[Birth asphyxia in term newborns: Diagnosis, prognosis, neuroprotection].

Birth asphyxia occurs in 0.5% of term deliveries. Prognosis of newborns with birth asphyxia depends on clinical features of neonatal encephalopathy. The outcome of infants without encephalopathy is excellent. In contrast, neurological outcome of infants with encephalopathy is poor: 40 to 100% of neurodevelopmental disabilities according to the grade of encephalopathy. Prognosis can be more accurately assessed by EEG and MRI. Infants with encephalopathy following birth asphyxia must be referred as soon as possible in centers where neuroprotection by hypothermia is available.

Determinants of small for gestational age birth at term.

BACKGROUND: Being born small for gestational age (SGA) is an indicator of intrauterine growth restriction (IUGR) and later health risks. This study investigated determinants of severe and moderate SGA (respectively, birthweight <3rd percentile and 3rd to <10th percentile for gestational age and sex). METHODS: A total of 2195 term pregnancies from a prospective cohort were studied. Prenatal data arose from maternal interview at 10-22 weeks of gestation and perinatal data were collected from hospital charts. Severe and moderate SGA were classified by Canadian population standards. Risk factors

Endocrine and paracrine regulation of birth at term and preterm.

We have examined factors concerned with the maintenance of uterine quiescence during pregnancy and the onset of uterine activity at term in an animal model, the sheep, and in primate species. We suggest that in both species the fetus exerts a critical role in the processes leading to birth, and that activation of the fetal hypothalamic-pituitary-adrenal axis is a central mechanism by which the fetal influence on gestation length is exerted. Increased cortisol output from the fetal adrenal gland is a common characteristic across animal species. In primates, there is, in addition, increased outp

The epidemiology of FIRS in term and late preterm births.

The definition of FIRS requires systemic inflammation and elevated levels of IL-6 in fetal plasma. That definition does not specify how systemic inflammation is to be recognized, and perinatal measurement of IL-6 is not a standard procedure. FIRS has not been examined in a population-based study that included post-neonatal outcome so its incidence and natural history are not known. The overlap, and similarities and differences, of FIRS as compared with other causes of neonatal encephalopathy, and how these relate to findings in the placenta, have not been jointly examined in a generalizable sa

The association between periodontitis and pre-term birth and/or low birth weight: a literature review.

Infections have been strongly associated with adverse pregnancy outcomes like pre-term and/or low birth weight (PTLBW). There is substantial evidence on the direct association of genito-urinary infections and the incidence of PTLBW. Numerous cases of adverse pregnancy outcomes without maternal genito-urinary infection but nevertheless high levels of the tumour necrosis factor alpha (TNF-alpha) and prostaglandin E2 (PGE2) in the amniotic fluid, have been recorded. These findings alluded to the presence of infection elsewhere in the body. This paper reviews the literature on the association betw

Physical and social predictors for pre-term births and low birth weight infants in Taiwan.

The purpose of this study was to examine the risk factors associated with pre-term labor (PTL) (< 37 gestational weeks) and low birth weight (LBW) (< 2500 gm) infants in a healthy Taiwanese population. From December 1998 through June 1999, a total of 633 healthy pregnant women were recruited at three teaching hospitals in Taipei. Using a prospective study design, the pregnancy outcome information was followed up by telephone or from medical records during the first month postpartum. Data were statistically analyzed by multiple logistic regression. The prevalence of premature births was 5.4%, a

Post-term birth: risk factors and outcomes in a 10-year cohort of Norwegian births.

OBJECTIVE: To identify factors associated with post-term birth and factors associated with adverse outcomes in post-term births. METHODS: The sample was a 10-year cohort (1978-1987) of term (n = 379,445) and post-term (n = 65,796) births from the Medical Birth Registry of Norway. Gestational age was based on mothers' recall of the last menstrual period. Multivariate analyses included proportional hazards regression and binomial logistic regression. RESULTS: After controlling for covariates, there was only a slightly increased risk of perinatal mortality in post-term as compared with term bir

Associations between heat wave during pregnancy and term birth weight outcomes: The PARIS birth cohort.

