Giant hibernoma of the thoracic pleura and chest wall.

Hibernoma is a rare tumor containing prominent brown adipocytes that resemble normal brown fat. Brown fat (versus white fat) is predominantly found in hibernating mammals and infants. Brown fat adipocytes contain a higher number of small lipid droplets and a much denser concentration of mitochondria. The tumor can occur in a variety of locations however the extremities, followed by the head and neck, have been the most common sights. All variants of hibernoma described have followed a benign course with the majority presenting as a small, lobulated, nontender lesions. We present a case of a gi

[Maffucci's syndrome with giant tumor of the thoracic wall].

Maffucci's syndrome is a rare nonhereditary malformation of mesodremal dysplasia origin which consists of multiple hemangiomas of the soft tissue and multiple enchondromas. Only approximately 170 cases of this disease have been reported in the literature. Maffucci's syndrome is known to be associated with tumors of mesodermal origin. Chondrosarcoma, osteosarcoma and angiosarcoma are the most common malignant neoplasms and the benign tumors consist of pituitary adenoma, adrenal cortical adenoma, parathyroid adenoma, thyroid adenoma and breast fibroadenoma. We present a case report of a 26-year

Stabilisation of thoracic wall in patients with chest injury.

The authors analyse a group of 38 polytraumatised patients with unstable thoracic injury who were subdued to internal surgical stabilisation of the thoracic wall during the period from 1st October 1994 to 30th September 2001. The average period of controlled pulmonary ventilation (CPV) was 3 days and the average period of hospitalisation at the Anaesthesiology and Intensive Care Department (AICD) was 10 days. In indicated cases, the authors recommend an active surgical approach in the treatment of patients with unstable thoracic injuries. (Tab. 2, Fig. 2, Ref. 3.).

[Surgical treatment of congenital deformities of the thoracic wall].

The author describes an original procedure for the correction of deformities of the antero-lateral thoracic wall, stressing the tactical and technical principles. The mandatory operational steps are the following: wide chondrectomy, borad retrosternal separation of the periost, transversal osteotomy of the sternum and contention by metalic blades placed behind the sternum. The author stresses the surgical indications in all anterolateral deformities of the thorax and considers that the best age for performing the correction is after 10-12 years.

Incidence of the accessory internal thoracic artery and its safety area in the thoracic wall.

OBJECTIVE: To provide the incidence, origin, size and the safety area of the accessory internal thoracic artery (AITA) in the fourth intercostal space. MATERIAL AND METHOD: One hundred and thirty two thoracic cages were dissected and examined for the presence of AITA. The origin, course, diameter and location in the fourth intercostal space of detected AITA were investigated. RESULTS: AITA with diameter ranging from 0.6 to 3.05 (mean = 1.76 +/- 0.69) millimeters existed in 10 of 132 (7.58%) cadavers. They originated from the upper part of the internal thoracic artery, close and inferior to t

Thoracic wall defects: surgical management of 205 consecutive patients.

In this article, we review our experience during the past 9 years with 205 consecutive thoracic wall reconstructions. The 100 female and 105 male patients ranged in age from 12 to 85 years (mean, 53.4 years). One hundred fourteen patients had thoracic wall tumors, 56 had radiation necrosis, 56 had infected median sternotomy wounds, and 8 had costochondritis. Twenty-nine of these patients had combinations of the aforementioned conditions. One hundred seventy-eight patients underwent skeletal resection. A mean of 5.4 ribs were resected in 142 patients. Total or partial sternectomies were perform

Abdominal and thoracic wall closure: damage control surgery's cinderella.

Damage control surgery principles allow delayed management of traumatic lesions and early metabolic resuscitation by performing abbreviated procedures and prompt resuscitation maneuvers in severely injured trauma patients. However, the initial physiological response to trauma and surgery, along with the hemostatic resuscitation efforts, causes important side effects on intracavitary organs such as tissue edema, increased cavity pressure, and hemodynamic collapse. Consequently, different techniques have been developed over the years for a delayed cavity closure. Nonetheless, the optimal managem

Pneumocephalus Following Thoracic Surgery with Posterior Chest Wall Resection.

