Prognostic factors for successful percutaneous tibial nerve stimulation.

OBJECTIVE: In sacral as well as tibial nerve stimulation test stimulation is the main prognostic factor for success. In our study we tried to identify prognostic patient characteristics to improve patient selection for neuromodulation therapy. METHODS: PTNS was applied to 132 patients in 8 study centers (51 men, 81 women, mean age of 53 years (range: 21-82)). 83 patients were treated for overactive bladder, 16 for non-obstructive urinary retention and 33 for chronic pelvic pain. All patients had to fill out micturition or pain diaries, as well as quality of life questionnaires before and afte

Iatrogenic posterior tibial nerve division during ankle arthroscopy.

Ankle arthroscopy has been increasingly applied to the diagnostic and therapeutic treatment of ankle disorders. Owing to the complex cutaneous anatomy of the ankle, neurological injuries are a potential complication of this procedure. All reports of neurological complications resulting from ankle arthroscopy have attributed them to use of a distractor pin or to portal placement. Several authors have noted the possibility of damage to the deep peroneal nerve from the use of motorized arthroscopic tools within the anterior ankle joint capsule. We present what we believe to be the first reported

Tibial nerve grafting for restoration of plantar sensation.

Five patients with a segmental loss of the tibial nerve and insensate plantar aspect of the foot were evaluated at an average of 5 years (2.5-9.5) postsural nerve grafting of the tibial nerve. Free-tissue transfer was required in three patients. The results as graded by restoration of superficial sensation, healing of plantar ulceration, and absence of neurogenic pain were four good and one fair at follow-up over 2 years. Assessment at 2 years or less yielded one good and four poor results, indicating a prolonged recovery period. Nerve grafting may be indicated in segmental injuries of the tib

Rotationplasty with Tibial Nerve Coaptation: A Case Report.

CASE: We present the case of a 14-year-old adolescent boy with a distal femoral osteosarcoma partially encasing the tibial nerve. He underwent rotationplasty with resection and coaptation (end-to-end repair) of the tibial nerve. By 1 year postoperatively, he had recovered sensation on the plantar aspect of his foot and Medical Research Council scale 4+/5 gastro-soleus contraction that powered extension of the new knee. CONCLUSION: Tibial nerve resection is not an absolute contraindication for rotationplasty, even in an adolescent. Nerve coaptation allows for well-functioning rotationplasty as

Tibial nerve decompression: reliable exposure using shorter incisions.

BACKGROUND: Patients and surgeons recognize the value of procedures that minimize scarring and tissue dissection, but technical standards do not exist with regards to incision lengths needed for tibial nerve decompression. This article introduces reproducible techniques that reliably provide exposure for release of known anatomical compression points of the tibial nerve, while minimizing the length of required skin incisions. METHODS: The senior author's approach to decompression of the tibial nerve at the soleus arch and the tarsal tunnel is presented. Typical incision lengths and surgical e

Results of posterior tibial nerve grafting at the ankle.

The results of interfascicular interposition nerve grafting for posterior tibial nerve deficit at the ankle are reported for eight patients. The sural nerve was the donor nerve in all cases. Mean age at injury was 36.3 years (range 22 to 50 years). Mean postoperative follow-up is 5.0 years (range 2.25 to 8.0 years). In five of the patients, the primary indication for nerve grafting was pain or numbness in degree sufficient to consider amputation. Mean graft length was 9.8 cm (range 4 to 18 cm). After grafting, all eight patients ambulate without assistive devices and are productively employed

Intraneural Lipoma of the Tibial Nerve: A Case Report.

Intraneural lipomas, neurofibrolipomas, lipofibromatous hamartomas, and perineural lipomas are subsets of hamartomas that typically present as fibroadipose, soft tissue masses within the epineurium of a nerve. Several cases involving intraneural lipomas of the median nerve in the upper extremity have been reported; however, owing to the lesion's rare incidence in the foot and ankle, only a select few cases involving the superficial peroneal nerve have been reported. We present the first case of a tibial nerve intraneural lipoma in a 42-year-old female with a follow-up period of 2 years. We dis

Combination of sacral nerve and tibial nerve stimulation for treatment of bladder overactivity in pigs.

