Self-reported learning among residents of a men's prison-based therapeutic community.

Background: The prison-based Therapeutic Community (TC) is a widely utilized program for individuals with substance use disorders that has proven effective in reducing recidivism. Yet some recent findings are more equivocal and suggest sources of heterogenous treatment engagement and response. Objectives: Given minimal prior research on the experiences and perspectives of prison-based TC participants, we assessed how men narrate the knowledge or skills they acquired from the TC program. Specifically, we analyzed responses to an open-ended survey item, "What have you learned in the TC?"Methods:

Development of Deaf Adults with Intellectual Disability in a Therapeutic Living Community.

People with intellectual disabilities who are deaf face obstacles participating in social environments that do not take into account their need for accessible visual communication. In the present case series, we describe the development of the adaptive skills profiles of eight participants in a fully inclusive therapeutic living community, designed specifically for people with developmental disabilities who are deaf and focused on supporting communication, social relationships, conflict resolution, and work satisfaction. Adaptive skills ratings collected at enrollment and twelve years later su

Outcomes from a therapeutic community for homeless addicted mothers and their children.

A women's therapeutic community (TC) designed to prevent homelessness was evaluated using a quasi-experimental process. Propensity analysis selected comparable experimental (E) and comparison (C) participants. Significant improvements were found for the E group at the domain level, both in "psychological" dysfunction on symptoms (e.g., depression), and in "health," including ratings of health and adherence to medication regimens. No significant difference was found at the domain level for "parenting" or "housing stabilization," but specific outcomes did differ. For example, a greater number of

Community-acquired pneumonia: Epidemiology, diagnosis, prognostic severity scales, and new therapeutic options.

This narrative review article explores the current scientific knowledge on the definition, epidemiology, diagnostic criteria, microbiology, treatment, and prevention of severe community-acquired pneumonia (SCAP) in immunocompetent adults. At present, despite major scientific advances in diagnostic evaluation, clinical management, antimicrobial therapy, and prevention, severe community-acquired pneumonia remains a major cause of morbidity and mortality, as well as having a major economic impact in terms of increased healthcare expenditure worldwide. This pathology is considered one of the leadi

Treatment engagement in a prison-based Therapeutic Community: A mixed-methods approach.

Given the high proportion of inmates with histories of substance use disorders in the era of mass incarceration, many correctional institutions have integrated the prison-based Therapeutic Community (TC) into their programming. While most evaluations provide evidence of lowered rates of relapse and recidivism, the majority of TC graduates still have negative outcomes post-release. Filling an important gap in the TC treatment literature, we analyze quantitative measures of treatment engagement alongside qualitative narratives from prison-based TC residents, providing an essential look into the

[Adapted Physical Activity for patients with chronic diseases in a therapeutic community].

The French therapeutic communities ("Appartements de Coordination Therapeutique (ACT)") are mostly members of the "National Federation of Accommodation for HIV+ and other chronic diseases. They provide accommodation for people living with chronic conditions (HIV hepatitis, cancers...) and in a situation of high psychosocial frailty. As a result of their coordinated multidisciplinary intervention, these structures provide the required support to access health care and facilitate social inclusion. They are designed to provide an appropriate response to people with cumulative medical and social c

An in-body networking system for communication between wearable and implantable therapeutics.

Networks of bioelectronic sensors and actuators enable closed-loop therapies that connect distant, related physiological dynamics. Currently, device interactions are limited by communication methods that inefficiently penetrate tissue and require bulky components. Inspired by the ionic signaling of the nervous system, we engineered a communication platform that uses tissue as a conductive medium to transmit signals among implants and wearables through epidermal, subcutaneous, intraperitoneal, and gastrointestinal spaces. Devices generate transient electric fields that selectively activate othe

Human-Animal Therapeutic Relationships: Communication, Welfare and Interpretative Asymmetry.

Human-animal therapeutic relationships are commonly evaluated through their effects on human psychological, emotional and social well-being, whereas the experiences and welfare of non-human animals remain less consistently examined. This narrative interdisciplinary review analyses such relationships through interpretative asymmetry, integrating human-animal studies, ethology, animal welfare science and biological approaches to interspecies communication. Selected cultural examples are used only as illustrative boundary narratives. Interpretative asymmetry is defined not as a statistically meas

Prison-based democratic therapeutic communities, medication, and the power to exclude.

In 1962, the first custodial Democratic Therapeutic Community (DTC) was established in the English prison estate at HMP Grendon. Today, the Category B male prison estate in England and Wales has three DTCs and three 'TC+' units for prisoners with learning disabilities. There is one DTC in the female estate at HMP Send. The services fall under the remit of the Offender Personality Disorder Pathway, a jointly commissioned initiative that aims to provide a pathway of psychologically informed services for a highly complex and challenging group of prisoners who are likely to have a severe personali

New concept of the pathogenesis and therapeutic orientation of acquired communicating hydrocephalus.

