Imaging of the temporal bone.

This review will focus on key recent advances in imaging of the temporal bone. The role of magnetic resonance imaging (MRI) in providing aetiological and prognostic information for patients with sudden sensorineural hearing loss will be discussed. Novel MRI sequences, such as delayed contrast-enhanced 3D fluid-attenuated inversion recovery (FLAIR) and their utility in the identification and grading of endolymphatic hydrops in Ménière's disease will be described. Furthermore, we will document the considerable advances in auditory implant technology (including active middle ear implants), and ho

Haemangioma of the temporal bone.

INTRODUCTION: Haemangiomas of the temporal bone are rare tumours and haemangiomas involving the middle ear are even rarer. The exceptional nature of these lesions makes their management particularly complicated. CASE REPORT: The authors report the case of a 16-year-old girl, who presented with an osteolytic lesion of the left petrous temporal bone that proved to be a haemangioma with extension to the middle ear, causing conductive hearing loss. DISCUSSION: Surgical biopsy is essential to establish the diagnosis of haemangioma because imaging alone only rarely provides a definitive diagnosis.

Tumors of the temporal bone.

Temporal bone tumors are rare entities. Histologically these pathologies represent heterogeneous group of diseases. The most common tumors of the temporal bone are vestibular schwannoma, tympanojugular paragangliomas and temporal bone carcinomas. Inner ear tumors, tumors of intratemporal segment of facial nerve and petrous bone tumors represent specific problematics. Clinical symptomatology reflects localization, extent of temporal bone invasion and involvement of individual temporal bone structures. Atypical symptomatology, dysfunction of the cranial nerves of the temporal bone and clustering

Imaging of the temporal bone.

A variety of congenital, infectious, inflammatory, vascular, and benign and malignant neoplastic pathology affects the temporal bone. Knowledge of normal temporal bone anatomy and space-specific differential diagnoses is key to imaging interpretation of temporal bone. Correlation with clinical history and physical examination is vital to making the correct diagnosis or providing an appropriate differential. Computed tomography and magnetic resonance imaging are complementary imaging modalities in the evaluation of temporal bone abnormalities.

[Trauma of the temporal bone].

Temporal bone fractures are mostly due to high-energy head trauma with high rates of concurrent intracranial and cervical spine injuries and belong to the wider spectrum of lateral skull base trauma. Given that the temporal bone represents the most complex bone structure in the human body, containing a multitude of vital neurovascular structures, variable clinical presentations may arise from such fractures, ranging from asymptomatic courses to serious consequences, such as conductive and/or sensorineural hearing loss, vascular and/or cranial nerve injury as well as cerebrospinal fluid leakage

Osteoblastoma of the temporal bone.

Benign osteoblastoma is an uncommon primary tumor of the bone. Any area of the skeleton may be affected by this tumor, but its occurrence in the temporal bone and middle ear is extremely rare. Clinical symptoms are nonspecific, even in the middle ear, and the diagnosis is often difficult in spite a complete physical and radiological examination. A biopsy is usually necessary for definitive diagnosis. Because of its potential for recurrence, local invasion, and, rarely, malignant transformation, a complete surgical excision remains the treatment of choice for osteoblastoma. We report a case of

Chondrosarcoma of the temporal bone.

Chondrosarcoma of the temporal bone is a rare disease, with only 36 cases of this tumor having been reported in the English language literature. We report the case of a 52-year-old woman with myxoid chondrosarcoma of the temporal bone whose only symptom was progressive hearing loss. The tumor was removed using aspiration, mastoidectomy and tympanotomy, with retention of the jugular bulb. No recurrence has been noted 45 months after the operation. Diagnosis was established from histological examination of the surgical specimens. The clinical characteristics and management of this rare lesion ar

[MRI of the temporal bone].

Lesions arising within the temporal bone, where audition and balance receptors are located, are multiple. Beginning with a short clinical and technical overview, this presentation aims to review the most common temporal bone lesions, according to their location. Tumors and malignant otitis externa are the most common lesions of the external auditory canal. MRI features of cholesteatoma redux, transtegmental masses, and paragangliomas are described in the middle ear MRI study. The diagnosis of petrous apex abnormalities is emphasized: cholesterol granuloma, malignant tumor, epidermoid cyst, cho

Imaging of the temporal bone.

High-resolution CT remains the imaging modality of choice for investigation of the temporal bone, especially if there is suspicion of bony involvement. The advances in two- and three-dimensional reconstruction are helpful for visualizing complex osseous structures. Nevertheless, the most exciting advances regarding imaging of the temporal bone were achieved in MR imaging. Improvement of surface-coil technology, application of gadopentetate dimeglumine, and development of new imaging sequences led to an increasing importance for MR imaging, especially in inflammatory diseases. Soft tissue struc

Imaging of the temporal bone.

Computed tomography remains the imaging modality of choice in investigating conductive hearing loss. The improvement of MR imaging surface-coil technology is allowing a number of inflammatory and neoplastic diseases in the temporal bone to be detected more effectively. These two modalities when used correctly will detect many of the soft tissue and bony abnormalities of the temporal bone.

Chondroblastoma of the temporal bone.

Chondroblastomas are highly destructive tumors that are derived from immature cartilage cells. The occurrence of this tumor in the temporal bone or skull base is uncommon. Approximately 70 cases have previously been reported, several of which have involved the temporomandibular joint (TMJ). We report here the case of a 67-year-old woman who presented with right-sided mixed hearing loss, a right external auditory canal mass, ear fullness, otalgia, blood-stained otorrhea, and pain around the TMJ, associated with difficulty in opening the mouth. CT and MRI revealed a mass involving the TMJ, infra

Chondrosarcoma of the temporal bone.

Eight patients with a chondrosarcoma of the temporal bone have been treated at the National Hospital for Neurology and Neurosurgery over a 16-year-period. Patients usually presented with symptoms and signs of lower cranial nerve palsies, though in most cases these resolved after surgery. This result, combined with the fact long-term survival can be achieved, makes surgical treatment of these tumours the best option, as the response to primary radiotherapy is uncertain. Surgical access is difficult, but an infratemporal approach is probably the most satisfactory. The use of post-operative adjuv

Adenomas of the temporal bone.

Three cases of primary adenomas of the petrous bone are presented and compared to an adenoma arising from the external ear canal. In contrast to other adenomatous tumors, these caused slowly progressive inner ear symptoms without other signs, hence making differentiation from other benign lesions difficult. In one case, association with von Hippel-Lindau disease is suspected because of a strong family history and recent discovery of a similar lesion in the opposite ear. Bone destruction was found radiographically and clinically with vascular tumor tissue and cysts permeating the bone, causing
