Parameters of heart rate variability can predict prolonged asystole before head-up tilt table test.

BACKGROUND: We aimed to evaluate whether heart rate variability (HRV) could predict prolonged asystole before head-up tilt table test (HUTT) by comparing time domain HRV parameters of patients with type 2B vasovagal syncope (VVS) and patients with other types of VVS. METHODS: Ninety-eight patients who examined with 24-hour Holter electrocardiogram monitoring before HUTT and diagnosed with VVS were enrolled. Patients were divided into two groups in accordance with their VVS type as group 1 (n = 43) consisting of patients with type 2B VVS and group 2 (n = 57) consisting of patients with other t

[Therapy of orthostatic dysregulation. Comparative studies on anti-hypotensive sympathomimetics with a tilt-table test].

In 7 patients suffering from orthostatic hypotension the early behaviour of arterial mean blood pressure was studied in the tilting table experiment under influence of etilefrine, norfenefrine, gepefrine (D-(+)-1-(3-hydroxyphenyl)-2-aminopropan-(2R, 3R)-hydrogentartrat) and of its I-isomere. Significant improvement of early orthostatic disregulation of the arterial pressure could be only observed under gepefrine. Amelioration seems to be induced by sensitization of the pressoreceptors.

The clinical utility of qualitative electroencephalography during tilt table testing - A retrospective study.

OBJECTIVE: To assess electroencephalography (EEG) changes during tilt table testing in syncope and other orthostatic syndromes. METHODS: We retrospectively reviewed consecutive tilt table studies with simultaneous EEG from April 2014 to May 2016 at our center. All patients had video EEG during tilt table. All patients had at least 10 min of head up tilt unless they had syncope or did not tolerate the study. Video EEG was interpreted by epileptologists. RESULTS: Eighty-seven patients met the inclusion criteria. Mean age was 45 years, and 55 were women. Seven patients (∼8%) had syncope during

Head-upright tilt-table testing in evaluation and management of the malignant vasovagal syndrome.

Vasovagally mediated cardiac asystole has been proposed as a potential cause of sudden cardiac death. To assess this possibility and identify characteristics that define patients with vasovagally mediated asystole, head-upright tilt-table testing was performed in 50 consecutive patients (26 women and 24 men, mean age 42 +/- 10 years) with recurrent unexplained syncope. The upright tilt-table test was performed in the fasting state for 30 minutes, with or without the use of intravenous isoproterenol (1 to 3 micrograms/min). The production of ventricular asystole lasting greater than 4 seconds w

Tilt Table Testing in Paediatric Orthostatic Intolerance: Is 10 Min Long Enough?

AIM: Recent cardiovascular literature suggests that orthostatic testing protocols should be tailored according to diagnostic purpose, distinguishing syncope evaluation from assessment of autonomic dysfunction in chronic orthostatic intolerance (COI). In adults, autonomic testing is commonly interpreted within a 10-min observation period, such as the active stand test. However, no specific time-based protocol has been clearly established for paediatric groups. This study examined whether a 10-min head-up tilt table test (HUTT) provides sufficient diagnostic yield in paediatric COI. METHODS: We

Isoproterenol Causing Coronary Vasospasm and ST Elevations During Tilt Table Testing.

Syncope is a sudden but reversible brief loss of consciousness secondary to an acute reduction of cerebral perfusion. Reflex syncope denotes neurologically mediated syncope, which includes vasovagal, carotid sinus syndrome, and other situational syncope. The most frequent form of syncope is vasovagal, which is triggered by emotional stress or prolonged standing, and may be diagnosed with the tilt table test. A thorough investigation of syncope is necessary as serious cardiovascular disorders may also be a cause. A tilt table test is a widely used tool utilized by clinicians to diagnose vasovag

High incidence of ST-T changes in women during tilt-table testing.

INTRODUCTION: We have observed electrocardiographic (ECG) changes primarily in women during tilt table testing. METHODS: We reviewed 12 lead ECGs during tilt studies between 2012 and 2016 for changes in ST segments and T waves during tilt table testing. Patients with distinctly abnormal baseline ECGs were excluded. RESULTS: Of the 180 tilt studies, 117 (65%) were in women. There were 32 patients with ECG changes during tilting. Of these, 28 (87.5%) were in women with an average age of 45years. None had a history of CAD or exertional chest pain. Echocardiograms were available in 21 of the 28

[The tilt-table test: is it essential for adequate treatment of vasovagal syncope? Arguments in favor].

Head-Up tilt test (HUT) permits the identification of vasovagal syncope in patients with syncope of unknown origin. Several authors have pointed out its usefulness for the therapeutic management of these patients. However, even though it has limitations, this paper discusses those situations in which HUT may be useful. First, as HUT permits the recognition of the etiologic diagnosis of syncope, it may be helpful in the evaluation of its treatment. On the other hand, the characterization of different components of vasovagal reaction, cardioinhibition and/or vasodepressor response, may be helpfu

Which is preferable for orthostatic hypotension diagnosis in older adults: active standing test or head-up tilt table test?

