Tarsal coalitions in children are a group of disorders that typically present as a rigid flatfoot deformity. Operative treatment generally consists of resecting the coalition alone or resection plus flatfoot reconstructive procedures. The purpose of this study was to evaluate the rate and risk factors for complications, including infection, recurrence, and reoperation, following the surgical management of tarsal coalitions in children. This is a retrospective cohort study on the surgical outcomes of pediatric tarsal coalitions alone or in combination with adjunctive procedures. This study was

Tarsal coalitions are common, but fortunately the majority of patients with coalitions are asymptomatic and do not require intervention. When symptomatic, preoperative radiographs and CT scans are useful to characterize the type and extent of coalition. If a trial of nonsurgical management fails, resection, deformity correction, and triple arthrodesis may be considered. Barring contraindications, resection has been shown to be an effective and reliable first line surgical option. Arthrodesis should be reserved for cases of failed resection or significant arthritis. Associated deformity should

Symptomatic tarsal coalition is often considered to be synonymous with peroneal spastic flatfoot. The association of the cavovarus foot type with tarsal coalition is less well established and has been described only in children. This article describes a case of an adult female with symptomatic cavovarus feet with talocalcaneal coalition. The authors theorize about the pathology of muscle spasm and pain in patients with this condition.

The purpose of this case report is to illustrate how the literature can be used to guide clinical decisions related to a relatively uncommon pathological condition of the foot. This case report describes the approach used to examine and treat a 14-year-old boy referred by a physician for physical therapy with a diagnosis of peroneal spastic flatfoot (PSFF). Peroneal spastic flatfoot is a syndrome typically characterized by limited tarsal joint motion, a clonus response of the evertors, and a pes planus deformity. The patient reported having a limp for several years, but he said he was pain-fre

Coalition is a congenital fusion of two or more tarsal bones. Symptoms, if present, usually begin in early adolescence. The typical symptom complexes are aching pain in the rearfoot exacerbated by activity, progression of the flatfoot deformity, decreased tarsal motion, and sometimes peroneal spasm. When patients are asymptomatic, no treatment is necessary. If symptoms begin, the individual should be treated conservatively. If that fails, surgical intervention should be undertaken. Calcaneonavicular coalitions respond well to excision with extensor digitorum brevis interposition. Talocalcaneal

Tarsal-carpal coalition syndrome is a progressive condition involving synostosis of the wrist, ankle and digits. We describe a mother and her newborn that have this rare inherited condition where the diagnosis was made only after the baby's birth. The baby's condition was suspected on antenatal scanning, and he was born with reduced range of motion of his digits, elbows and ankles. The mother's condition has progressed to involve a fixed flexion deformity of her bilateral elbows, synostoses of her second to fifth digits and extensive coalition of her tarsal and carpal bones. She has required r

OBJECTIVE: To assess, utilizing MRI, tarsal tunnel disease in patients with talocalcaneal coalitions. To the best of our knowledge, this has only anecdotally been described before.
MATERIALS AND METHODS: Sixty-seven ankle MRIs with talocalcaneal coalition were retrospectively reviewed for disease of tendons and nerves of the tarsal tunnel. Interobserver variability in diagnosing tendon disease was performed in 30 of the 67 cases. Tarsal tunnel nerves were also evaluated in a control group of 20 consecutive ankle MRIs.
RESULTS: Entrapment of the flexor hallucis longus tendon (FHL) by osseous

The authors studied 12 adolescent patients with a known diagnosis of tarsal coalition. These patients were also active participants in athletics. Of the 12 patients studied, there were eight with calcaneonavicular bars and four with talocalcaneal bars. Even though nonoperative treatment was not successful, only eight of the 12 elected to have surgery. Following surgical resection of the tarsal coalition, five of six patients who had calcaneonavicular bar excisions as well as two patients with talocalcaneal bar excisions were able to return to competitive athletics. Nonoperative treatment of th