BACKGROUND: Climate change will make extreme weather events more frequent in the 21st century. Extreme ambient temperatures during the prenatal period have been associated with adverse pregnancy outcomes such as preterm birth. It is unclear, however, whether heat waves during pregnancy impact fetal growth in apparently healthy term newborns. OBJECTIVES: We aimed to investigate associations between heat wave during pregnancy and birth weight outcomes in term newborns from the PARIS birth cohort, and to explore meteorological conditions and air pollution as possible intermediate factors. Comparison of placental pathology in preterm, late-preterm, near-term, and term births.

OBJECTIVE: The purpose of this study was to determine whether placental pathologic condition supports the recent suggestion of subcategorizing preterm and term births into smaller gestational age subgroups with different perinatal mortality and morbidity rates. STUDY DESIGN: Twenty-seven clinical and 43 placental phenotypes were retrospectively compared in 4617 third-trimester births: 1332 preterm pregnancies (28-33 weeks' gestation), 1066 late preterm pregnancies (34-36 weeks' gestation), 940 near-term pregnancies (37-38 weeks' gestation), and 1279 term pregnancies (≥39 weeks' gestation). The association between birth weight at term and long-term endocrine morbidity of the offspring.

OBJECTIVE: To investigate whether small-for-gestational-age (SGA) and large-for-gestational-age (LGA) birth weight at-term poses an increased risk for long-term pediatric endocrine morbidity. STUDY DESIGN: A retrospective population-based cohort study compared the incidence of long-term pediatric hospitalizations due to endocrine morbidity of singleton children born SGA, appropriate-for-gestational-age (AGA), and LGA at-term. A multivariate generalized estimating equation (GEE) logistic regression model analysis was used to control for confounders. RESULTS: During the study period, 235,614 d

Long-term alterations of cerebellar structure after premature birth.

BACKGROUND: Alterations of cerebellar volume were reported in prematurely-born children, adolescents and adults, suggesting long-lasting effects on cerebellar growth. However, studies mostly examined global cerebellar volumes, which could disguise focal, subregional differences. METHODS: Based on T1-/T2-weighted MRI, this voxel-based morphometry study used the SUIT toolbox for optimized analysis of cerebellar and brainstem gray matter (GM) and white matter (WM) volumes in young adults born very preterm (<32 gestational weeks) and/or with very low birth weight (<1500g; N=101, M=26.7 years) and

Long-Term Maternal Mental Health after Spontaneous Preterm Birth.

OBJECTIVE: The aim of this study is to investigate whether a history of spontaneous preterm birth (SPTB) is associated with maternal depressive and anxiety symptoms, or psychosocial distress in the fifth decade of life. STUDY DESIGN: This is a secondary analysis of the PreCaris-study, a prospective observational study in which we included 350 women with a history of SPTB between 220/7 and 366/7 weeks of gestation and compared them to 115 women who had a term birth. Primary outcomes were the Depression and Anxiety scores measured using the Hospital Anxiety Depression Scale and Psychosocial dis

Birth trauma--risk factors and short-term neonatal outcome.

OBJECTIVE: The ability to predict birth trauma (BT) based on the currently recognized risk factors is limited and there is little information regarding the short-term neonatal outcome following BT. We aimed to identify risk factors for BT and to evaluate the effect of BT on short-term neonatal outcome. METHODS: A retrospective, cohort, case-control study of all cases of BT in a single tertiary center (1986-2009). The control group included the two subsequent full-term singleton neonates who did not experienced BT. Short-term neonatal outcome was compared between the groups including Apgar sco

Timing of indicated late-preterm and early-term birth.

The growing public health awareness of prematurity and its complications has prompted careful evaluation of the timing of deliveries by clinicians and hospitals. Preterm birth is associated with significant morbidity and mortality, and affects more than half a million births in the United States each year. In some situations, however, a late-preterm or early-term birth is the optimal outcome for the mother, child, or both owing to conditions that can result in worse outcomes if pregnancy is allowed to continue. These conditions may be categorized as placental, maternal, or fetal, including con
