Growing skin: tissue expansion in pediatric forehead reconstruction.

Tissue expansion is a common surgical procedure to grow extra skin through controlled mechanical over-stretch. It creates skin that matches the color, texture, and thickness of the surrounding tissue, while minimizing scars and risk of rejection. Despite intense research in tissue expansion and skin growth, there is a clear knowledge gap between heuristic observation and mechanistic understanding of the key phenomena that drive the growth process. Here, we show that a continuum mechanics approach, embedded in a custom-designed finite element model, informed by medical imaging, provides valuabl

Nasal augmentation in chondrodysplasia punctata using tissue expansion.

Chondrodysplasia punctata is an uncommon group of congenital bone dysplasias. A common feature of this disease is a characteristic facial appearance including nasal hypoplasia. This paper describes the management of a case presenting with this disease in which soft tissue augmentation was achieved with tissue expansion of the nasal skin. Follow-up of more than 4 years demonstrates that a satisfactory result has been achieved, with no facial scars. The relevant literature is discussed.

Serial Tissue Expansion at the Same Site in Pediatric Patients: Is the Subsequent Expansion Faster?

BACKGROUND: Serial tissue expansion is performed to remove giant congenital melanocytic nevi. However, there have been no studies comparing the expansion rate between the subsequent and preceding expansions. In this study, we analyzed the rate of expansion in accordance with the number of surgeries, expander location, expander size, and sex. METHODS: A retrospective analysis was performed in pediatric patients who underwent tissue expansion for giant congenital melanocytic nevi. We tested four factors that may influence the expansion rate: The number of surgeries, expander location, expander

Pharmacological enhancement of tissue expansion for breast reconstruction.

Tissue expander-based breast reconstruction is widely used following mastectomy due to its relative technical simplicity, minimal additional anesthesia, and reversibility. However, complications such as tissue necrosis, impaired vascularity, postoperative pain, and infections are frequent. Pharmacological interventions have been explored to improve tissue viability and reduce these complications. A literature search was performed in PubMed, Scopus, and Cochrane Library, focusing on clinical and preclinical studies investigating pharmacologic agents applied topically or systemically in tissue e

Cervicofacial Pediatric Tissue Expansion: Aesthetic Unit-Based Algorithm.

BACKGROUND: Tissue expansion is a powerful tool in reconstruction of pediatric soft-tissue pathologies, but complication rates in children have been reported to be as high as 40%. Infection and implant extrusion lead to premature removal and delays in reconstruction. Expanding the head and neck is uniquely challenging because the confluence of facial aesthetic units must be respected. These challenges prompted the senior author (R.J.R.) to create an aesthetic unit-based algorithm. METHODS: A retrospective study of pediatric patients who underwent cervicofacial tissue expander placement over a

Tissue expansion in limbs vs other body regions.

INTRODUCTION: The development of tissue expansion technique as an important routine procedure in plastic and reconstructive surgery in the past two decades reflects the tendency of getting quality skin that would replace the defect. The aim of this study was to present our experience in the application of tissue expanders in the limbs, and to compare the complications of this procedure in limb and non-limb sites. MATERIAL AND METHODS: This retrospective study included 16 expanders in 14 patients. Indications for tissue expansion were post burn scarring and contractures, post mastectomy recons

[Application of quantitative tissue expansion in ear reconstruction].

OBJECTIVE: To explore the application of tissue expander in ear reconstruction and to deal with the complications. METHODS: 50 ml kidney-shape tissue expanders were implanted subcutaneously in the mastoid area. The drainage tube was removed 3 days after operation. The suture was removed 10 days later. Since 7 days after operation, 5 ml NS was injected into the expander every time, three times a week. The total injection volume was about 60 ml. After that, the expander was maintained for one month. RESULTS: From January 1992 to December 2006, 5,248 patients of microtia were treated with 6,252

External tissue expansion in head and neck reconstruction.

UNLABELLED: Tissue expansion has been of great value in plastic surgery especially in head and neck reconstruction. This study used external tissue expansion in 40 patients for head and neck reconstruction to avoid the complications associated with internal expansion. The study included 40 patients suffering from lesions in the head and neck, which needed excision and reconstruction. We designed external tissue expanders of different sizes and shapes to be suitable for head and neck application. The external expander was applied to the normal skin on one or both sides of the lesion and a negat

External tissue expansion successfully achieved using negative pressure.

The practice of internal tissue expansion by implanting a silicone balloon into the soft tissue under the skin is now known to be associated with a significant risk of complications. Therefore, we designed and developed a new technique of mechanical external tissue expansion achieved by using negative pressure created through special valved cups. The subjects were 60 patients suffering from postburn scar contractures in different parts of their bodies. A valved cup was applied to the healthy skin on both sides of the scar and negative pressure of -100 to -200 mbar was connected to the cup thro

[Labia minora skin flap vaginoplasty using tissue expansion].