We describe a surgical technique and report outcomes for fibrin glue interposition after resection of a tarsal condition. An institutional review board--approved retrospective review of all pediatric patients with a tarsal coalition managed with resection was conducted between January 2002 and July 2010 by a single surgeon. All coalitions were resected with interposition of fibrin glue. Patients were evaluated for postoperative complications, pain, weight-bearing status, return to sports, and ankle and subtalar range of motion. Six feet without a coalition were used as a control group. Nine pa

Tarsal coalitions often present in young adults as a painful pes planovalgus hindfoot deformity. Resection of moderate and even large coalitions has become accepted as an alternative to arthrodesis. A review of the literature, however, suggests that coalitions with severe preoperative planovalgus malposition treated with resection are associated with continued disability and deformity. The authors believe that malposition contributes to persistent pain and disability after simple coalition resection. The hypothesis is that resection of the coalition with simultaneous hindfoot reconstruction ca

Tarsal coalition is abnormal fusion of two or more tarsal bones. The union may be fibrous, cartilaginous, or osseous and can be congenital or acquired in response to infection, articular disorders, trauma, or surgery. We report a case of fibrous talocalcaneal coalition in a 15-year-old boy in whom bone scintigraphy employing pinhole lateral views confirmed the clinical diagnosis when plain radiographs showed minimal changes and computed tomography was equivocal. The diagnosis of symptomatic tarsal coalition is important in that it is a common remediable cause of peroneal spastic flat foot, a f

A 34-year-old female had been suffering from pain in her left ankle for more than 6 years. Physical examination revealed no abnormalities other than a rigid subtalar joint. Using a CT scan and an MRI scan the diagnosis tarsal coalition was established. This condition is a fairly frequent cause of ankle pain, with a prevalence of 1-2%. However, it is easily missed by treating physicians. In this article the clinical and radiological pointers for establishing the diagnosis are described, and information about the treatment of tarsal coalitions is provided.

A review of charts and x-ray films of individuals who sustained ankle sprains was carried out to determine the presence of calcaneonavicular coalition. Coalitions were present in 63% of 215 patients with 223 ankle sprains. In 130 ankle sprains recorded as occurring directly in athletics, 65% of the x-ray films showed some degree of calcaneal navicular abnormality. This study found a much higher percentage of coalitions than anticipated in either the general population or the injured athletic population and reinforced the belief that there may be an underlying anatomic predisposition to ankle s

A 9-year follow-up of a case of acquired talonavicular coalition following avascular necrosis of the tarsal navicular is presented. A review of the literature suggests that this is the only reported case of acquired talonavicular coalition.

Tarsal coalitions are abnormal fibrous or bony connections between the tarsal bones of the foot. While not always symptomatic, coalitions can cause pain, alterations in forefoot and hindfoot morphology, and alterations in foot and ankle biomechanics. Previous research has described the association of tarsal coalitions with fractures of the lower extremity. Multiple reports of acute fracture in the presence of tarsal coalition have been presented, as have reports of stress fractures of the foot and ankle with concomitant coalition, insidious in onset and thought to be related to aberrancies in

Inasmuch as midfoot coalitions do not occur frequently, a preadolescent or early adolescent patient who has persistent complaints of painful flat foot with peroneal or anterior tibial muscle spasm warrants special evaluation. The patient may complain of becoming easily tired or report the onset of pain with increased activity. Clinical examination may reveal local tenderness over the area of the coalition, but this is not a consistent finding. Restricted inversion of the foot with spasm of the peroneals, forefoot abduction or pronation should alert the examining physician that a tarsal coaliti

We report a 19-year-old female with tarsal tunnel syndrome arising from a ganglion and a bony prominence from talocalcaneal coalition. However, in this case, tarsal tunnel syndrome was caused by ski boots compressing the tibial nerve within the tarsal tunnel. The patient was successfully treated by surgery. This is believed to be the first case with double causes (ganglion, tarsal coalition) evoked by trauma (skiing).
