Thoracic aortic aneurysms and other thoracic aortic lesions may become life-threatening conditions if they remain untreated. Conventional open surgical reconstruction with placement of an interposition graft is regarded as a definitive form of treatment, but is associated with considerable operative morbidity and mortality. Thoracic aortic lesions involving the aortic arch require more complex surgical interventions necessitating cardiopulmonary bypass, and hypothermic circulatory arrest. Outcomes from this form of treatment have a reported early stroke and death rate of up to 25%. Thoracic en

Robotic-assisted thoracic surgery (RATS) has undergone rapid technological evolution over the past two decades, progressively expanding from early minimally invasive applications to increasingly complex thoracic procedures. This narrative review examines the historical development of robotic platforms in thoracic surgery, the emergence of new approaches and alternative robotic systems, and future technological perspectives. A literature search was conducted using PubMed/MEDLINE, Embase, Scopus, Web of Science Core Collection, and the Cochrane Library to identify relevant English-language publi

BACKGROUND: In April 2024, we aimed to reform the physicians' working style in our hospital, which is a core regional hospital where only two full-time thoracic surgeons work. We report three efforts to enhance productivity despite of limited number of staff in the department of thoracic surgery. Efforts:1)A new on-call system for thoracic surgery was introduced in April, 2024. To reduce the burden of on-call, we designated certain days when the on-call service is unavailable, where as the minimum level of medical care is ensured by transporting patients to other hospitals on those days. 2)The

Artificial intelligence (AI) is moving rapidly from retrospective prediction and image analysis into treatment selection, operative planning, intraoperative guidance, and postoperative prognostication in cardiothoracic surgery. This transition raises an ethical problem that cannot be resolved by model accuracy alone: when an algorithm begins to shape a high-stakes clinical decision, the distribution of knowledge, authority, and responsibility also changes. This review synthesizes cardiothoracic and closely related medical evidence available through August 2026, with emphasis on quantitative pe

Cardiothoracic surgeons work in high-intensity environments starting in surgical training and throughout their careers. They deal with critical patients. Their routine procedures are delicate, require extensive attention to detail, and can have detrimental effects on patients' lives. Cardiothoracic surgeons are required to perform at their best capacity incessantly. To do this, they must safeguard their mental and physical well-being. Preserving health through sleep, nutrition, exercise, and routine medical checkups ensures a cardiothoracic surgeon's well-being. Great personal effort and disci

OBJECTIVE: Children rarely undergo thoracic surgery. When they do, the procedures fall into five main groups: oncologic indications, immune defects, malformations, infections and trauma. In addition to considerations associated with the underlying indication, the different proportions of the anatomical structures in children require special modifications in both diagnostics and surgical technique compared to corresponding procedures in adults.
METHODS: Of a total 2137 thoracic surgical procedures performed between 1992 and 2001, 49 were performed in children (n = 37; age: 3 months-15 years; m

Oncological thoracic surgery comprises challenging procedures not only from a surgical point of view but also regarding the postoperative management. The most significant procedures performed for malignancy are those requiring resection of the lung, the pleura, the esophagus, mediastinal structures, the chest wall and airways. Especially after major procedures, postoperative complications occur frequently and usually require a multidisciplinary approach involving thoracic surgeons, anesthesiologists, pulmonologist, physiotherapists and oncologists. Thus, a proper postoperative management is as

The lateral retropleural thoracic approach offers minimally invasive access for the treatment of thoracic spine pathology, specifically thoracic herniated discs. Alternatives to the retropleural approach traditionally included posterolateral or anterior approaches, which carry increased morbidity. The retropleural approach affords lateral access to the thoracic spine that allows for addressing pathology such as herniated discs, corpectomy, tumor, or trauma. This article outlines preoperative workup and planning, intraoperative steps, tips, and postoperative care.

Minimally invasive thoracic surgery has come a long way. It has rapidly progressed to complex procedures such as lobectomy, pneumonectomy, esophagectomy, and resection of mediastinal tumors. Video-assisted thoracic surgery (VATS) offered perceptible benefits over thoracotomy in terms of less postoperative pain and narcotic utilization, shorter ICU and hospital stay, decreased incidence of postoperative complications combined with quicker return to work, and better cosmesis. However, despite its obvious advantages, the General Thoracic Surgical Community has been relatively slow in adapting VAT

To describe the use of the I-gel supraglottic airway device when placing a bronchial blocker for single-lung ventilation, as applied in a preliminary study to explore the feasibility of adopting the technique with appropriate, selected patients undergoing certain thoracic surgery procedures. We used the technique for single-lung ventilation in 25 patients who required isolation of a lung for a variety of thoracic surgical procedures. A bronchial blocker was placed under direct visualization through a fiberoptic bronchoscope and the I-gel supraglottic device. The I-gel mask allowed us to establ

BACKGROUND: The aim of this working group of the German Society for Thoracic Surgery (DGT) was to develop an expert consensus based on the Delphi method to define "tissue handling" and depict intraoperative handling of specific anatomical structures during thoracic surgery.
METHODS: Invited experts (thoracic surgery specialists; n = 93) completed two consecutive rounds of electronic Delphi questions on four main topics: transection of lung parenchyma, dissection/separation of pulmonary vessels, angioplasty/vascular anastomoses, and bronchus settling closure/plasty/anastomosis. Consensus was s

Significant extension of thyroid masses into the mediastinum has historically necessitated high-morbidity sternotomy or thoracotomy incisions during substernal thyroidectomy. This preliminary experience from a single-center, tertiary-care academic institution illustrates a combined minimally invasive robotic-assisted thoracic surgery (RATS) and transcervical approach for substernal thyroidectomy. Patients are considered for the minimally invasive approach for substantial goiter extension into the anterior and posterior mediastinum, in addition to risk factors for wound healing complications as

Tuberculosis (TB), particularly multidrug-resistant TB (MDR-TB), continues to pose a major public health challenge in India (1). Thoracic surgery plays an important adjunctive role in selected patients with complex pulmonary TB; however, access to trained thoracic surgeons remains limited. Under the guidance of the Central TB Division, the National Institute of Tuberculosis and Respiratory Diseases (NITRD), New Delhi, in collaboration with The Union, initiated a 3-month Residential Skill Training Programme in Thoracic Surgery in 2022. This short communication describes the experiences and perc

Noteworthy in Cardiothoracic Surgery 2025 highlights several of the most influential trials and emerging trends shaping cardiothoracic surgical practice. In structural heart disease, new randomized data have expanded consideration of transcatheter aortic valve replacement to asymptomatic patients while reinforcing the importance of longer-term durability and lifetime valve management in lower-risk populations. In heart transplantation, advances in minimally invasive and robotic techniques, alongside growing international experience with donation after circulatory death, underscore both the pro
