Nomenclature and classification of temporomandibular joint disorders.

Currently, there are basically two approaches to classification, one based on structural and one on positional changes occurring within the joint. Despite the increase in knowledge of pathologic changes occurring within the temporomandibular joint (TMJ), the disc still seems to be a central issue in nomenclature and classifications of TMJ disorders. Basic pathologies of the TMJ involve inflammation and degeneration in arthritic disorders (irrespective of the presence or position of the disc) and structural aberrations in growth disorders. Some internal derangements may occur independent of und

Orthodontics for treating temporomandibular joint (TMJ) disorders.

BACKGROUND: Temporomandibular disorders (TMD) relate to discomfort of the temporomandibular joint (TMJ). The disorder is multifactorial with a degree of psychogenic influence varying throughout an individual's life with phases of symptoms affecting the quality of life. In an attempt to treat this complex group of disorders many treatment modalities have been identified some of which are also considered in other Cochrane reviews. The disorder also has a normal cycle of events appearing to spontaneously improve without treatment. OBJECTIVES: To establish the effectiveness of orthodontic interve

Otalgia in patients with temporomandibular joint disorders.

Otalgia without organic causes is a common symptom in temporomandibular dysfunction (TMD) patients even though the etiology is controversial. Investigations of the influence of TMD treatment on otalgia are scarce. This follow-up study analyzed the clinical profile of TMD patients with otalgia and evaluated the treatment outcome. A total of 400 consecutive TMD patients (75% women) were divided in two groups: group 1 consisted of 233 patients (58%) with no complaint of ear symptoms and group 2 consisted of 167 patients (42%) with complaint of otalgia. The patients were examined with a standardiz

MRI characteristics of the asymptomatic temporomandibular joint in patients with unilateral temporomandibular joint disorder.

OBJECTIVE: To determine the association between unilateral temporomandibular joint disorder (TMD) and the presence of imaging abnormalities in the contralateral, asymptomatic joint. METHODS: MRI studies of 219 subjects with symptoms of unilateral TMD were examined for signs of disc displacement, osteoarthritis, disc deformation, and effusion in both temporomandibular joints (TMJ). The Chi-Square test and stepwise logistic regression analysis were performed. RESULTS: Disc displacement, osteoarthritis, disc deformation, and effusion were more common on the symptomatic side. However, in the cat

Temporomandibular joint disorders: artificial joint replacements and future research needs.

Temporomandibular disorders (TMD) affect a significant section of the American population. According to the National Institutes of Health (NIH), an estimated 3% to 5% of Americans suffer from temporomandibular disorders. Majority of TMDs can be treated by conservative methods, albeit surgical interventions are indicated for some pathological/clinical conditions (according to the American Association of Temporomandibular Joint Surgeons). A distinction can be made between the TMD and the TMJ disease/dysfunction. In the case of TMD, it most correctly relates to a neuromuscular type of problem in

Relationship between temporomandibular joint pain and magnetic resonance imaging findings in patients with temporomandibular joint disorders.

The purpose of this study was to evaluate abnormal magnetic resonance imaging (MRI) findings related to temporomandibular joint (TMJ) pain. This study included 245 joints of 152 patients with temporomandibular disorders with anterior disc displacement; of these, 129 joints had joint pain whereas 116 joints had no joint pain. MRI was used to evaluate the reduction of anterior disc displacement, joint effusion, mandible condylar morphology, bone marrow oedema of the mandibular condyle, and signal intensity of the posterior disc attachment (PDA) on fat-suppressed T2-weighted images. The odds rati

Clinical view of the temporomandibular joint disorder.

Temporomandibular pain has a musculoskeletal origin because it occurs as a consequence of masticatory muscle function disorder and temporomandibular joint disorder. Most common diagnoses of disorders are disc displacement and osteoarthritis, but their comorbidity can also occur. Pain is the most common symptom, where chronic temporomandibular pain may con- tribute to the occurrence of psychological disorders in the patient population. Splint is the most widespread dental method of treatment but other, noninvasive methods of musculoskeletal pain treatment are also recommended. Electronic axiogr

Pathogenesis and Differential Diagnosis of Temporomandibular Joint Disorders.

Temporomandibular disorders (TMDs) are an umbrella term including disorders of the temporomandibular joint and muscles of the masticatory system. They are the most common nonodontogenic cause of pain in the orofacial region. A clear understanding of various conditions, underlying mechanisms, clinical presentation, and examination skills is required to effectively diagnose and manage these patients.