PURPOSE: Sacral nerve and tibial nerve stimulation have been singly used to treat overactive bladder (OAB). This study evaluated the effects of both combined stimulation on treating bladder overactivity in pigs and explored a novel treatment modality for OAB. METHODS: An implant-driven stimulator of the S3 spinal nerve was implanted in 5 pigs. The contralateral tibial nerve was stimulated by an external stimulator. Multiple cystometrograms were performed to determine the effects of single nerve stimulation and combination sacral nerve stimulation (SNS) and tibial nerve stimulation (TNS) on th

[Postoperative palsies of the common peroneal nerve and the tibial nerve associated with lateral position].

A 36-year-old woman with systemic lupus erythematous secondary to diabetes underwent right total hip arthroplasty in the left lateral position. An epidural catheter was inserted at the L1-2 interspace and placed cephalad prior to induction of general anesthesia. After the operation, she complained of numbness on the dorsum and plantar of the left foot and was unable to dorsiflex and plantarflex the ankle. The symptoms persisted after discontinuation of the epidural infusion of a local anesthetic, and they became exacerbated after the second operation in the left lateral position. Electromyogra

Compressive neuropathy of the tibial nerve and peroneal nerve by a Baker's cyst: case report.

We report a case of Baker's cyst that induced compression of both the tibial and common peroneal nerves. The patient presented with calf atrophy and foot drop over a 6-month period. These signs and symptoms could have been mistaken for those of spinal origin. Based on an electrodiagnostic study and magnetic resonance imaging, compression of nerves by an asymptomatic Baker's cyst measuring 6x4 cm was confirmed. This cyst communicated with the articular joint which was also associated with a medial meniscal lesion. We treated the patient arthroscopically by performing partial medial meniscectomy

Nerve sheath ganglion of the tibial nerve presenting as a Baker's cyst: a case report.

Nerve sheath ganglion is a relatively rare clinical entity commonly found in the peroneal nerve in the lower limb or the ulnar nerve in the upper extremity. It is rarely found in the tibial nerve. The occurrence of a nerve sheath ganglion in a patient's tibial nerve has been identified. The initial presentation of the tumor mass has been very similar to that of a Baker's cyst, namely a soft undulating popliteal mass. Yet, the case also presented symptoms and signs of tibial nerve compressive neuropathy. We present here a rare case of nerve sheath ganglion of the tibial nerve. Clinical courses

Anomalous origin of the sural nerve in a patient with tibial-common peroneal nerve anastomosis.

Recognition of anomalous innervation patterns may be important in dealing with peripheral nerve injuries. We describe a patient who suffered injury to the common peroneal nerve (CPN) at the knee when he laterally dislocated his knee. Intraoperative nerve conduction studies during surgical exploration revealed that nerve fibers from the tibial nerve crossed over to join the CPN immediately distal to a neuroma incontinuity. The sural nerve was also found to arise totally from the CPN.

Peripheral Nerve Stimulation for the 21st Century: Sural, Superficial Peroneal, and Tibial Nerves.

OBJECTIVE: Peripheral nerve stimulation (PNS) of the lower extremity has progressed significantly over the last decade. From the proof of concept that ultrasound-guided, percutaneous implantation was possible to advances in waveforms, the field has been rapidly evolving. While most nerves in the lower extremity can be PNS targets, consideration must be given to the ergonomics of pulse generator placement, patient comfort, and avoidance of lead migration. For this paper, we examine some of the conditions amenable to lower extremity PNS, review the evidence and history behind PNS for these condi

Minced nerve tissue in vein grafts used as conduits in rat tibial nerves.

INTRODUCTION: Peripheral nerve injuries are encountered frequently in clinical practice. In nerve repair, an end-to-end suture is the preferable choice of treatment. However, where primary closure is not possible, the defect is to be repaired with a nerve graft. METHODS: A total of 21 female Wistar rats weighing 230 to 290 g were used in the study. They were classified into the following 3 groups: (I) nerve graft, (II) vein graft, and (III) minced nerve graft. In group I, after exposure of the tibial nerve, a 1-cm-long nerve gap was created on the tibial nerve, and the defect was repaired epi
