Post-traumatic humero-ulnar synostosis.

A humero-ulnar synostosis is a bony connection between the humerus and the ulna. This is a very rare finding and it results in a serious disability of the elbow. Usually, a synostosis of the elbow occurs as a congenital anomaly. In this case, a 6-year-old girl was seen with a post-traumatic humero-ulnar synostosis, which has never been reported in the literature before. Surgical resection of the humero-ulnar synostosis was performed. Along with rapid intensive physical therapy, almost full recovery of function was achieved. The short-term result is very satisfactory, but the long-term results

Zygomatic dysmorphology in unicoronal synostosis.

INTRODUCTION: Unicoronal synostosis (UCS) imparts a fronto-facial deformity, the hallmark feature being orbital and forehead dysmorphology. The facial and malar regions also consistently display asymmetry, however, zygomatic structural characteristics have not been investigated. The purpose of this study is to objectively analyze the zygomata of UCS patients compared to normal controls. METHODS: Three dimensional-computed tomographic images and demographic information were obtained from normal control and UCS patients. Volumetric and morphometric analyses were performed and results statistica

Management of posttraumatic radioulnar synostosis.

Posttraumatic radioulnar synostosis is a rare complication following fracture of the forearm and elbow. Risk factors for synostosis are related to the initial injury and surgical management of the fracture. Typically, patients present with complete loss of active and passive forearm pronation and supination. Evidence of bridging heterotopic bone between the radius and ulna can be seen on plain radiographs. Although nonsurgical management is sufficient in some cases, surgical excision is typically required. The timing of surgical intervention remains controversial. However, early resection betw

Surgical treatment of metopic synostosis.

Metopic synostosis is characterized by keel-shaped forehead (trigonocephaly), prominent midline ridge of the forehead, bitemporal narrowing, bilateral retrusion of supraorbits, egg-shaped orbits, low nasal dorsum, and reduced volume of the anterior cranial fossa. The mainstay treatment is early surgical intervention before the age of 12 months, which usually consists of bifrontal craniotomy with bilateral recontouring, lateral advancement, and lateral displacement of the superior orbital rims. Here, we have developed a new simplified technique for surgical treatment of trigonocephaly. A total

Spondylocarpotarsal synostosis with epiphyseal dysplasia.

We report on an 11-year-old boy with thoracolumbar fusion, carpal synostosis, short stature, scoliosis, lordosis, defective dentition, and recurrent otitis media consistent with the diagnosis of spondylocarpotarsal synostosis syndrome. Unlike other documented cases, radiographs of our patient at 5(1/2) years of age show delay in ossification of many of the epiphyses in addition to delay in carpal ossification. Such delay may have contributed to the distinctly short stature of this boy. This report both confirms and extends the phenotype of this recently delineated syndrome.

Unicoronal synostosis. A surgical intervention.

Computer-assisted medical imaging technology provides useful tools for the in vivo study of congenital craniofacial deformities. Using these tools, a standardized operation for infants with nonsyndromal unicoronal synostosis has been shown to consistently normalize orbital dimensions and decrease exocranial base angulation. Normalization of endocranial base angulation is less consistent. The plasticity of the infant's cranial base after cranial vault surgery suggests a locus of the primary pathology at a site other than the cranial base.

Functional anatomy of cranial synostosis.

An understanding of how sutural growth processes relate to the totality of cranial growth is necessary to cure the cause and not just the symptoms of cranial synostosis. There is no direct genetic determination for the origin, growth, size, shape or maintenance of bones. Rather, phenotypic expression of skeletal tissues is regulated via genetic information encoded in the cells of functional matrices. The primary morphogenetic event in neurocranial growth is the volumetric expansion of the neural mass, which causes the surrounding neurocranial capsule to expand. Calvarial bones arise at widely

Development of secondary unilateral coronal suture synostosis with a sagittal suture synostosis in a nonsyndromic patient.

Multiple-suture synostosis is typically associated with syndromic craniosynostosis but has been occasionally reported in large series of nonsyndromic children. The diagnosis of multiple fused sutures usually occurs at the same time, but rarely has the chronological development of a secondary suture synostosis been noted. The development of secondary bicoronal suture synostosis requiring surgical intervention has only been reported, to date, after surgical intervention and is hypothesized to arise from a disruption of inhibitory factors from the dura. The disinhibition of these factors permits

Additional squamosal suture synostosis and segmented intracranial volume in patients with non-syndromic sagittal synostosis.