Mischievous mandibular third molars camouflaging temporomandibular joint disorders.

OBJECTIVES: The aim of this study was to observe the relationship between impacted mandibular third molars and development of temporomandibular joint (TMJ) disorders. Knowledge of the factors that have an adverse effect on the TMJ is necessary for proper diagnosis, treatment, and prognosis of TMJ disorders. MATERIALS AND METHODS: The study was performed on 80 patients aged between 20 and 60 years with impacted mandibular third molars, over a period of two months. The patients were examined clinically and radiologically to determine the type of impaction and detect the associated TMJ symptoms

Asporin levels in patients with temporomandibular joint disorders.

BACKGROUND: Understanding the pathogenesis of temporomandibular joint disorder (TMD) is important for diagnosis and treatment planning. Thus, biochemical analysis is usually used for the detection of tissue damage. OBJECTIVE: In this study, we aimed to investigate the serum asporin levels in patients with TMD. METHODS: Our study was planned to be performed on 43 healthy individuals (control group) without any joint problems and 43 patients with temporomandibular joint internal derangement (TMJ-ID; patients group) according to the Wilkes classification (stages 3, 4 and 5). Serum asporin level

WITHDRAWN: Orthodontics for treating temporomandibular joint (TMJ) disorders.

The Cochrane Oral Health Group withdrew this review as of Issue 1, 2016. The review is out of date and does not meet current Cochrane methodological standards. It will be superseded by a new Cochrane review on Occlusal interventions for managing temporomandibular disorders. The editorial group responsible for this previously published document have withdrawn it from publication.

Synovial plicae and temporomandibular joint disorders: surgical findings.

PURPOSE: Synovial plicae and their relation to pain and disability have been reported in the orthopedic literature in association with the knee and other extremity joints. However, the occurrence of synovial plicae in the temporomandibular joint (TMJ) have rarely been reported. This report describes the surgical appearance, distribution, and histologic findings of synovial plicae in patients with TMJ recurrent dislocation and internal derangement. MATERIALS AND METHODS: Twenty consecutive patients, 16 with recurrent dislocation and 4 with internal derangement, who underwent open TMJ surgery b

Skeletal pattern in subjects with temporomandibular joint disorders.

Cell cultures are a valuable tool for the study of in vitro disease processes, especially where such processes concern wild and/or threatened animal species. However, the collection of adequate samples for cell line preparation can be challenging under field conditions due to logistical challenges and access to equipment. In this paper, we describe the generation of fibroblast cell lines derived from wing-punch biopsies of Australian eastern bent-winged bats (Miniopterus orianae oceanensis), expanding on and modifying existing protocols. Twenty wing-punch biopsies were collected from free-rang

A new surgical classification for temporomandibular joint disorders.

The role of temporomandibular joint (TMJ) surgery is ill-defined, so a universal classification is needed to collate the evidence required to justify the surgical interventions undertaken to treat TMJ disorders. The aim of this article is to introduce a new classification that divides TMJ disorders into 5 categories of escalating degrees of joint disease that can be applied to TMJ surgery. Using a category scale from 1 to 5, with category 1 being normal, and category 5 referring to catastrophic changes to the joint, the new classification will provide the basis for enhanced quantitative and de

Craniofacial pain syndromes that mimic temporomandibular joint disorders.

Undiagnosed pain in the face, head and neck is very frustrating to all parties involved: the patient, the family of the patient, and the clinician. Simple pains may be treated with ease; however, chronic pain is a different matter. Often, patients consult numerous doctors, clinics, and hospitals in their search of pain relief. The patient may then undergo extensive and at times, unnecessary invasive treatment with the hope that the relentless agony of their suffering will be relieved. The purpose of this article is primarily one of education. Physicians and dentists alike encounter these unfor

IncobotulinumtoxinA Injection for Temporomandibular Joint Disorder.

OBJECTIVES: Temporomandibular disorder (TMD) involves dysfunction of the temporomandibular joint and associated muscles of mastication causing pain with chewing, limitation of jaw movement, and pain. While the exact pathophysiology of TMD is not completely understood, it is thought that hyperfunction of the muscles of mastication places stress on the temporomandibular joint, leading to degeneration of the joint and associated symptoms. We hypothesize that chemodenervation of the muscles of mastication with IncobotulinumtoxinA (Xeomin) will decrease the stress on the temporomandibular joint and
