community nursing centers since 2023. While this policy represents a novel care model, its impact on nurses an

Service mix and financial performance in rural hospitals: A contingency theory perspective.

BACKGROUND: Rural hospitals are increasingly at risk of closure. Closure reduces the availability of hospital care in rural areas, resulting in a disparity in health between rural and urban citizens, and it has broader economic impacts on rural communities as rural hospitals are often large employers and are vital to recruiting new businesses to a community. To combat the risk of closure, rural hospitals have sought partnerships to bolster financial performance, which often results in a closure of services valuable to the community, such as obstetrics and certain diagnostic services, which are

Risk Behaviors Among Suburban Women who Use Methamphetamine: Social Harms and Social Solutions.

We propose that female methamphetamine users who live in suburbia experience risks for disease transmission stemming from their social environment that remain under the radar of public health surveillance networks. The data analyzed in this paper were collected from 2007 to 2011 and were drawn from two sequential studies on methamphetamine use. The studies were conducted in the suburbs of a southeastern United States metropolis. We analyzed a total of 65 qualitative interviews with former and active methamphetamine-using women. Data from focus groups also were included in the analysis. The par

A community survey of psychological impairment among Anglo- and Mexican Americans and its relationship to service utilization.

Discussions of Mexican American mental health need have been limited to analyses of service utilization statistics, most of which have shown Mexican Americans to use mental health services proportionally less than other racial/ethnic groups. Consequently, Mexican Americans have been viewed as a group having greater unmet mental health needs. Yet, these utilization-based needs assessments make an implicit and untested assumption: that the proportion of the psychologically impaired is comparable across racial/ethnic groups. The purpose of the present investigation was, first, to contribute to a

Introduction of cyclofem once-a-month injectable contraceptive in Mexico.

A large introductory study of Cyclofem, a once-a-month injectable contraceptive, was conducted in three Mexican provinces. A total of 3457 healthy women participated: 640 women from rural areas (community-based component) and 2817 women from urban and suburban areas (health center-based component). A total of 20,316 women-months of treatment experience were accumulated during a one year period. Cyclofem proved its use-effectiveness (pregnancy rate of 0.03%) and its safety under routine service conditions of family planning facilities in Mexico. The overall life table continuation rate at 1 yea

Prevalence of drugs and alcohol in autopsied homicide cases in St. John Parish, Louisiana.

This article presents a retrospective review of all autopsies conducted by the St. John Parish coroner's office between January 1992 and June 1995. The overall prevalence of illicit drugs or alcohol detectable in the autopsied homicide cases for that time period was 60%. Cocaine was the most prevalent substance, detected in 40% of homicide victims. This study also describes a preponderance of firearm violence among homicide cases, and an overrepresentation of young black men with detectable blood levels of drugs or alcohol at time of death. The data from this rural/suburban parish closely para
