Surgical treatment of posttraumatic syringomyelia.

OBJECTIVE: The present study evaluates the effectiveness of 2 surgical procedures, shunting and untethering, for posttraumatic syringomyelia. METHODS: We retrospectively reviewed the medical charts of all surgical patients with posttraumatic syringomyelia in our department. Shunting was performed before 1997; after 1997, we used arachnoidolysis and untethering. RESULTS: Shunting was performed in 15 patients, and 19 patients underwent arachnoidolysis. Statistical analysis found that the 2 groups did not differ in age or initial clinical or radiological presentation. All patients suffered from

Syringomyelia presenting as shoulder instability.

Three middle-aged women presented with shoulder instability as the first sign of syringomyelia. The patients' symptoms were unilateral in each case. Two patients had posterior shoulder instability, and one patient had anterior instability. Two patients had undergone surgery to stabilize the shoulder before referral to our center. In both cases the surgery had been unsuccessful. Review of two of the patients' plain radiographs after referral demonstrated radiographic changes highly suggestive of neuropathic arthropathy. The diagnosis of syringomyelia was confirmed by myelography, computed tomog

Cordectomy for post-traumatic syringomyelia.

We performed cordectomy, a surgical technique that is infrequently used at present, for a patient with post-traumatic syringomyelia (following complete paraplegia of both lower limbs due to dislocation fracture of the 9th thoracic vertebra), yielding a favourable result. We recommend cordectomy as a surgical technique to which spinal surgeons should give utmost consideration for patients with post-traumatic syringomyelia demonstrating progressive symptoms assumed to be attributable to the syrinx and with an anatomically transected spinal cord of the mid-to-lower thoracic vertebral level.

Syringomyelia caused by cervical spondylosis.

Syringomyelia is generally associated with Chiari type malformations, spinal tumors, or spinal trauma. Cervical spondylosis is only rarely involved. We here present a case of a 64-year-old woman with severe radicular pain in the right arm and the syringomyelic syndrome. Lateral radiographs of the cervical spine demonstrated spondylotic change at the C4/5 and C6/7 levels, and instability at C4/5. Dynamic magnetic resonance (MR) imaging revealed the spinal cord to be compressed at C5 and C6 with the body in extension, and the syrinx extended from C2 to the Th3 level on sagittal images. It was re

Focal (segmental) dyshidrosis in syringomyelia.

The features or mechanisms of dyshidrosis have not been sufficiently clarified. Neither has the difference between hyperhidrosis and hypohidrosis. To clarify the features and mechanisms of dyshidrosis (hyperhidrosis and hypohidrosis) in syringomyelia, the clinical features focusing on hidrosis of 30 patients with syringomyelia and Chiari malformation located from a syringomyelia database were prospectively analysed. The patients were classified into three groups: eight patients (26.7%) had segmental hypohidrosis, 10 (33. 3%) had segmental hyperhidrosis, and 12 (40.0%) had normohidrosis. We fou

Cerebrospinal fluid drainage for syringomyelia.

Twenty-eight years of experience with syringomyelia of various causes in the syringomyelia clinic at the Midland Centre for Neurosurgery and Neurology has provided a database of 723 patients, mostly adults, with either hindbrain herniation, syringomyelia or both. Treatment of syringomyelia by drainage has never been the optimum primary treatment on the basis that the cavity is usually secondary to some other disturbance of the cerebrospinal fluid pathways. Over this period 73 patients had either syrinx drainage (56 syringopleural, 14 syringo-subarachnoid shunts) or other procedure such as myel

Post-traumatic syringomyelia, an update.

Post-traumatic and post-arachnoiditic syringomyelia is described in 31 patients from the Midland Centre for Neurosurgery and Neurology (MCNN). It is suggested that the mechanisms may be similar in the two groups and that treatment is best directed to disabling the likely filling mechanisms by opening up the cerebrospinal fluid (CSF) pathways and deliberately leaving the dura open to create an artificial meningocele. It is suggested that drainage on its own is inappropriate but that when drainage of the syrinx is chosen as an ancillary technique then syringopleural drainage may be the procedure

Somatosensory evoked potentials in syringomyelia.