Pneumocephalus can be seen after head injury with fracture of the skull-base or in cerebral neoplasm, infection, or after intracranial or spinal surgery. We report on a 69-year-old male patient with pneumocephalus after right-sided lobectomy and en bloc resection of the chest wall for non-small-cell lung cancer. Postoperatively, the patient showed a reduced vigilance level with no response to pain stimuli and anisocoria. The CCT scan revealed an extensive pneumocephalus; following which, the patient underwent neurosurgery with laminectomy and ligature of the transected nerve roots. After opera

Thoracic wall ultrasonography - normal and pathological findings. Pictorial essay.

Ultrasonography represents an efficient diagnosis method for thoracic pathology evaluation. The diseases of the pleura and thoracic wall receive the greatest benefit from the ultrasound examination. Establishing the cause of an acute thoracic pain or assessing a palpable tumor may easily benefit from essential data provided by ultrasonography. Afterwards it is possible to use a complementary imaging method or to perform US guided interventional procedures. This pictorial presents the ultrasonographic aspects of thoracic wall pathology, considering also differential diagnosis issues.

Late recurrent ovarian carcinoma metastatic to the thoracic wall.

Ovarian cancer is the second most common gynaecologic malignancy. Ovarian carcinomas typically metastasize to multiple sites via exfoliation, lymphatic spread or direct invasion. We present a rare case of a very late recurrence of ovarian carcinoma into the thoracic wall, heralded by thoracic pain in a patient otherwise disease-free for 23 years. This unusual and late presentation of an ovarian cancer metastasis underscores the need for continued awareness and attention to new symptoms in patients with ovarian cancer who show prolonged disease-free intervals.

High Thoracic Tortuosity Is Associated With CT-Markers of Degeneration of the Descending Thoracic Aortic Wall.

OBJECTIVE/INTRODUCTION: Increased vascular tortuosity may be an independent marker of generalized aortic pathology. This study investigates the association between descending thoracic aortic tortuosity, aneurysm pathophysiology, and outcomes following EVAR in AAA patients. METHODS: Patients who underwent elective EVAR between 2004 and 2018 were reviewed. Thoracic tortuosity index (TTI) was measured using 3D reconstruction software. Patients were dichotomized across the median TTI into high tortuosity [HT] (TTI >1.27, Figure1(a)) and low tortuosity [LT] (TTI ≤1.27, Figure 1(b)) groups. Periope

Penetration of screw into the wall of thoracic aorta after stabilization of thoracic spine--case report.

In our research we have discussed a rare case of penetration of screw into the thoracic aorta after stabilization of thoracic spine due to compressive fracture of IV thoracic vertebra. We have focused on existence of potential danger of damage of aorta during stabilization of the posterior part of thoracic spine. We have emphasized the meaning of prophylactic implantation of stent-graft into the site of potential damage of aorta before the restabilization of the vertebral column.

A Rare Lesion of the Thoracic Wall: Giant Scapulothoracic Bursitis.

Scapulothoracic bursitis, a rare lesion of the thoracic wall, usually presents as a cystic mass growing at the scapulothoracic interface. Histopathologically, it is characterized by the presence of synovial cells lining the interior of the thickened fibrotic cystic wall and capillary proliferation. A 48-year-old male patient was admitted to our clinic with a complaint of swelling in the back. The magnetic resonance imaging of the lung and mediastinum showed a 43 mm × 130 mm axial lesion in the left infrascapular area between the external muscles and the serratus anterior muscle, hyperintense o

[Various possibilities of reconstruction of the thoracic wall in radiation defects].

Reconstruction of the thoracic wall is often necessary after cancer surgery of the breast or irradiation. The resulting large defects often cause an instable thoracic wall. To reconstruct stability, we can use fascia or dura mater. The large defects can be closed either by latissimus flap, or, if the thoracodorsal artery is destroyed, which is often the case by rotating flaps according to the technique of Schrudde or Bohmert, in very rare cases by distant flaps from the neighbouring upper arm.

[False mass of the thoracic wall in obese adolescent girls].

Each of four obese adolescent females were thought to have pre-sternal anterior chest wall "masses" when evaluated by lateral chest radiograph. All had normal physical examinations. The apparent chest wall masses were found to be an artifact created by partial midline contact of both, large breasts, surrounded by a partial midline air space adding contrast to the adjacent soft tissue. Manual abduction of the breasts was followed by disappearance of the apparent radiographic abnormality.
