Essential hypernatremia: disordered thirst and blood pressure control.

A 24 year old man developed partial central diabetes insipidus with impaired thirst and an elevated osmotic threshold to the release of arginine vasopressin (AVP). Plasma AVP was present in inappropriately small concentrations despite severe hyperosmolality. In addition, marked hypertension accompanied this disorder and all abnormalities, including the hypertension, responded to 1-desamino-8-D-arginine vasopressin therapy. Several lines of evidence suggest this disorder may be a disturbance of hypothalamic function.

Prevalence and Predictors of Thirst in Patients With Heart Failure.

BACKGROUND: Thirst is among the most unpleasant symptoms in patients with heart failure (HF). Intense thirst can lead to uncontrolled fluid intake, resulting in congestion and frequent readmissions. Many predictors of thirst were identified, but little research addressed its association with salt intake. OBJECTIVE: The aims were to describe the prevalence and characteristics of thirst in patients with HF, examine its association with salt intake, and identify its predictors. METHODS: Using a descriptive correlational design, 100 patients with New York Heart Association classes I, II, and III

[Hypokalemia due to a liquorice-containing thirst-quencher].

BACKGROUND: Liquorice consumption can cause pseudohyperaldosteronism and potentially lead to life-threatening complications. Besides correcting hypokalemia and hypertension, finding the triggering factor for pseudohyperaldosteronism is essential to prevent recurrence. CASE DESCRIPTION: A 68-year-old Syrian man presented in the Emergency Department with complaints of fatigue, weakness and exercise-related shortness of breath. Blood tests revealed severe hypokalemia for which suppletion and cardiac rhythm surveillance was necessary. Talking to the patient's son, it occurred that our patient dra

The hydration influence on the risk of stroke (THIRST) study.

OBJECTIVE: There is a decreased sensation of thirst and often dehydration among the elderly population. It is unclear whether it represents a contributing factor for cerebral ischemic events. DESIGN: Consecutive patients presenting to a University Hospital within 24 h of symptom onset, and a discharge diagnosis of acute ischemic stroke or transient ischemic attack in the year 2005, were identified. Healthy controls matched to gender, age, and presence of diabetes mellitus were obtained from the National Health and Nutrition Examination Survey (NHANES) 1999-2004. Calculated plasma osmolality o

Disturbance of osmoregulated thirst and vasopressin secretion in thyrotoxicosis.

OBJECTIVE: To assess the effect of untreated thyrotoxicosis on osmoregulated thirst sensation and AVP secretion. DESIGN: Measurements were made at 30-minute intervals while untreated thyrotoxic patients were given sodium chloride 855 mmol/l intravenously for 2 hours followed by water drinking ad libitum for 2 hours. The protocol was repeated when the patients were euthyroid. PATIENTS: Eight newly diagnosed thyrotoxic patients were studied. MEASUREMENTS: Thirst sensation (visual analogue scale), plasma osmolality, AVP and plasma renin activity were measured. RESULTS: Prior to osmotic stimul

[Regulation of thirst in end-stage kidney insufficiency].

About 30% of hemodialyzed patients are suffering from chronic fluid overload despite advice to restrict the oral fluid intake. To investigate the cause of the abnormal drinking behaviour a clinical study was performed in 51 non-diabetic patients with endstage renal disease exhibiting lower interdialysis weight gain (less than 3 kg, n = 17) and increased interdialysis weight gain (greater than 3 kg, n = 34). Blood pressure, body weight self-estimated thirst intensity before and after hemodialysis were analyzed. Biochemical and behavioral variables were measured including hormonal factors of wat

Osmoreceptor mediation of thirst and vasopressin secretion in the dog.

Water deprivation leads to depletion of both the intracellular and extracellular compartments. The resulting cellular dehydration and extracellular dehydration stimulate thirst and the secretion of vasopressin. The cellular stimulus is thought to arise from a change in volume and hence in the hydration status of specific osmoreceptor cells located in the anterior hypothalamus. Thus, reduction in cell volume because of either water deprivation or administration of hyperosmolar solutions of solutes that cannot penetrate cell membranes would stimulate thirst and vasopressin secretion, whereas an

Disordered control of thirst in hypothalamic-pituitary sarcoidosis.

