Preventing Retained Surgical Items Through the Development of a Safer Telescopic Sponge-Counting System.

A retained surgical item (RSI) remains a serious patient safety risk in perioperative nursing, with surgical sponges representing the most common RSI. After an RSI event involving a sponge occurred at a large academic medical center, a root cause analysis identified the stacking of sponge bags as a contributing factor. The nurse innovator was tasked with finding a solution. A telescopic sponge-counting system (SCS) prototype, featuring expandable arms, allowed visualization of 120 sponges without stacking bags. A three-phase evaluation was conducted: a staff assessment of the standard SCS curr

Retained Surgical Sponge in the Pelvis From Magnetic Resonance Imaging-Guided Cervix Brachytherapy.

Magnetic resonance imaging-guided brachytherapy is an essential component of curative treatment in locally advanced cervical cancer. The use of interstitial needles improves local control rate for locally advanced cervical cancer compared to intracavitary brachytherapy alone. Bleeding is one of the most common complications from cervix interstitial brachytherapy, typically managed by pressure with surgical sponge/packing with or without a hemostatic agent. Herein, we present a case of stage IVA cervical cancer with retained surgical sponge in the pelvis from magnetic resonance imaging-guided i

Retained Surgical Sponge Expelled Through a Rectal Fistula With Spontaneous Passage Per Rectum.

Gossypiboma is a condition that occurs as a result of leaving sterile gauze, surgical sponges, or similar materials inadvertently retained in the body following surgery. The most common localization of gossypibomas is the intraabdominal cavity. Patients with gossypiboma can remain asymptomatic for years. Clinical manifestations and complications vary widely. Gossypibomas can result in significant complications based on the affected organ and its anatomical location. It is difficult to determine the exact incidence due to medicolegal issues. While prevention remains the most effective strategy

Gossypiboma: complete transmural migration of retained surgical sponge causing small bowel obstruction.

The authors report a case of recurrent intestinal obstruction due to complete transmural migration of a retained surgical sponge into the small intestine. A 41-year-old woman presented with recurrent episodes of intestinal obstruction following a previous laparotomy in a private hospital for some gynaecologic procedures. Relaparotomy revealed a mass within the ileum which was resected and, when dissected out, was found to be a completely intraluminal retained surgical sponge. Although rare in daily practice, a retained intraperitoneal foreign body should be remotely considered in patients who

Sudden suffocation by surgical sponge retained after a 23-year-old thoracic surgery.

A 70-year-old male patient with a 23-year-old history of right lower lung lobectomy for primary pulmonary adenocarcinoma (T1 N0 M0) presented with recurrent bronchopneumonia and purulent sputum. Pleural callus, lung abscess, bronchopleural fistula, and stitch granulomas were confirmed by chest x-ray, computed tomography scan, and bronchoscopy in the background of his complaints. An attempt to remove the bronchial purulent discharge and tissue sampling was made by using a flexible bronchoscope. The area of the lower trachea suddenly became clogged during bronchoscopic removal of the suspected p

Complete migration of retained surgical sponge into ileum without sign of open intestinal wall.

A 24-year-old woman came to the emergency room with a history of diffuse abdominal pain in the form of colic, nausea, vomiting and intestinal constipation. Clinical and ultrasound findings suggested intestinal obstruction due to foreign body. She had been submitted to a cesarean section 4 months previously at another hospital. At laparotomy, a ileum loop was found to be distended by an inside large and hardened mass with another intestinal loops and omentum density adherent. An ileotomy was performed on the compromised segment with terminating anastomosis. When opened surgical specimen it was

Progressive ossification due to retained surgical sponge after upper leg amputation: a case report.

INTRODUCTION: Numerous cases are described of patients in whom foreign objects were found after surgery. Foreign body granuloma caused by retained surgical sponge, also called gossypiboma, mostly occur in the abdominal cavity but very seldom in limbs. CASE PRESENTATION: A 29-year-old Caucasian man presented with asymmetrical walking pattern and progressive pain in his leg, which was severely injured and amputated seven years before. A firm swelling of soft tissue with calcifications was localized in the stump. Roentgenogram and MRI showed a retained surgical sponge with calcifications. Open s

Gossypiboma: A case of retained intra-luminal surgical sponge leading to delayed diagnosis and treatment in a Nigerian patient.

Gossypiboma, a term used to describe a retained foreign body mass of cotton (sponge, abdominal mop or gauze) within the body after a surgical procedure, is an uncommon but serious surgical complication. It can manifest with various clinical presentations and often leads to delayed diagnosis and significant morbidity. This report highlights the need for a repeat exploration at the end of open abdominal surgeries as routine. The case presented is that of a young female who underwent open myomectomy in an outreach setting, and subsequently developed symptoms of an acute abdomen due to a retained

Urinary Retention as the Only Symptom of Retained Surgical Sponge (Gossypiboma), 29 Years After Cesarean Procedure.

A 68 year old lady presented to urology department suffering from acute urinary retention. The U/S revealed hydronephrosis in left kidney and a mass at the left side of the small pelvis which pushed forward the uterus and the bladder. The CT scan confirmed a mixed (solid and cystic) mass, with diameter of 12 cm with interpretation of the left ovary. The patient underwent laparotomy and the mass was excised after a difficult dissection due to severe adhesion with the bowel. The well-capsulated mass was a retained surgical sponge. The patient had undergone cesarean procedure 29 years ago.

Local administration of gentamicin collagen sponge in surgical excision of sacrococcygeal pilonidal sinus disease: a systematic review and meta-analysis of the literature.

Surgical site infections occur in up to 24 % of patients after surgical excision of sacrococcygeal pilonidal sinus disease with primary wound closure. Local administration of antibiotics by a gentamicin collagen sponge could reduce this infection rate. The objective of this systematic review and meta-analysis was to evaluate the effect of a gentamicin collagen sponge on outcome after surgical excision in patients with sacrococcygeal pilonidal sinus disease. A structured literature search was performed in the PubMed, Embase, The Cochrane Library, and Scopus databases. Studies comparing surgical

Evaluating the Effectiveness of Gentamicin-Impregnated Collagen Sponge in the Prevention of Surgical Site Infection in Colorectal Surgery: A Systematic Literature Review and Meta-Analysis.

Surgical site infection (SSI) is a frequent postoperative complication, particularly in colorectal surgeries, which exhibit higher SSI rates than other procedures. The application of Gentamicin-Collagen Implant (GCI) at a local surgical site maximises not only the unique qualities of collagen in enhancing wound healing but also the antibacterial effects of gentamicin. The objective of this study is to determine if SSI is reduced by the use of GCI during colorectal procedures. Included studies were randomised controlled trials (RCTs) comparing GCI use with a defined control in colorectal surger
