Therapeutic conclusions are derived from the anatomical peculiarities of the extensor apparatus of the fingers and the extensor tendons of the metacarpus and the wrist. In open wounds primary sutures are universally possible and promising except in cases with tendon defects or with special risk of infection. Our preferred technique employs a central wire suture which is barbed to resist proximal traction, combined with fine peripheral sutures. In closed ruptures at the DIP-level conservative as well as operative treatment can be advised, whereas at the PIP-joint and in more proximal regions op

Tendon injuries in the child's hand can be repaired primarily with better results than in the adult; this is true at all levels, including the digital sheath. In less than ideal circumstances, secondary conventional tendon grafting also provides predictable satisfactory results and can also salvage a failed primary repair. In children under the age of six years with digital flexor tendon repairs, the limb must be immobilized above the elbow. In the cooperative child over the age of six years with a primary repair of a lacerated flexor tendon, the dorsal splint with rubber band traction as desc

Although the Achilles tendon is a highly robust tendon, it is susceptible to ruptures and overuse. To date, there are no definitive or conclusive methods of management for the various pathologic conditions that can adversely affect the tendon. Endoscopic techniques have gained popularity in recent years because of their minimally invasive nature, improved cosmetic outcomes, relatively rapid recovery, and low morbidity. Several endoscopic methods have been developed to treat the various injuries and pathologic conditions that can affect the Achilles tendon. The outcomes of such measures have be

Extensor tendon injuries of the thumb include lesions of the tendons of the extensor pollicis longus, extensor pollicis brevis and abductor pollicis longus muscles. The latter is practically only affected in open injuries. Open injuries require a tendon reconstruction by suture followed by immobilization in the distal and an adequate aftercare depending on the zone of injury. In distal injuries static splinting is applied, whereas proximal injuries from T4 on require a dynamic after-treatment. Different courses of the tendon of the extensor pollicis brevis muscle exist distal to the metacarpop

OBJECTIVE: The primary purpose of this study was to investigate the overall accuracy of bedside extremity tendon ultrasound performed by emergency physicians in the emergency department. We also sought to investigate whether or not bedside tendon ultrasonography can be used to expedite the diagnosis and discharge planning in patients with suspected tendon injuries.
METHODS: This was a prospective study conducted at 2 academic level 1 trauma centers. Thirty-four patients were enrolled and underwent a comprehensive physical examination of the injured extremity, followed by a bedside ultrasound

Injuries to the flexor and extensor tendons in the hand are common and require cautious evaluation and treatment or repair. Flexor tendon injuries are often associated with neurovascular injury and thus must be considered in the differential workup. Extensor injuries may involve intra-articular damage, which must also be identified. The diagnosis and management of common flexor and extensor tendon injuries of the hand are described.

The long head of the biceps tendon is a known pain generator of the shoulder. There are numerous pathologic entities that may affect this tendon, including tendonitis, partial tearing, and subluxation. These conditions are often associated with rotator cuff tears, especially those involving the subscapularis. Operative interventions include tenotomy and tenodesis. Tenodesis can be preformed in a proximal or distal location. Subpectoral tenodesis may have a lower recurrence rate than proximal-based techniques.

The treatment of extensor tendon injuries of the hand is based on Verdan's division of the different sections into eight zones. Taking the anatomic characteristics of those zones into consideration, good functional results can be achieved in most cases by differentiating between indications for operative or conservative treatment. The most frequent extensor tendon injury of the hand is the closed tendon rupture over the DIP joint. In almost all cases it can be completely cured by immobilization for at least 8 weeks. Open extensor tendon injuries on the other hand require surgical therapy. This

The treatment of tendon injury in combined complex injuries to the hand is dictated by the presence of concomitant injuries. Early range of motion is desirable. To achieve this, fractures must be stabilized and the soft tissue envelope and vascular integrity maintained or reconstituted. In those instances in which these conditions cannot be met, the surgeon and patient should be prepared for secondary surgeries, including reconstruction or tenolysis. Although nerve integrity is not necessary for early functional success following tenorrhaphy, nerve injuries should be repaired or grafted primar

Until recently, extensor tendon injuries were often discounted as an important hand problem. However, studies have shown that not all extensor lacerations fare well and that loss of flexion can be problematic. Newer postoperative protocols emphasizing tendon gliding have improved results, and better repair techniques and postoperative rehabilitation regimens are under investigation. This article reviews the evaluation of acute open and closed extensor tendon injuries, their conservative and surgical treatment, and postoperative rehabilitation options.

Extensor tendon injuries are often initially evaluated and diagnosed in the Emergency Department. These injuries may be painfully obvious or frightfully subtle, but if the emergency physician has a firm understanding of the anatomy involved and he or she embarks on a careful and complete history and physical examination, the diagnosis should be made. There are various considerations in the ultimate treatment and outcome for the patient, and the patient's best interests are served when the emergency physician provides the initial appropriate treatment and proper disposition.

Cases of subcutaneous tendon ruptures of the hand were compiled over a period of 10 years; the lesions were all traumatic rather than due to degenerative illness. The 914 injuries fall into two categories: 867 extensor and 47 flexor tendon ruptures. The localisation of the lesions is most often distal. The extensor tendon lesion is accompanied more often by a ruptured bony fragment; the distal flexor tendon is mostly torn-off bony fragment. The extensor tendon ruptures can be subdivided as follows: distal injuries of the extensor aponeurosis in the DIP joint (751 cases), IP joint (31), tractus

Experience will show that proper management during the first 3 postoperative weeks will be the major determinant for the final result in extensor tendon injuries. In the simple injury, meticulous care in properly immobilizing each joint, combined with edema control, allows the patient to begin the mobilization phase with few problems, and a simple course of therapy can be anticipated. In the complex injury, early passive motion techniques will reduce the devastating complications of an increased fibroblastic response common to crushing injuries or combined lesions of the extensor tendons.
