[Daily problems involving contact with terminally ill patients with renal failure].

Practitioners and physicians working in emergency rooms are often confronted with dialysis patients or patients who have received a kidney transplant. For dialysis patients, the mode of dialysis treatment needs to be assessed and dialysis access should be secured. Furthermore, the indications for the next dialysis treatment need to be determined. Dialysis patients often present themselves because of fluid overload, hypo- or hypertensive episodes, electrolyte disturbances, fever or cardiovascular events. Patients undergoing continuous peritoneal dialysis are at an increased risk of infection of

Sedation for terminally ill patients with cancer with uncontrollable physical distress.

BACKGROUND: Relief of distressful symptoms in terminally ill patients with cancer is of prime importance. Use of sedation to accomplish this has been the focus of recent medical studies in countries other than Japan. We investigated the influence on consciousness of sedative drugs in a Japanese hospice. DESIGN AND SUBJECTS: We defined sedation as medical procedure to decrease level of consciousness in order to relieve severe physical distress refractory to standard interventions. We excluded increases in doses of morphine or other analgesic drugs resulting in secondary somnolence from the pre

Determinants of the sensation of thirst in terminally ill cancer patients.

While a sensation of thirst causes severe distress for a certain proportion of cancer patients in the terminal stage, the factors contributing to this symptom have not been established. To clarify the association between sensation of thirst and medical factors, especially dehydration, a cross-sectional observational study was performed on terminally ill cancer patients receiving inpatient hospice care. On admission to a palliative care unit, 88 consecutive patients underwent blood sampling and were requested to rate the intensity of thirst on a visual analogue scale (VAS). Physicians prospecti

Assessing and managing delirium in terminally ill adults in the community.

Delirium is a complex and serious neuropsychiatric syndrome that causes an acute disturbance in attention, awareness and cognition. It is common among people with a terminal illness, especially as the end of life approaches. Delirium can cause distressing symptoms, significant carer burden, acute hospital admission and increased mortality. Identifying delirium early is important because it increases the person's chance of recovery and enables better management in line with their preferences. This article discusses how to assess and manage delirium in terminally ill people cared for in communit

Impact of Artificial Intelligence on the Care of Terminally Ill Patients.

Introduction: In recent decades, demographic aging has led to an inversion of the population pyramid, with a marked increase in the proportion of older adults. This shift has been accompanied by a higher prevalence of chronic and life-limiting diseases, while there have also been significant technical and scientific advances. However, these developments have not been matched by a proportional expansion of healthcare human resources, including in palliative care (PC). Consequently, healthcare systems face increasing pressure, particularly in the provision of end-of-life care. Artificial intelli

Mechanical Thrombectomy for Trousseau Syndrome in a Terminally Ill Cancer Patient.

Trousseau syndrome was first described by Armand Trousseau in 1865 and is characterized by hypercoagulation resulting from malignant tumors. This complication can markedly impact quality of life (QOL). This is the first report of a terminally ill patient who developed large-vessel occlusion stroke from Trousseau syndrome and underwent mechanical thrombectomy. A 75-year-old woman presented with Stage IV ovarian cancer. Goals of care were transitioned to palliative care. The patient was hospitalized with vertebral compression fracture and suddenly developed right hemiparesis and total aphasia du

Assessing the Decision-Making Capacity of Terminally Ill Patients with Cancer.

OBJECTIVE: Despite the clinical, ethical, and legal magnitude of end-of-life decision-making, the capacity of terminally ill patients to make the medical decisions they often face is largely unknown. In practice, clinicians are responsible for determining when their patients are no longer competent to make treatment decisions, yet the accuracy of these assessments is unclear. The purpose of this study was to explore decision-making capacity and its assessment in terminally ill cancer patients. METHODS: Fifty-five patients with advanced cancer receiving inpatient palliative care and 50 healthy

Femoral tunneled central venous catheters for terminally ill patients with cancer.

BACKGROUND: Oral administration of medication is often difficult in terminally ill patients with cancer. These patients require intravenous routes for high-dose opioid administration and/or parenteral nutrition. When the superior vena cava (SVC) is unsuitable for central vein catheter insertion (i.e., in patients with mediastinal masses involving the SVC), alternative access routes are needed. Of these, the femoral vein is most utilized. In our experience, the femoral tunneled catheter (FTC) is easy and safe to use. We retrospectively studied FTC outcomes in terminally ill patients with cancer

Effect of nebulized furosemide in terminally ill cancer patients with dyspnea.