PURPOSE: To evaluate the incidence of squamosal suture synostosis (SQS) in children with non-syndromic sagittal synostosis and to evaluate whether the additional SQS affects the intracranial volume (ICV). METHODS: Thirty-four consecutive patients (23 boys) who had been operated by cranial vault remodelling because of sagittal synostosis were compared retrospectively from 3D-CT imaging data sets obtained from volumetric CT. The mean age of the patients at preoperative CT imaging was 0.48 (range 0.13-1.3) years. Mann-Whitney U test was used in the statistical analyses. RESULTS: Sagittal synost

Comprehensive craniometry for sagittal synostosis.

OBJECTIVE: Sagittal synostosis is the most common type of craniosynostosis, resulting in deformity with distinctive morphological characteristics. These include occipital narrowing, parietal narrowing, anteriorly shifted vertex with parietal depression, and exaggerated frontal bossing. The traditional cephalic index affords limited reliability in quantifying initial severity and correction. The purpose of this study was to conceptualize and evaluate a set of novel metrics based on optical surface scanning (OSS) technology used for cranial remolding orthosis (CRO) treatment. METHODS: The 25 mo

Late presenting bilateral squamosal synostosis.

Premature fusion of one or other of the minor sutures can subtly influence the shape of the human skull. Although infrequently reported or not clinically recognized, it can such contribute to a variety of craniofacial dysmorphisms. We herein report a case of late presenting, isolated bilateral synostosis of the squamosal suture dysmorphologies whose presentation mimics aspects of sagittal synostosis.

Adult post-traumatic radioulnar synostosis.

Post-traumatic radioulnar synostosis is a rare complication of forearm fracture. Resulting in loss of forearm axial rotation, it is functionally very disabling. The surgical indication, timing of operation, surgical technique, interest and type of adjuvant treatment are all issues with which physicians managing radioulnar synostosis must deal. No therapeutic consensus yet exists, but a wide variety of surgical techniques and adjuvant treatments are suggested. A literature review sought to identify risk factors for synostosis, with a view to prevention and determining a suitable therapeutic att

Endoscopic technique for coronal synostosis.

PURPOSE: This paper aims to present a 15-year experience treating coronal craniosynostosis with endoscopic-assisted techniques and postoperative cranial orthotic therapy. METHODS: A total of 100 patients with coronal craniosynostosis were treated between 1996 and 2010. There were 36 males and 64 females. A single 2-cm incision was made halfway between anterior fontanelle and the squamosal on the affected side. Using endoscopic-assisted visualization, a strip of bone was removed between the aforementioned points. Following surgery, all patients were placed in cranial orthoses to assist in the

Surgical correction of metopic synostosis.

BACKGROUND: Premature closure of the metopic suture results in deformation of the anterior portion of the calvarium, which can vary from mild to severe. In mild forms, there is only prominent ridging of the metopic suture; more severe forms result in a marked narrowing of the frontal and temporal regions that in turn affects the supraorbital rims and produces hypotelorism. METHODS: The authors retrospectively reviewed 39 consecutive cases of metopic synostosis treated over a 12-year period. RESULTS: The average age at referral was 5 months, with surgery performed at an average age of 7.5 mon

Spondylocarpotarsal synostosis with ocular findings.

We report on three sibs presenting with spondylocarpotarsal synostosis, short-trunk dwarfism of postnatal onset, scoliosis, unsegmented thoracic vertebrae with unilateral bar, and carpal bone fusion. Tarsal bone fusion and dental abnormalities were noted in some of them, indicating pleiotropy and intrafamilial variability. Lens opacities, rarefaction of retinal pigmentation, and narrowing of retinal vessels, detected in two patients, are findings that have not been described to date in this condition.

Unilateral lambdoid synostosis: morphological characteristics.

The accurate diagnosis of unilateral lambdoid synostosis versus deformational occipital plagiocephaly has remained a complex and controversial issue in the field of craniofacial surgery. Over the past 30 years, numerous studies have been published describing the treatment for "lambdoid synostosis," the diagnosis having been ascribed despite radiographically patent sutures and pathology specimens displaying fibrous, nonfused suture lines. Over the past 16 years, 204 patients with unilateral occipital plagiocephaly have been seen at our unit, only 2 of whom (approximately 1%) manifested the clin

Cubonavicular synostosis. A case report.

Tarsal coalition is an entity with a great number of reported variants and with many unsolved questions, from its physiopathology to its treatment. Because of its exceedingly low frequency, we report a case of cubonavicular coalition. The symptoms cleared with conventional analgesic therapy, and no other therapeutic measures were required.
