A study of the side effects of phenothiazine drug treatment on 211 schizophrenic patients was carried out using the Simpson Scale. There were 21 cases of tardive dyskinesia, giving a prevalence of 10% which is low compared with other studies. Compared with the control patients without tardive dyskinesia, there was no significant relationship between tardive dyskinesia and variables like sex, age group, duration of drug treatment, ECT, benzhexol, and anticholinergic and other side effects, as reported in Western countries.

The continuing concern about tardive dyskinesia (TD) has stimulated a broad search for therapies for this disorder. Since neuroleptic drugs are thought to be the etiological agents, acting presumably through dopamine receptor blockade, nondopaminergic drugs have been the focus of recent study. However, no uniformly safe and effective drug treatment has been identified. Augmentation of cholinergic function is theoretically attractive, but further research is needed to develop practical and effective compounds. GABA drugs do not consistently suppress TD. The effect of benzodiazepines in TD is un…

We reviewed the medical records and videotapes of 100 patients with tardive dyskinesia (TD) referred to our movement disorders clinic to characterize the spectrum of hyperkinetic movement disorders caused by dopamine receptor blocking drugs (DRBD). Tardive stereotypy, present in 78 patients, was the most common type of TD, followed by tardive dystonia, akathisia, tremor, chorea, and myoclonus. Sixty-four had a combination of these hyperkinesias. In a second study, a "blind" review of videotapes of patients with a variety of movement disorders found that DRBD were the cause of stereotypic movem…

Tardive dyskinesia (TD) is characterized by abnormal and involuntary movements that generally occur after prolonged exposure to neuroleptic medications. In this article, we present the case of a 29-year-old man with schizophrenia who developed TD following treatment with haloperidol. Despite various attempts with benzodiazepines, amantadine, and anticholinergics, the dyskinesias persisted. However, after 2 years of treatment with olanzapine alone, a progressive improvement occurred, leading to the complete disappearance of the dyskinesias. We also provide a brief review of reported cases of an…

Tardive dyskinesia (TD) is a hyperkinetic movement disorder that may result from treatment with antipsychotics or other dopamine receptor blocking agents. Underlying pathophysiology is incompletely understood but since the 1970s dopamine depleting agents have been used to reduce involuntary movements. The search for safe, effective treatments for TD is ongoing. Valbenazine, a novel VMAT2 inhibitor, has recently been FDA approved for treatment of TD. Areas covered: An overview of TD, unmet medical needs and current treatment guidelines are presented. The background, chemistry and clinical devel…

Chronic intake of typical neuroleptics or centrally acting dopamine receptor blocking antiemetics may cause onset of tardive syndromes. Various types exist. One of them is tardive dyskinesia, characterised by often stigmatising, purposeless, rapid, repetitive, stereotypic, involuntary movements of face, limbs or trunk. Effective symptomatic drug treatment options beyond application of tetrabenazine are rare. Tetrabenazine is usually administered three times daily due to the short half life of this agent. Areas covered: This narrative review discusses the value of valbenazine for the treatment …

Valbenazine (NBI-98854, Ingrezza) is a highly selective vesicular monoamine transporter 2 (VMAT2) inhibitor that modulates dopamine release during nerve communication. The pharmacological target VMAT2 may play an important role in the treatment of tardive dyskinesia (TD), an iatrogenic condition associated with the administration of antipsychotic medication for long periods and characterized by rapid, repetitive, stereotypic, involuntary movements of the face and extremities. There are currently no Food and Drug Administration (FDA)-approved drugs for the treatment of TD. Results of clinical t…

Tardive Dyskinesia (TD) is a movement disorder associated with the clinical administration of antipsychotics. It is believed that TD is due, among other factors, to an increase in the oxidative damage produced by free radicals. Antioxidants, like vitamin E, have been used in the treatment of TD but there is no evidence of their effectiveness. Melatonin (MEL) is 6 to 10 times more effective, as an antioxidant, than vitamin E and it has been used with an apparent higher effectiveness in the treatment of TD, although the results have not been conclusive. A randomized, double blind, placebo contro…

This is a case report of a 24-year-old African lady, who developed symptoms of tardive dyskinesia (TD) following 9 months of treatment with aripiprazole 15 mg. She has no family history of mental illness, not used illicit substances and has a medical diagnosis of idiopathic hypertension which is well controlled. The patient has an 18-month history of paranoid schizophrenia with three psychotic episodes, one severe enough to warrant admission. Upon discontinuation of aripiprazole and switch to quetiapine, the symptoms of TD disappeared rapidly. A PubMed search revealed one previous case report …

INTRODUCTION: Neuroleptic induced tardive dyskinesia is a late appearing extrapyramidal disorder of involuntary, choreoatetoid movements. It may appear during chronic treatment with classical neuroleptics or a short time after its prolonged administration is interrupted. At present, there is no agreement on what would be the best way to treat dyskinesias. Clozapine is an alternative treatment to take into account, although the risk of agranulocytosis may be excessive to use it when there is a mild or moderate form of dyskinesia. Cases of improvement of dyskinesias both with olanzapine as well …

Currently, tardive dyskinesia (TD) remains an important clinical problem. The average prevalence is estimated at 30%. The appearance of antipsychotics has opened new paths. The extrapyramidal profile of these molecules is more favorable than that of conventional neuroleptics. In order to assess their prophylactic as well as curative potential, we reviewed the literature concerning four of these atypical antipsychotics: clozapine, risperidone olanzapine and amisulpride. Clozapine seems to induce fewer cases of TD than the conventional neuroleptics, and has a specific therapeutic effect. However…

Respiratory dyskinesia, a variant of tardive dyskinesia, may mimic chronic psychogenic hyperventilation syndrome, hence pseudopsychogenic hyperventilation. Respiratory alkalosis and sympathetic discharge may occur in both conditions. Neurological symptoms, dyspnea, chest pain, muscle spasms may also occur. Ventilation increases with stress and disappears with sleep in both conditions. However, respiratory dyskinesia has been seen in association with other choreiform movement disorders. Speech is interrupted by breathing and breathing is interrupted by grunts and groans. Respiratory dyskinesia …

Benzodiazepines have several advantages over other antidyskinetic drugs in treating tardive dyskinesia. The authors conducted a controlled study of clonazepam versus the active placebo of phenobarbital in 21 psychiatric patients with tardive dyskinesia. Both drugs significantly reduced dyskinetic movements: clonazepam had a stronger effect on orofacial dyskinesia, and phenobarbital was more effective for limbs and axial movements. Clonazepam was also more effective for drug-free patients and those receiving low doses of neuroleptics than for all patients given phenobarbital and for clonazepam …

Drugs that block dopaminergic transmission are currently widely used in the treatment of psychiatric diseases. One of the significant, common complications of therapy are tardive dyskinesias, which develop after prolonged, at least 3 months, therapy with antipsychotics and significantly reduce the quality of life of patients. Tardive dyskinesia is an extrapyramidal disorder, mainly manifested by involuntary hyperkinesis of the muscles of the face and tongue. These movements negatively affect the patient's daily activities and quality of life. This article reviews the currently available treatm…