We evaluated the effect of ultrasonically nebulized furosemide (20 mg) on dyspnea uncontrollable by standard therapy in patients with terminal cancer. Dyspnea was evaluated using the Cancer Dyspnea Scale (CDS) before and 60 min after inhalation. Changes in arterial blood gases, hemoglobin oxygen saturation (SpO2), heart rate (HR), and respiratory rate (RR) also were evaluated. In 12 of 15 patients (80%), total dyspnea scores by CDS improved significantly after inhalation of furosemide (P = 0.007), especially concerning a reduced sense of effort (P = 0.013) and reduced anxiety (P = 0.04). Pain and Fatigue Interrelations in Terminally Ill Inpatients: A PCOC-Based Analysis.

CONTEXT: Pain and fatigue are among the most common and burdensome symptoms experienced by patients with advanced serious illness. However, few studies have captured their daily fluctuations using standardized, multidimensional tools. OBJECTIVES: This study aimed to examine the factors associated with daily pain and fatigue severity among patients with advanced serious illness using the palliative care outcomes collaboration (PCOC) framework in a Taiwanese palliative care setting. METHODS: A retrospective observational study was conducted at a medical center in central Taiwan between July 20

Factors influencing Jordanian nursing students' attitudes toward caring for terminally ill patients.

BACKGROUND: Providing care for terminally ill patients is demanding and many nursing students are insufficiently prepared to care for terminally ill patients. OBJECTIVES: This study aimed to explore nursing students' attitudes toward caring for terminally ill patients at universities in Jordan and identify the factors influencing these attitudes. METHODS: A quantitative, descriptive cross-sectional design was used. A multistage random sampling method was employed to recruit 500 nursing students from five governmental universities in Jordan. A structured questionnaire consisting of two parts,

Factors influencing spiritual well-being in terminally ill cancer inpatients in Japan.

PURPOSE: Spiritual well-being is very important in patients undergoing palliative care. Although psychosocial factors have been suggested to be associated with spiritual well-being, the relationship between physical signs and spiritual well-being has not been fully elucidated. The aim of this study was to explore diverse factors associated with spiritual well-being among palliative care patients in Japan. METHODS: This study is a secondary analysis of a multicenter prospective cohort study involving patients admitted to palliative care units in Japan. Physicians recorded all data prospectivel

Population pharmacodynamic modelling of midazolam induced sedation in terminally ill adult patients.

AIMS: Midazolam is the drug of choice for palliative sedation and is titrated to achieve the desired level of sedation. A previous pharmacokinetic (PK) study showed that variability between patients could be partly explained by renal function and inflammatory status. The goal of this study was to combine this PK information with pharmacodynamic (PD) data, to evaluate the variability in response to midazolam and to find clinically relevant covariates that may predict PD response. METHOD: A population PD analysis using nonlinear mixed effect models was performed with data from 43 terminally ill

Aggravation of fatigue by steroid therapy in terminally ill patients with cancer.

Steroids are commonly used for fatigue relief in terminally ill cancer patients. However, steroid-induced adverse effects including depression, myopathy, and hyperglycemia may contribute to fatigue. We report our experiences with aggravation of fatigue with steroid use in three cases. Case 1 was a 65-year-old man with advanced gastric cancer. He was started on betamethasone (2 mg/d) for fatigue, but the fatigue worsened due to steroid-induced depression. Discontinuation of steroids and initiation of an antidepressant ameliorated the fatigue. Case 2 was a 68-year-old man with advanced lung canc

Family-perceived distress from delirium-related symptoms of terminally ill cancer patients.

Delirium is a frequent symptom of terminally ill cancer patients and can cause serious distress to family members. To clarify the degree of emotional distress of family members concerning terminal delirium, a survey of bereaved families was performed. A questionnaire was mailed to 300 bereaved families with a request to rate the frequency and level of their distress for 12 delirium-related symptoms. A total of 195 responses were analyzed (effective response rate=65%). Seventy-four percent and 62% of the family members reported that the patients had symptoms of physical restlessness and mood la
