Neonatal thyrotoxicosis (hyperthyroidism) is less prevalent than congenital hypothyroidism; however, it can lead to significant morbidity and mortality if not promptly recognized and adequately treated. Most cases are transient, secondary to maternal autoimmune hyperthyroidism (Graves disease [GD]). This article summarizes recommendations for screening and management of hyperthyroidism in both the fetal and neonatal periods, with a focus on neonatal thyrotoxicosis secondary to maternal GD. Early monitoring and treatment are crucial for optimizing short-term and long-term patient outcomes.

Clinical manifestations of thyrotoxicosis range from silent to florid and can mimic those of other conditions occurring in pregnancy and old age. The diagnosis may be particularly vexing in elderly patients with apparent dementia. Once clinical suspicion has been aroused, the workup to confirm the diagnosis and establish the cause is easily accomplished.

Subclinical thyrotoxicosis is defined as low serum thyrotropin (TSH) and normal serum thyroid hormone concentrations. It must be distinguished from nonthyroidal illness and secondary hypothyroidism. The most common causes are excessive thyroid hormone therapy, autonomously functioning thyroid adenoma, multinodular goiter, and Graves' disease, but many patients have no evident thyroid disease. A few patients have minor symptoms and signs of hyperthyroidism. The likelihood of progression to overt thyrotoxicosis is low, and many patients have normal serum TSH concentrations weeks or months later.…

A previously fit 37-year-old woman was admitted with acute left ventricular failure, atrial fibrillation and a left-sided thromboembolic stroke. Her ventricular rate progressively increased despite full digitalisation. She had a cardiac arrest and was successfully resuscitated. Investigations revealed evidence of hyperthyroidism and she was treated for thyrotoxic crisis. She has made a complete recovery from the hemiparesis and is now in sinus rhythm. She denied any symptoms of hyperthyroidism. This patient had apathetic thyroid crisis, which is exceedingly rare in the young.

The incidence of juvenile thyrotoxicosis has not exceeded one case per 100,000 population per year in Hungary during the past decades. From more than 200 simultaneous determinations of serum thyroid hormones (T3, T4, FT4) it was concluded that increased FT4 concentrations are the most frequent findings in thyrotoxicosis, even when both or one of the other thyroid hormone concentrations (T3, T4) are normal. TSH and prolactin responses to TRH were studied in 18 patients; TSH remained always undetectable whereas prolactin increased in half of the patients. A significant negative relationship was …

A 41-year-old man had thyrotoxicosis, diffuse goiter, exophthalmos, high titer of antithyroglobulin antibodies but undetectable long-acting thyroid stimulator. Initially, he had both elevated serum thyroxine (T4) and reverse triiodothyronine (rT3) levels, but normal serum triiodothyronine (T3) and free triiodothyronine (FT3) levels and 24-hour radioactive iodine uptake. Observations prior to radioactive iodine therapy uptake. Observations prior to radioactive iodine therapy showed a persistently normal T3 level in spite of development of atrial fibrillation. Iodine excess was not present, nor …
