Acute suppurative thyroiditis is a rare disorder that is mostly found in the left lobe of the thyroid gland of children due to congenital patency of the pyriform sinus fistula. Here, we report a 61-year-old man with acute right-sided suppurative thyroiditis without pyriform sinus fistula. He also showed infectious hip arthritis, spondylitis and Roth's spots. He presented with heart failure and was diagnosed with infectious endocarditis by sequential transesophageal echocardiography. A replacement with a prosthetic valve was performed and cured him. It is important to recognize that infectious

BACKGROUND: Pyriform sinus fistulae are the major routes of infection in acute suppurative thyroiditis (AST). There have been only a few reports describing imaging studies in AST. We reviewed our imaging studies in patients with AST to elucidate its features so as to facilitate its diagnosis and treatment.
METHODS: We reviewed ultrasonography (US) examinations, computed tomography (CT) scans, and barium swallow studies performed on 60 patients with the AST who were seen for medical care between 1998 and 2008 and were retrospectively reviewed. All of these patients had pyriform sinus fistulae.

A case of thyrotoxicosis caused by acute suppurative thyroiditis after repeated fine-needle aspiration (FNA) is described. A 39-year-old woman with atopic dermatitis showed rapid enlargement of a left thyroid cyst after a third FNA. She had a high fever, painful swelling of the left thyroid, and elevated thyroid hormone levels. Ultrasonography revealed abscess formation in the left thyroid cyst. The cytologic examination of an FNA specimen showed abundant neutrophils, and culture of the aspirate yielded Staphylococcus aureus. Because antibiotic treatment for 1 month failed to improve the infla

A patient with suppurative thyroiditis due to infection with Salmonella brandenburg is reported. Localization of the infection occurred to a pre-existing thyroid nodule after Salmonella bacteremia. S. brandenburg was isolated in pure culture from the fluid obtained by needle aspiration of the suppurated thyroid nodule. Surgical drainage followed by subtotal thyroidectomy was required to cure the disease. No evidence of pyriform sinus fistula was found. Suppurative thyroiditis due to Salmonella ubiquitous serotypes is an extremely rare condition, and infection to the thyroid produced by S. bran

A previously healthy 72-year-old man, who had been treated with steroids for one month on a presumptive diagnosis of polymyalgia rheumatica, developed a suppurative thyreoiditis. Salmonella enteritidis was isolated from the thyroid aspirate. Although the bacterial strain was sensitive to ampicillin in vitro, the disease was not cured until treatment with trimethoprim-sulfamethoxazole was initiated. The patient developed a high antibody response of the IgM class against both salmonella serogroup B and D O-antigenic determinants, as estimated by enzyme-linked immunosorbent assay (ELISA). Absorpt

Acute suppurative thyroiditis (AST) is a rare endocrinological emergency in pediatrics that may require immediate infectious disease and surgical intervention. It is generally associated with bacterial infections and, in some cases, congenital malformations of the branchial arch. We present the case of a 7-year-old girl with recurrent AST, in both episodes with cervical abscess formation, requiring intravenous antibiotics and surgical drainage. From the outset, a piriform sinus fistula was suspected, which was confirmed by direct laryngoscopy in the operating room during the hospitalization fo

Acute suppurative thyroiditis is a rare disease because the thyroid gland is remarkably resistant to infection. We present a 2-year-old girl with refractory acute suppurative thyroiditis due to a pyriform sinus fistula (PSF). She complained of fever and painful anterior neck swelling. Her condition did not completely improved by multiple parenteral antibiotics along with incision and drainage. Barium esophagogram to detect PSF demonstrated no specific finding. Computed tomography scan showed air bubble superior to the left thyroid gland which indicated a possible fistula connected to the pyrif

This review describes the microbiology, diagnosis and management of suppurative thyroiditis (ST). Staphylococcus aureus, Streptococcus pyogenes, Streptococcus epidermidis, and Streptococcus pneumoniae, are the predominant aerobic isolates. The most common anaerobic bacteria are Gram-negative bacilli and Peptostreptococcus spp. Agents that are rarely recovered include Klebsiella spp., Haemophilus influenzae, Streptococcus viridans, Salmonella spp., Enterobacteriaceae, Mycobacterium tuberculosis, atypical mycobacteria, Aspergillus spp., Coccidioides immitis, Candida spp., Treponema pallidum, and

