The occurrence of autoimmune phenomena in 28 patients with thromboangiitis obliterans (Buerger's disease) was determined. The following were sought: antineutrophil cytoplasmic antibodies against proteinase 3 (cANCA) and myeloperoxidase (pANCA), antinuclear antibodies, anti-Ro antibodies, anticardiolipin antibodies, and antiendothelial cell antibodies (AECA). For the last, an enzyme-linked immunosorbent assay was developed which verified the presence of the AECA phenomenon with immunofluorescence and confocal microscopy. Seven patients with active disease had AECA titers of 1,857 +/- 450 arbitr…

Thromboangiitis obliterans (TAO), or Buerger's disease, is a distinct clinicopathologic entity. It is a relatively uncommon cause of occlusive peripheral vascular disease. The disease is more prevalent in some geographic areas than in others, and there is a probable ethnic predisposition. Buerger's disease is decidedly rare in women; only 24 cases with histologic proof of the disease have ever been recorded, including 12 new cases described in this article. The clinical presentation and histopathology of TAO in women differ in no way from that in men. There is no evidence that the disease is m…

During a review of patients admitted with thromboangiitis obliterans (TAO), there was evidence of normochromic normocytic anemia and abrupt changes in hemoglobin (Hgb) levels in patients with several hospital admissions. Therefore, the evidence of hemolytic anemia was evaluated based on 37 banked plasma samples taken from Caucasian male TAO patients during disease exacerbation between 2012 and 2014. The patients' hospital records, including clinical manifestations and complete blood count, were evaluated. The following tests were performed on all samples: indirect antiglobulin test (IAT), C-re…

This report describes a small, nonrandomized trial of cyclophosphamide in the treatment of patients with advanced thromboangiitis obliterans (TAO) with modest results. The rationale of the treatment was based on the immunopathogenesis of the disease, ie, autoimmune vasculitis of peripheral arteries. Twelve male patient volunteers with TAO were included for the trial. Diagnosis was based on the history of chronic smoking or tobacco chewing, clinical features of ischemia of peripheral vessels, radioarteriography showing arterial block, and characteristic histopathologic changes of affected arter…

PURPOSE: When critical limb ischemia (CLI) occurs in patients with thromboangiitis obliterans (TAO) or Buerger's disease, smoking cessation alone may be insufficient to relieve rest pain and promote wound healing. Accordingly, adjunctive measures are warranted to restore adequate blood flow required for limb salvage. This study aimed to evaluate the feasibility and efficacy of percutaneous transluminal angioplasty (PTA) for the treatment of patients with TAO and CLI. In addition, a review of the literature on endovascular management of TAO is included.
METHODS: Between April 2012 and June 201…

Thromboangiits obliterans (Buerger's disease) is a non-atherosclerotic, segmental inflammatory and obliterative disease affecting small and medium sized arteries and veins. The etiology is still unknown, but it is in close relationship with tobacco use. Symptoms begin under the age of 45 years and the undulating course is typical. Patients usually present with acute and chronic ischemic or infectious acral lesions. Diagnosis is usually based on clinical and angiographic criteria and it is important to exclude autoimmune disease, thrombophilia, diabetes, and proximal embolic sources. Even thoug

Two years before the appearance of distal peripheral manifestations of thromboangiitis obliterans (Buerger's disease), a young man had acute peritonitis attributable to an ischemic perforation of the sigmoid colon. Only the histological examination of excised tissue was able to differentially diagnose this entity unambiguously.

In two female patients with an ischaemic peripheral disease, 49 and 32 years old, thromboangiitis obliterans (Buerger's disease) was diagnosed. One suffered from ulceration of her left second toe, the other from pain of both hands followed by ulceration of two fingers. Both were smokers. Surgical treatment (amputation) was required, and they also had a prostacyclin infusion. They recovered and stopped smoking. Thromboangiitis obliterans is a non-arteriosclerotic, segmental occlusive disease of medium-sized and small arteries and veins, occurring predominantly in young male habitual smokers. In…

Thromboangiitis obliterans (TAO) is a recurrent, inflammatory, segmental occlusive disease of distal limb arteries which may involve veins as well. Young smokers are predisposed to TAO. The intestinal vessels are rarely affected. We report the clinical, angiographic and histologic findings of a 23-year-old smoker with extended thrombophlebitis who developed severe abdominal angina and liver hypoxia due to occlusion of the coeliac artery and subtotal occlusion of the superior mesenteric artery. Two surgical bypassing procedures were followed by early graft occlusion. Subsequently, a plastic pat…

A case of thromboangiitis obliterans (Buerger's disease) showing vascular lesions in the oral cavity is presented. To our knowledge, this is the first report of involvement of branches of the external carotid artery with this disease. The clinical, radiographic, plethysmographic, and histologic features of Buerger's disease are presented. Etiologic factors, pathogenesis, and treatment modalities are discussed.

The existence of thromboangiitis obliterans has been questioned for some years. However, it is now accepted as a specific condition within the wide group of vasculitis. We report a patient with this condition, who, as usual, was a male smoker. The illness presented with thrombophlebitis migrans, which is an uncommon initial feature. The patient also had small vessel cutaneous vasculitis, which is an extremely uncommon association. We report the case and discuss some aspects of the etiology and pathogenesis of the disease.

Thromboangiitis obliterans, a distinct clinical and pathologic entity characterized by segmental inflammatory and proliferative lesions of the tunica media of small arteries and veins, has been reported frequently in men who have a history of heavy cigarette smoking. We report a case of thromboangiitis obliterans in a 38-year-old man that was clearly associated with the use of smokeless (chewing) tobacco. In addition to a physical history and examination and laboratory evaluation, a biopsy of the skin and deep subcutaneous tissue of the patient's left thigh was performed, and this revealed occ…

Thromboangiitis obliterans, or Buerger disease, is a rare nonatherosclerotic segmental inflammatory vasculitis that generally affects young tobacco smokers. Although no surgical treatment is available, the most effective way to halt the disease's progress is smoking cessation. In this case report, a 29-year-old smoker showed up to emergency department with gangrene of his fifth left toe and extensive plantar ulceration. After investigative angiography, he was diagnosed with Buerger disease. On November 2017, he underwent fifth left toe amputation and hyperbaric therapy. Five months after amput…
