A 25-year-old woman with daily episodes of syncope and several related traumatic injuries was referred for electrophysiological study. Structural heart disease was excluded. Electrophysiological study revealed inappropriate sinus tachycardia with heart rates up to 190 beats/min. A sinus node modulation was performed. However, 5 days later the patient again developed syncope during sinus tachycardia at rates of 140 beats/min with systolic blood pressure of 60 mmHg. The patient subsequently underwent AVN ablation and implantation of a dual chamber pacemaker. The patient has remained asymptomatic

A case of a 42-year-old woman who had combined sinus and atrioventricular nodal tachycardia is reported. Programmed atrial stimulation study showed the induction of the two types of paroxysmal supraventricular tachycardia. The mechanisms of these supraventricular tachycardias were both sinus nodal and atrioventricular re-entrant tachycardia. Sinus nodal re-entrant tachycardia could be induced only after injection of atropine. The tachycardias could no longer be induced after the administration of verapamil.

Sinus node re-entry (SNR) usually appears as a single beat. Tachycardias (SNRT) consistent with sustained SNR were seen in six patients and were initiated by premature stimulation of the high right atrium (six patients) and coronary sinus (four patients), and after continuous pacing from the high right atrium (four patients) or right ventricle (one patient) at rates of 130 to 200 per minute. During SNRT: (1) atrial beats exhibited a high-to-low atrial activation sequence, (2) the P-waves were similar in morphology to P-waves during sinus rhythm, and (3) re-entry in the A-V node or at the site

Sinus tachycardia (ST) is ubiquitous, but its presence outside of normal physiological triggers in otherwise healthy individuals remains a commonly encountered phenomenon in medical practice. In many cases, ST can be readily explained by a current medical condition that precipitates an increase in the sinus rate, but ST at rest without physiological triggers may also represent a spectrum of normal. In other cases, ST may not have an easily explainable cause but may represent serious underlying pathology and can be associated with intolerable symptoms. The classification of ST, consideration of

A 77-year-old woman (case 1) and a 57-year-old woman (case 2) with paroxysmal "sinus tachycardia" are reported in whom the tachycardia repeatedly occurred associated with respiration. In both cases, reentrant P' waves are almost the same in configuration as sinus P waves, and P'R intervals are also the same in length as PR intervals. In case 1, the tachycardia was initiated during inspiration, and was terminated during expiration; showing tachycardia-dependent initiation. On the contrary, in case 2, the tachycardia was initiated during expiration, and was terminated during inspiration; showing

Despite advancements in medical therapy, managing symptomatic inappropriate sinus tachycardia (IST) remains challenging. The role of catheter ablation in addressing this condition remains ambiguous according to multiple cardiac society guidelines. In this case study, we illustrate the efficacy of a hybrid approach involving sinus node modification and ablation in a patient with refractory symptoms, while also addressing the associated challenges and safety considerations of this procedure. A 58-year-old female patient was troubled with recurrent palpitations secondary to IST. Due to the proxim

Sinus tachycardia has been reported after radiofrequency catheter ablation of supraventricular tachycardia. Frequently, these patients require beta-blocking agents for symptomatic control. The purpose of this study was to evaluate prospectively the incidence of inappropriate sinus tachycardia and heart rate variability after ablation of atrioventricular nodal reentrant tachycardia and accessory pathways. Patients undergoing ablation had 24-h ambulatory monitoring ECG (Holter) performed before the procedure, on the day of the ablation, and 3 months afterwards. There were 170 patients, mean age

We have developed a tachycardia detection scheme for use in an antitachycardia pacemaker in which the use of a properly timed atrial extrastimulus provides a means of discriminating sinus tachycardia from pace-terminable 1:1 tachycardias. An atrial extrastimulus is delivered in late diastole (80 ms premature), and the ventricular response is monitored. In sinus tachycardia, the ventricular response is expected to appear early as well, but in pace-terminable tachycardias, such as AV reentrant and ventricular with VA conduction, the ventricular rhythm will be unperturbed. Testing of the algorith

This case report discusses the diagnosis and management of a 25-year-old pregnant patient presenting with persistent tachycardia. The patient, with a past medical history of thyroiditis, polycystic ovarian syndrome, and obesity, was admitted due to palpitations and was diagnosed with atrial tachycardia. Despite medical management with metoprolol, adenosine, digoxin, and flecainide, the tachycardia persisted, necessitating discussion about cardiac ablation. The report emphasizes that atrial tachycardia poses a significant clinical challenge when refractory to medical therapy. It also highlights

Paroxysmal phase 4 block is a type of atrioventricular block that occurs in patients with a diseased His-Purkinje system. It is usually triggered by a premature ventricular contraction or premature atrial contraction or slowing of a sinus rhythm. We report a case of phase 4 atrioventricular block triggered by a sinus tachycardia that slows down and discuss the management plan and the mechanism causing the block.

A 26-year-old man presented to our syncope service with debilitating daily palpitations, shortness of breath, presyncope and syncope following a severe viral respiratory illness 4 years previously. Mobitz type II block had previously been identified, leading to a permanent pacemaker and no further episodes of frank syncope. Transthoracic echocardiography, electophysiological study and repeated urine metanepherines were normal. His palpitations and presyncope were reproducible on deep inspiration, coughing, isometric hand exercise and passive leg raises. We demonstrated rapid increases in heart

Survivors of childhood cancer and hematopoietic stem cell transplantation are subject to a range of cardiac late effects including conduction abnormalities. We present, the case of a 12-year-old girl diagnosed with inappropriate sinus tachycardia after a history of multiply relapsed acute lymphoblastic leukemia and an exposure history including anthracycline-based chemotherapy followed by the total body irradiation in the context of a hematopoietic stem cell transplant. The clinical importance of this late cardiac effect along with the potential options for therapy is discussed.

A case of ventricular tachycardia (VT) was aggravated by carotid sinus massage in a woman with an old myocardial infarction. The patient presented with a wide QRS complex tachycardia with a rate of 143 beats/min, which fulfilled the classic and newer electrocardiographic criteria for the diagnosis of VT. Carotid sinus massage performed during the tachycardia resulted in its conversion to another, wide QRS complex tachycardia, with different morphology and a faster rate resembling ventricular flutter. This latter tachycardia was converted to sinus rhythm by a thump on the patient's chest. The i

Sinus node reentrant tachycardia is a relatively uncommon (5%-15%) form of recurrent paroxysmal supraventricular tachycardia (SVT). We describe a case of symptomatic sinus node reentrant tachycardia in a 67-year-old male with ischemic heart disease, congestive heart failure, and depressed ventricular function. Adenosine administered during an electrophysiology study caused prolongation of the tachycardia cycle length due to atrial cycle length prolongation (without atrio-His prolongation) prior to tachycardia termination. Right atrial mapping revealed the earliest site of atrial activation in

A 23-year-old male with a history of sinus tachycardia was treated with heart-rate feedback in eight sessions. From a pretreatment average of 103 beats per minute (BPM) as recorded in two separate baseline sessions, the heart rate dropped to 66 BPM in the last treatment session. At follow-up the heart rate seemed stabilized within the normal range, averaging 68 BPM after 1 month and 73 BPM after 12 months. Simultaneous EMG recordings of m. frontalis suggest a generalized effect of this treatment. Results are discussed in terms of somatic and respiratory mediation.