OBJECTIVE: This is to introduce a method for vagina reconstruction using the expanded labia minora flap. METHODS: Two tissue expanders were implanted in the labia minora bilaterally and expanded slowly over 4 weeks. In the operation, the expanded labial tissue was advanced as a bipedicle flap to line the reconstructed vagina. Five patients were treated with this method. Postoperative stent placement and dilation resulted in a vaginal canal exceeding 8 cm in depth. RESULTS: During the follow-up of 6 months to 2 years, four of the five patients got married. The vulva exhibited almost indisting

[Facial profile improving interventions using tissue expansion techniques].

If there is a lack of soft tissue in the face, no matter what the cause, the reconstruction should be performed using tissue from neighboring areas because tissue in the same aesthetic and functional area guarantees the best results. Two methods of dilation are described for such skin expansion explained by examples, and substantiated by statistics: (1) dilation with the aid of osteosynthesis plates; and (2) dilation with tissue expansion. To make an objective evaluation, only cases where considered that were finished 3 years ago.

Increased sensitivity in objective monitoring of tissue expansion.

The end point for incremental filling of any tissue expander normally may reliably be assessed using subjective parameters, especially pain, as a guideline. However, in unusual circumstances, such as within denervated areas, the presence of marginal skin circulation, or if rapid expansion is required, an objective monitor may be beneficial to ensure safer expansion. We have previously examined the role of serial measurement of transcutaneous oxygen in this regard. In an effort to refine this sometimes cumbersome approach, a comparison using laser Doppler flowmetry as well has been undertaken i

Tissue expansion in reconstructive hand surgery: case report.

A 22-year-old musician requested the correction of his digital syndactyly deformity without the use of skin grafts. Failure of the original correction to heal primarily resulted in the formation of dense scar tissue. A small, custom-designed tissue expander was inserted within the web space and was slowly expanded. The additional expanded tissue permitted reconstruction of the web space and the sides of the fingers without the use of skin grafts.

Intraorbital tissue expansion in the management of congenital anophthalmos.

Seven cases of intraorbital tissue expansion for the treatment of congenital anophthalmos or microphthalmos are presented. The ages of the patients at insertion of the expander ranged from 4 months to 8 years. A 4 ml spherical tissue expander with a remote injection port was inserted into the affected orbit via a bicoronal approach. Expansion periods ranged between 4 months and 3 years and are continuing in 2 patients. Results were assessed by clinical examination, comparison of photographs, 3D CT scans and orbital measurements taken from axial CT scans which were compared with established nor

Applications and complications of tissue expansion in pediatric patients.

We have reviewed our experience with 61 tissue expansions in 49 pediatric patients. Indications for expansion included traumatic alopecia or soft tissue loss, myelomeningocele, congenital hairy nevi, burn scars, and other less common defects. The complication rate, necessitating interruption of expansion, was 25%. This occurred more frequently with scalp and burn reconstruction. Nevertheless, 86% of patients ultimately achieved good to excellent results.

A protocol for tissue expansion in upper extremity reconstruction.

Twenty tissue expanders were used in twelve patients for reconstruction of the upper extremity after trauma or burns with extensive soft tissue loss. A protocol, designed to avoid complications, included defects not exceeding thirty percent of the arm circumference, use of low-profile expanders, and slow expansion. Attention to these details avoided all complications except for transient neuropathies.

Failed tissue expansion in a patient with osteogenesis imperfecta.

This is a report of a failure of tissue expansion in a 43-year-old patient suffering from a mild form of osteogenesis imperfecta tarda. Filling of the expander was not possible. Skin histology showed thrombosis of the blood vessels and vessel wall necrosis. This finding supports the theory that tissue expansion does not depend as much on mechanical skin laxity as on the proliferative potential of the skin.

Tissue expansion: a promising trend for reconstruction in urology.

To evaluate the role of the tissue expansion concept in reconstructive urology, the contemporary literature was reviewed, together with our own results with tissue expansion. The principle of tissue expansion has been applied in cases of bladder augmentation, ureteral dilation for the generation of indigenous tissue and subsequent ureterocystoplasties, ureteral elongation, and dilation of the renal pelvis, producing native tissue for the reconstruction of defects or strictures of the upper ureter. Even though the exact mechanism of action of mechanical strain in different tissues is not known,

Tissue expansion of the facial nerve in an animal model.

The usefulness of tissue expanders in reconstructive surgery has been well established for skin, muscle, and fascia. There has been only anecdotal examination of tissue expansion techniques in peripheral neural tissue. Segmental sacrifice of cranial nerves is at times required in head and neck surgery. Nerve lengthening for re-anastomosis after segmental resection would be a valuable technique. This study used tissue expansion techniques in a porcine model to explore its effects on the peripheral facial nerve. Inflation of the expander in all animals resulted in no clinical signs of neural deg