Acute suppurative thyroiditis is a rare disease in childhood. The etiology of this condition is not always obvious. The description of a pharyngeal pouch fistula extending from the pyriform sinus to the thyroid gland or perithyroid area, high-lighted the etiopathogenesis of some of the thyroiditis cases. Presented here are the clinical, radiological and pathological aspects of an internal pharyngeal fistula manifesting with recurrent suppurative thyroiditis, until it was completely removed. The clinical features of caudal branchial pouch remnants and their role in the development of acute supp

A case of suppurative thyroiditis due to A.naeslundi is recorded. The clinical features (including the difficulty in establishing a clinical diagnosis) have been noted. This is the first instance of A.naeslundi being considered the etiological agent of actinomycosis of the thyroid. It is also the first report incriminating A.naeslundi as a pathogen, since up to now it was considered only a saprophyte. The etiological role of A.naeslundi in this case of suppurative thyroiditis is strongly supported by the following findings:1. A.naeslundi was isolated without the presence of other pathogenic mic

Riedel's thyroiditis (RT) is a rare form of infiltrative and inflammatory disease of the thyroid, first described by Bernard Riedel in 1896. The concurrent presence of RT and other thyroid diseases has been reported, but, the association of RT with Hashimoto's thyroiditis and acute thyroiditis has not yet been reported. We present a case of concurrent Riedel's, Hashimoto's and acute thyroiditis that occurred in a 45-year-old patient.

Thyroid gland suppuration is a rare condition with nonspecific features. For this reason, diagnosis is often delayed, which may lead to a life-threatening situation. Causative agents are often gram-positive bacteria, such as Staphylococcus aureus and Streptococcus pneumoniae. With appropriate antibiotics and drainage of the abscess, the prognosis is usually excellent. Herein, we describe a unique case of acute suppurative thyroiditis in an adult male presenting with nonspecific sore throat after a fish bone injury to the throat. The patient had a history of essential hypertension and gouty art

BACKGROUND: Acute suppurative thyroiditis (AST) is a rare form of thyroid inflammation prevalently of bacterial origin that usually affects subjects with risk factors such as immunodeficiency, sepsis, and neck fistulas. The most prevalent pathogens associated with AST are gram-- positive aerobic bacteria, followed by gram-negatives, while infections by anaerobic germs are exceptionally rare. Gemella morbillorum is a facultative anaerobic gram- positive bacterium that commonly populates the upper respiratory tract. Infections by Gemella Morbillorum have been previously documented in different r

Acute suppurative thyroiditis is an uncommon disorder caused by a bacterial infection, usually presenting with normal thyroid function. It is a serious condition that requires a prompt diagnosis and treatment with antibiotics and supportive measures. A 62 years-old female presented with a painful cervical induration and odynophagia a week after a fish bone had been removed from her pharynx. She was febrile, and tachycardic and, on physical examination, a painful thyroid mass was detected. High inflammatory parameters and thyrotoxicosis were confirmed: thyroid stimulating hormone (TSH) < 0.01 m

Acute suppurative thyroiditis (AST) is a rare clinical condition. Streptococcus anginosus has a propensity of producing empyema in the head and neck. Here, we reported two cases of AST caused by S. anginosus. A 44-year-old man presented with anterior neck tender swelling and odynophagia for 12 days. He had thyrotoxicosis. He was initially diagnosed as thyroid cancer due to the misleading computed tomography report. Fine needle aspiration (FNA) yielded pus and neutrophils. S. anginosus was isolated from pus. After aspiration of abscess and treatment with sensitive antibiotics, he recovered unev

Acute suppurative thyroiditis is very rare and often related to congenital abnormalities of the branchial pouches or immunodeficiency. A case of acute thyroiditis due to fistula of the left pyriform sinus in a young man with no relevant history is reported. We review the embryology, diagnosis, and treatment of this rare entity.