BACKGROUND: Correct evaluation of orthostatic hypotension (OH) is crucial in geriatric practice, since OH is associated with mortality and morbidity. The study aimed to determine the most appropriate method for measuring blood pressure in transition from supine to upright position in order to diagnose OH in older adults. METHODS: Active standing test (AST) and head-up tilt table (HUT) test as well as comprehensive geriatric assessment (CGA), including mini-mental state examination or the cognitive state test, mini-nutritional assessment, basic and instrumental activities of daily living, and

A case of orthostatic tachycardia syndrome presenting with panic attack during tilt table testing.

A 26-year-old woman experienced syncope on standing 2-3 times a year for more than 15 years. The attack was typically associated with palpitations and frequently accompanied by a feeling of intense fear. The patient underwent head-up tilt table testing at 70° for 40 min to determine the cause of the syncope. The tilt test results suggested that the etiology of the syncope was orthostatic tachycardia syndrome complicated by panic attack-associated hypocapnic hyperventilation, which presumably caused a greater degree of deep cerebral hypoperfusion than would be expected with orthostatic tachycar

Dynamic changes in T-wave amplitude during tilt table testing: correlation with outcomes.

BACKGROUND: Changes in autonomic tone may play a role in syncope. Autonomic tone has been shown to affect cardiac repolarization in the ECG. Changes in the T wave can be seen during head-up tilt table (HUT) testing with unknown significance or relationship to outcomes. METHODS: Twelve-lead ECGs during HUT testing from 150 patients were reviewed from a prospectively collected registry database. ECGs during supine-rest, 30-45-70 degrees tilt, and 5-minute supine recovery were reviewed. Changes in the T wave, that is, decreased amplitude with or without becoming negative or flipping from negativ

The Evolution of Head-Upright Tilt-Table Testing in Toronto, Ontario, Canada: 2014-2024.

The real-world use of head-upright tilt-table tests (HUTTs) in North America is not known. This single-centre retrospective study in Toronto, Ontario, Canada reports on 1063 HUTTs conducted between 2014 and 2024. Use of the HUTT increased year over year, with a greater increase in HUTTs conducted as a result of a referral diagnosis of postural orthostatic tachycardia syndrome (POTS). Using specific diagnostic criteria for syncope and POTS, patients with either diagnosis who underwent testing, after January 2021 had an almost 50% reduction in the odds of a positive HUTT (syncope: odds ratio: 0.

Head-upright tilt table testing. A safe and easy way to assess neurocardiogenic syncope.

Head-upright tilt table testing has emerged as an exciting technique for evaluating episodes of autonomically mediated syncope. It may be performed using gravity alone as the provocative stimulus or with the addition of isoproterenol. The classic neurocardiogenic response to the test is a sudden drop in blood pressure followed by a decrease in heart rate, but other abnormal response patterns have also been reported. Continuing investigations will improve understanding of autonomic disturbances that cause orthostatic intolerance and will help elaborate the role of tilt table testing in their ev

PR Interval as a Valuable Predictor of Tilt Table Test Results in Patients With Neurally Mediated Syncope.

BACKGROUND: Neurally mediated syncope (NMS) is the primary cause of temporary and self-limiting loss of consciousness. The tilt table test (TTT) has been consistently employed as a supplementary diagnostic tool for syncope evaluation. However, TTT requires specialized equipment, which is lacking in several emergency room and clinic environments. We hypothesized that patients susceptible to NMS may have higher parasympathetic tone. Thus, this study investigates the correlation between PR interval and Herat rate variability parameters as indicators of parasympathetic tone and TTT results. METHO

Safety and tolerability of an aggressive tilt table test protocol in the evaluation of patients with suspected neurocardiogenic syncope.

Safety and tolerability of a one-step tilt table test with high dose (5 micrograms/min) isoproterenol (ISO) without intermediate stages were evaluated in a symptomatic population of 300 patients referred for clinical syncope, near syncope, or dizziness. ISO has been used as a provocative test but remains controversial. A population of 118 male and 182 female patients with a mean age of 45 (range 5-90) years underwent 300 tests. Heart rate and blood pressure were monitored continuously. A positive test was one in which clinical symptoms were reproduced or hemodynamic criteria met. Patients were

A novel tilt table testing protocol for investigating patients suspected to have neurally mediated syncope.

We have observed that a series of 3 Valsalva maneuvers performed at 1-minute intervals between the 15th and 18th minute of a drug-free tilt table test resulted in intense presyncope and hypotension in a 32 yr old male with micturition syncope. In contrast, five apparently healthy subjects tolerated a 70 degree head-up tilt table test for 45 min with five Valsalva maneuvers incorporated into the testing protocol. Thus, we hypothesize that the use of a series of Valsalva maneuvers during a drug free head-up tilt table test might improve the diagnostic yield without compromising test specificity

Electrophysiologic testing in the upright position: improved evaluation of patients with rhythm disturbances using a tilt table.

Patients are traditionally evaluated in the supine position in the electrophysiology laboratory, although symptoms due to a cardiac rhythm disturbance are often maximal clinically during standing. The assumption of the upright position results in dependent displacement of blood, followed by prompt vasoconstriction to maintain arterial pressure. This normal response may aggravate tachyarrhythmias by increasing catecholamine levels or may precipitate vasodepressor syncope if the vasoconstrictor response is absent. The use of a tilt table during electrophysiologic testing was evaluated over a 12