Hydrocephalus is a common medical condition characterized by abnormalities in the secretion, circulation and absorption of cerebrospinal fluid (CSF), resulting in ventricle dilatation. For the communicating hydrocephalus, without etiological treatment, its pathogenesis has been considered as a research emphasis. Many factors can damage the CSF system and trigger communicating hydrocephalus, including tumor surgery and hydrocephalus neurological diseases, such as brain trauma, infection, ICH and SAH. But according to our clinical experience, a big proportion of patients do not develop hydroceph

Intraventricular administration of urokinase as a novel therapeutic approach for communicating hydrocephalus.

Fibrosis of the subarachnoid space (SAS) after infection, inflammation, or hemorrhage can impair cerebrospinal fluid absorption and circulation, causing diffuse ventricular dilatation. In the present study, we tested the hypothesis that urokinase (also known as urokinase-type plasminogen activator [uPA]), a fibrinolytic agent, attenuates fibrosis and ventriculomegaly in a rat model of kaolin-induced communicating hydrocephalus and thus may have potential as a therapy for these conditions. Thirty microliters of sterile 25% kaolin suspension was injected into the basal cisterns of adult Sprague-

Communication intervention and therapeutic issues in pediatric human immunodeficiency virus.

This article provides an overview of treatment strategies that target communicative disorders exhibited by children exposed to human immunodeficiency virus (HIV). The myriad of communicative disorders presented by these children is discussed in relationship to the complex needs of culturally diverse families. Rehabilitation considerations, the impact of health status on treatment outcomes, and the need to work collaboratively with other health care professionals in meeting children's needs from a family-focused perspective are further discussed.

Lipophagy in disease: signaling control, organelle communication, and therapeutic opportunities.

Lipid droplet (LD) accumulation and impaired lipid mobilization induce lipotoxic stress and contribute to metabolic, cardiovascular, cancer, and neurodegenerative diseases (NDDs). Although LD-selective autophagy (lipophagy) is being increasingly studied, the mechanisms that confer LD specificity and enable tissue- and stage-specific therapeutic modulation remain unclear. Effective lipophagic flux requires coordinated LD coat remodeling, nutrient and energy sensing, and organelle contact. Coat remodeling governs substrate access and droplet recognition. The AMPK-mTORC1-TFEB axis links autophagy

Dysregulation of Neutrophil-Endothelial Communication in Sepsis: Mechanisms and Therapeutic Perspectives.

Sepsis is a clinical syndrome defined as life-threatening organ dysfunction caused by a dysregulation in immune response to infection. Dysregulated neutrophil activity plays a critical role in sepsis-induced organ failure through interactions with the vascular endothelial cells during forward and reverse migration, resulting in vascular barrier disruption and increased neutrophil trafficking into vital organs. Therapeutic approaches for treating sepsis are mainly supportive. Due to limited clinical translation from rodent models, complexity of the pathophysiology, and most importantly, the het

Therapeutic aspects of cell signaling and communication in Duchenne muscular dystrophy.

Duchenne muscular dystrophy (DMD) is a devastating chromosome X-linked disease that manifests predominantly in progressive skeletal muscle wasting and dysfunctions in the heart and diaphragm. Approximately 1/5000 boys and 1/50,000,000 girls suffer from DMD, and to date, the disease is incurable and leads to premature death. This phenotypic severity is due to mutations in the DMD gene, which result in the absence of functional dystrophin protein. Initially, dystrophin was thought to be a force transducer; however, it is now considered an essential component of the dystrophin-associated protein

Communication challenges for chronic metastatic cancer in an era of novel therapeutics.

Advances in the production of novel therapies for cancer management are creating new challenges for the support of increasing numbers of persons surviving for extended periods with advanced disease. Despite incurable and life-limiting metastatic conditions, these patients are living longer with serious disease, pushing the boundaries of what science explains and clinicians can confidently interpret using available evidence. Here we report findings from an early subset of such individuals within a longitudinal qualitative cancer cohort study on clinician-patient communication across the cancer

A structured environment for heroin addicts: the experiences of a community-based American methadone clinic and a residential Dutch therapeutic community.

The experiences of a community-based American methadone clinic and a Dutch residential Therapeutic Community are described. Both programs, which provided comprehensive psychotherapeutic services for a diverse subpopulation of heroin addicts who lived in different cultures independently, discovered that character disordered individuals respond positively to the structured environment and confrontation group psychotherapy orientation which are characteristic of the Therapeutic Community. Both programs modified the initial non-directive model into a structured one adopting the treatment technolog

Experience of the Therapeutic Community Regarding Individuals With Alcohol Use Disorder: Phenomenological Study.

This study aimed to explore and describe the experience of individuals with alcohol use disorder in the therapeutic community. The study was exploratory, with a qualitative design. Data were collected through in-depth interviews from 15 individuals with alcohol use disorder recovering in the therapeutic community and analyzed using Colaizzi's phenomenological method. There appear to be three categories: (1) feeling safe in an environment with structures and rules; (2) telling everything about myself without holding back; and (3) discovering the meaning of life. The healing elements in the ther