We evaluated water metabolism in 11 patients with hypothalamic-pituitary sarcoidosis. Seven patients had abnormal water metabolism: one had partial antidiuretic hormone (ADH) deficiency, one had severe ADH deficiency, one had severe ADH deficiency and deficient thirst, one had only deficient thirst, and three had excessive thirst. In four of the five with thirst disturbances, there was loss of congruence of osmotic thresholds for thirst and ADH release. Five of our patients presented with polyuria and polydipsia, but only two had true diabetes insipidus, whereas three had organic primary polyd

Successful treatment of hypernatremic thirst deficiency with chlorpropamide.

Two patients with hypodipsia and hypernatremia are described. The first patient, whose hypodipsia was of unknown cause, developed hypernatremia unless large volumes of fluid were urged upon him; upon treatment with chlorpropamide normal serum sodium levels were achieved with spontaneous fluid intake. The second patient had hypodipsia and diabetes insipidus resulting from a craniopharyngioma. Treatment with vasopressin and a prescribed daily water intake resulted in frequent hyper- and hyponatremia, but treatment with chlorpropramide yielded serum sodium values which were more often normal and

Impact of an interventional thirst bundle on thirst discomfort and dry mouth in nasogastric tube-fed patients: Quasi-experimental time-series study.

BACKGROUND: Nasogastric (NG) tube feeding is commonly used for patients unable to eat orally but often causes thirst discomfort, dry mouth, reduced sensory satisfaction, and poor appetite. This study evaluated the efficacy of an interventional thirst bundle on thirst discomfort and dry mouth in patients receiving NG feeding. MATERIALS AND METHODS: A quasi-experimental time-series study was conducted from 2018 to 2020 at a 1000-bed tertiary care hospital in South Karnataka, India. Ninety adult patients receiving NG feeding and reporting thirst (score > 5) were selected using convenience sampli

Opioid modulation of thermal dehydration-induced thirst in rats.

Male Sprague-Dawley rats were utilized to study the effects of the opioid receptor antagonists, naloxone and naltrexone, on thirst induced by thermal dehydration. In an initial experiment, the depressant effect of naloxone (1.0 mg/kg, IP) on the water intake of rats deprived of water for 24 h was confirmed. In subsequent experiments, rats were thermally dehydrated by exposing them without water to a 40 degrees C environment for 1-4 h. Following heat exposure, rats were injected with either naloxone or naltrexone either IP or ICV. Fifteen minutes later, rats were provided with water and water i

Osmoregulation of thirst and vasopressin during normal menstrual cycle.

Changes in osmoregulation during normal menstrual cycle were examined in 15 healthy women. In 10 women, studied repetitively during two consecutive menstrual cycles, basal plasma osmolality, sodium, and urea decreased by 4 mosmol/kg, 2 meq/l, and 0.5 mM, respectively (all P less than 0.02) from the follicular to luteal phase. Plasma vasopressin, protein, hematocrit, mean arterial pressure, and body weight did not change. In five other women, diluting capacity and osmotic control of thirst and vasopressin release were assessed in follicular, ovulatory, and luteal phases. Responses of thirst and

The renin-angiotensin system and thirst: some unanswered questions.

Recent experiments dispute the significance of four findings that usually are cited to support the hypothesis that angiotensin is a natural dipsogen. First, although administration of exogenous renin consistently increases water intake, the plasma renin activities that are produced seem to be outside of the normal physiological range when the elicited drinking is substantial. Second, although plasma renin activities are elevated following caval ligation, colloid, or isoproterenol treatment, this activity of the renin-angiotensin system appears to account for only a small portion of the observe

Thirst as an ingestive behavior: A brief review on physiology and assessment.

BACKGROUND: Thirst is a sensation normally aroused by a lack of water and associated with a desire to drink more fluid. AIM: The aims of this brief review are twofold: (a) to summarize the thirst mechanism in how it is initiated and diminished, and (b) to describe techniques to assess human thirst accurately in a variety of situations. DISCUSSION: Thirst is maintained via a feedback-controlled mechanism, regulated by central and peripheral factors, as well as social and psychological cues. Most studies of thirst have focused on the initiation of water intake and the neural mechanisms respons
