Secondary centers of ossification of the human toes: exceptional polymorphism and evolutionary perspectives.

As great morphological variability characterizes the phalanges of the human toes in adults, we hypothesized for a possible variability in the presence or absence of their secondary (= epiphyseal) centers of ossification linked to the unique morphology of the human foot within primates. The aim of this study was thus to provide original and detailed data on the occurrence of these centers. Classically, the big toe or hallux (I) presents two secondary centers and the lateral toes (II-V) three centers, and consequently the five toes present a total of 14 secondary centers. The material studied co

Dactylitis Due to Subcutaneous Sarcoidosis as a Manifestation of Sarcoidosis.

Dactylitis, or sausage fingers or toes, refers to the swelling of 1 or more digits. Sarcoid dactylitis is an uncommon form that tends to be caused by granulomatous bone involvement in the underlying phalanges. Sarcoid dactylitis can, however, clinically resemble subcutaneous sarcoidosis with finger or toe involvement. We describe 5 patients with dactylitis due to subcutaneous sarcoidosis of the fingers. In all cases, the proximal phalanges were involved; sarcoid dactylitis, by contrast, tends to affect the medial and distal phalanges. Differential diagnosis is important, as dactylitis due to b

Unusual localizations of unicameral bone cysts and aneurysmal bone cysts: A retrospective review of 451 cases.

Unicameral bone cysts (UBC) and aneurysmal bone cysts (ABC) are benign cystic lesions of bone which are easily diagnosed. However, unusual locations may lead to a false diagnosis. Therefore the aim of this retrospective study was to determine the frequency of unusual localizations. The authors studied 451 cases with histopathologically confirmed diagnosis of UBC or ABC, seen between 1981 and 2012. In the UBC group (352 cases) humerus, femur and calcaneus were found to be the most common sites, while acetabulum, scapula, scaphoid, lunatum, metacarpals, metatarsals, toe phalanges and ulna each a

Effects on toes from prenatal exposure to anticonvulsants.

BACKGROUND: Changes in the distal phalanges of the fingers, including coned epiphyses and hypoplasia of the phalanges, are recognized teratogenic effects of the anticonvulsant drugs phenytoin and phenobarbital. We hypothesized that the frequency of these changes would also be increased in the toes of children exposed to these drugs in comparison to unexposed children. METHODS: We report on the findings in an analysis of radiographs of the feet of 63 children exposed in utero to either phenytoin alone, phenobarbital alone or both drugs and 56 unexposed comparison children. RESULTS: Only subtl

Congenital Curved Nail of the Fourth Toe with Subungual Hyperkeratosis.

Congenital curved nail of the fourth toe (CNFT) is a rare nail deformity. We report a case of CNFT with thickening and hyperkeratosis at the hyponychium and peripheral nail bed that was successfully treated using a novel surgical technique. A 7-year-old Japanese boy had a clawed nail with thickening and hyperkeratosis at the hyponychium and peripheral nail bed on his bilateral fourth toes. His chief complaint was pain. The nails of the bilateral fourth toes curved plantarly and exhibited thickening and hyperkeratosis of the hyponychium and peripheral nail bed. A surgical approach was planned t

An autopsy case of Adams-Oliver syndrome.

We report an autopsy case of a male fetus with Adams-Oliver syndrome. His mother was a healthy, 31-year-old woman and her family and past histories were unremarkable. Therapeutic termination was done at 28(+6) weeks gestational age due to oligohydramnios detected by antenatal ultrasonography. Chromosomal study revealed normal karyotype. On autopsy, characteristic transverse terminal defect of four extremities was found. Both feet were short and broad. All toes were rudimentary with no nails and fingers were irregularly short. On infantogram, all toe-bones were stubby and rudimentary. The middl

Congenital Quintus Valgus: An Extremely Rare Anomaly: A Case Report.