The two types of upper limb somatosensory evoked potential abnormality observed in nine patients with syringomyelia were reduced amplitude or absent cervical potentials and an abnormal central conduction time. Although this pattern of abnormalities resembles that observed in other intrinsic spinal cord lesions, it differs from peripheral nerve diseases and cervical radiculopathy in which the central conduction time is normal.

Magnetic resonance imaging in syringomyelia.

The aim of the study was to assess the reliability of magnetic resonance imaging (MRI) in the detection of syrinx cavities and to determine the proportion of cavities associated with Chiari malformation. MRI was performed in over 120 patients with neurologic deficits referable to the cranio-cervical junction. Forty-one patients were also evaluated by CT myelography (CTM). CTM demonstrated appearances consistent with intramedullary cavitation in 14 patients. MRI detected 11 of these but in 3 the appearances, including cord size, were normal. In the entire study 27 syrinx cavities were demonstra

[Post-traumatic syringomyelia (author's transl)].

We have examined a clinical case concerning a patient who, five years after a spinal cord injury, showed a series of symptoms due to the presence of a intramedullary cyst according to neurologists' general opinion. They think this cavitation is a development of acute myelomalacic cores or of a central haemorrhage. The extension of the cavities is regarded as a result of trasmission of venous pressure, brought about by coughing, straining and sneezing, to the spinal cord and of tethering of the cord at the side of trauma by dense arachnoidal adhesions, hence its adaptability to the extremes of

Computerized axial tomography in syringomyelia.

The ACTA-scanner, a device for computerized axial tomography, permits cross-sectional radiographic study of the entire human body, including the spine. In the ACTA-scan, the spinal cord appears as a roundish formation surrounded by the less dense subarachnoidal cerebrospinal fluid. The spines of 18 patients with verified (nine cases) or suspected (nine cases) syringomyelia were studied by ACTA-scanning. In seven of the verified and in four of the nonverified cases, some evidence of cord cavitation was shown. The cystic part of a cervical-cord ependymoma was also demonstrated. ACTA-scanning is

Early Syringomyelia in Tubercular Meningitis.

Tubercular meningitis (TBM) can have various complications. Sometimes syringomyelia can also occur as a late complication of tubercular meningitis. Although syrinx formation in early stage of TBM is very rare. There are only four published case reports of syringomyelia in acute stage of TBM. Here we report a patient with tubercular meningitis who developed syringomyelia in early course of illness.

Syringomyelia in hereditary multiple exostosis.

We describe five children with Hereditary Multiple Exostosis (HME) who also had syringomyelia. Of these, four had a tethered cord/fibrolipoma. No spinal osteochondromas were found in these patients. All had antecedent neurological signs or symptoms that prompted spinal imaging with MRI. Of all patients with HME seen in the Midwest Regional Bone Dysplasia Clinic from 1982 to present, 44% (17/39) of patients had signs or symptoms concerning for possible cord-related neurological findings. However, only 10 of 39 had spinal imaging. Assuming that all individuals with syringomyelia were identified,

Post-traumatic syringomyelia: outcome predictors.

OBJECTIVE: To identify risk factors that predispose to post-traumatic syringomyelia (PTS) and describe the outcome of surgical management. METHODS: Retrospective cohort study of 27 patients with post-traumatic syringomyelia. Spinal cord injury of these patients spanned the period from 1963 to 2008. All data were collected retrospectively using available medical records and radiological images. RESULTS: There were 24 males and 3 females. The level of initial spine injury was thoracic in 21, cervical in 4, and lumbar in 2. The average age (±SD) at diagnosis of PTS was 40±13 years. The mean fol

Charcot's Elbow Following Syringomyelia: Revisited.

Neuropathic arthropathy of the elbow joint is uncommon with a reported incidence of 3-8%only. The natural history, pathogenesis and management approaches are debatable with an absence of clear guidelines. We report two cases of Charcot's elbow following syringomyelia with ulnar nerve manifestations. One patient required debulkingsynovectomy and ulnar nerve decompression while the other was managed conservatively. The article aims to enrich the limited knowledge of managing Charcot's elbow.
