Terminal heart failure care for women: better or more of the same?

BACKGROUND: Women with chronic heart failure (CHF) experience an increased symptom burden, activity impairment and frailty. Although receiving palliative care services has been shown to improve the quality of life of people with CHF, few patients access this in a timely manner. AIM: Explore whether there were differences in either referral to specialist palliative care or provision of elements of palliative care to women and men with CHF during their terminal admission in Australia. METHODS: This is a retrospective review of medical records for all admissions resulting in death from chronic

[Leadership in the promotion of inter-professional collaboration in terminal cancer care].

Home care requires indispensable inter-professional/inter-organizational collaboration. However, due to differences in specialization, the language used, and work structure, medical professionals may be confronted with a gap between the ideal and reality of collaboration. In the present case, a 50-year-old female with terminal cancer wished to receive home care and was consequently transferred to her home. The patient has been receiving a 24-h continuous drip infusion via a central venous route and using a patient-controlled analgesia pump for pain management. She has chronic respiratory failu

The "Palliative Care Quality of Life Instrument (PQLI)" in terminal cancer patients.

BackgroundThis paper describes the development of a new quality of life instrument in advanced cancer patients receiving palliative care.MethodsThe Palliative Care Quality of Life Instrument incorporates six multi-item and one single-item scale. The questionnaire was completed at baseline and one-week after. The final sample consisted of 120 patients.ResultsThe average time required to complete the questionnaire, in both time points, was approximately 8 minutes. All multi-item scales met the minimal standards for reliability (Cronbach's alpha coefficient > or =.70) either before or during pall

Beyond the Sound: Evidence-Based Nursing Care for Patients With Terminal Secretions.

Terminal secretions, commonly referred to as the "death rattle," are oropharyngeal secretions that accumulate in patients nearing the end of life who cannot clear them. Although physiologically benign, these secretions can cause emotional distress to families, nurses, and care providers. They are presumed to impact the quality of death, relating to comfort and dignity. This document reviews interventions for managing terminal secretions, emphasizing evidence-based practices. It explores pharmacologic methods, like anticholinergic medications-though little evidence supports their benefit-sugges

Wernicke encephalopathy observed in a cancer patient receiving terminal-stage home care.

We report an 80 year old man in the terminal stage of cancer receiving home care. On the 18th day after starting home care, he experienced a sudden onset of impaired consciousness, unsteadiness of gait, and ophthalmoplegia. Based on the clinical symptoms and a prolonged loss of appetite, Wernicke encephalopathy (WE) was suspected. Thiamine 100 mg was administered intravenously, and 20 minutes later, the patient responded to verbal stimuli. After 1 h, he was able to engage in conversation. When impaired consciousness occurs in cancer patients receiving home care, it is important to consider WE

Silent wounds: violence and adverse experiences in parents caring for terminally ill children in palliative care.

BACKGROUND: Primary caregivers of paediatric palliative care patients-mostly women-are disproportionately exposed to adverse events and violence, both directly related to the experience of disease and to other stressful life events. These experiences have been associated with a higher risk of psychopathology, which in turn impacts the children under their care. The purpose of this study was to describe the experiences of violence among primary caregivers of palliative care patients in a children's hospital in Mexico City and to analyse how these experiences affect their caregiving capacity. End-of-life issues in caring for patients with dementia: the case for palliative care in management of terminal dementia.

The number of people suffering with dementia is increasing in the general population and the trend is projected to continue as people live longer, especially in countries with developed economies. The most common cause of dementia (among the many other causes) is Alzheimer's dementia, which is considered a terminal illness. The disease could eventually lead to death, or death could occur as a consequence of co-morbid physical complications. The problem of end of life (EOL) care for patients suffering from dementia though spoken of and written about, does not get the attention and system suppor

Terminal HIV disease and extreme poverty: a review of 307 home care files.

This retrospective study of 307 patients with advanced HIV disease residing in downtown Montreal was carried out to examine the nature and consequences of the services they received according to their degree of poverty. The findings showed that, between 1 April 1991 and 31 March 1997, patients living in extreme poverty were more likely to complain of uncontrolled pain during the final week of home visits and were more likely to die in hospital than were their more financially secure counterparts. This held true even though the poorer group had a similar clinical profile at diagnosis and were p

Healthcare cost of palliative care for patients with terminal illness: A meta-analysis.

BACKGROUND: Palliative care for older people and individuals with terminal illnesses is currently under debate. Its impact on healthcare costs and potential savings during the disease course remains uncertain. OBJECTIVE: To systematically review the contribution of palliative care, compared with standard care, to savings in healthcare costs, and to explore the stages of end-of-life palliative care that may result in cost savings. INFORMATION SOURCES: Systematic searches were conducted in the PubMed, Medline, Embase, CINAHL, and CENTRAL databases for English-language cohort studies and random

Family transition through the termination of private duty home care nursing.

Health care professionals prepare families for discharge and for home care, but termination of home care nursing has been approached less methodically. The purpose of this retrospective study was to describe the experience of transition occurring with termination of private duty home nursing care for the families of technology-dependent children. The research question guiding this proposal was: How do families experience the transition from continuous home nursing care through termination of that care? This article will discuss the adjustments and preparations required by termination, families

The quality of care and symptom control--the effects on the terminal phase of ALS/MND.

The quality of care in the terminal phase of ALS/MND depends critically on the palliative care provided throughout the disease process. A retrospective review of 52 patients shows that patients with multiple symptoms and care needs can be cared for, and die, at home. To ensure that the care of patients is co-ordinated and allows patients and family to be cared for where they wish, a team approach is required. A co-ordinated team approach, involving health care, social services and voluntary groups, is described.