CASE: We present an unusual case of unilateral congenital quintus valgus of the left foot (congenital lateral deviation of the fifth [little] toe) in a 1.5-year-old boy. The child had no other congenital malformations. Radiographs revealed the abnormal presence of 2 accessory phalangeal ossicles in the fourth web space, leading to lateral deviation of the fifth toe. Remarkably, the ossicles of the middle and distal phalanges of the fifth toe were absent. Surgical excision of the accessory ossicles was performed, and correction of the deformity was achieved. CONCLUSION: Congenital quintus valg

Osteoid osteoma of the phalanx: enlargement of the toe--two case reports.

After a review of the literature, two cases of osteoid osteoma involving phalanges of the toes are reported. Physical findings of osteoid osteoma vary with the site of the tumor; in the foot, particularly the toes, it presents various atypical and characteristic features. Its more frequent cancellous and subperiosteal localization in the small bones of hand and foot, showing atypical roentgenographic findings, poses particular problems in diagnosis. In the osteoid osteoma of the phalanges, characteristic enlargement of the toe or finger seems to be frequent and with a typical clinical history

Diphenylhydantoin teratogenicity in man.

A case is presented of fetal teratogenic damage from an unusually high maternal dosage of diphenylhydantoin, 600 mg daily, to a 40-kg woman throughout pregnancy. The anomalies included narrowed ends of the fingers and toes; aplasia of the nails on the other fingers and toes; aplasia of the distal phalanges of the fingers and toes; a broad, depressed nasal bridge; anteverted nostrils; prominent upper lip; coarse, profuse scalp hair; pilonidal sinus; and simian crease of the left hand. At the age of 7 months, ossification of the distal phalanges of both middle fingers appeared, the anterior font

Rare case of tibial hemimelia, preaxial polydactyly, and club foot.

A seven-month old female presented with left tibial hemimelia (or congenital tibial aplasia; Weber type VIIb, Jones et al type 1a), seven-toed preaxial polydactyly, and severe club foot (congenital talipes equinovarus). Definitive amputation surgery disarticulated the lower limb at the knee. This case report describes the anatomical findings of a systematic post-amputation examination of the lower limb's superficial dissection, X-rays, and computed tomography (CT) scans. From the X-rays and CT scans, we found curved and overlapping preaxial supernumerary toes, hypoplastic first metatarsal, lac

Biphalangeal/triphalangeal fifth toe and impact in the pathology of the fifth ray.

BACKGROUND: Having reviewed the studies on the biphalangeal fifth toe, we have observed a great disparity of data depending on the research center. We have investigated the frequency of biphalangeal toes and also its handedness. We have also analyzed the relationship of pathological deviations of fifth toe with this feature and with the fifth metatarsal. METHODS: We performed a descriptive prospective study, which analyzed 2494 feet (1247 people) with bilateral dorsoplantar radiographs. We studied the number of phalanges of the fifth toe, the deviations in the sagittal and transverse plane, a

Qualitative and quantitative analysis of talons of diurnal bird of prey.

The aim of this work is to establish qualitative and quantitative differences and similarities among talons lacking the keratin sheath in diurnal raptors. We set out to find a tool that allows for the determination of toe (I-IV) and hindlimb (left or right) identity. Different methodologies were used, such as classical anatomical descriptions and Fourier analysis of the medial contour of the ungual phalanx. A series of measurements and ratios were obtained and analysed using t-tests and principal components analysis. The qualitative descriptors are more important than quantitative analyses for

Technique of Nonvascularized Second Metacarpal Transfer in the Treatment of Symbrachydactyly.

Various surgical techniques have been utilized for thumb reconstruction in the setting of aphalangia. Given abnormal proximal anatomy, nonvascularized toe phalangeal transfer has been a popular surgical technique in a subset of patients with monodactylous symbrachydactyly. However, altering the appearance of a noninvolved foot is often unappealing to many parents. Here we detail a surgical technique of thumb reconstruction utilizing the index metacarpal as a nonvascularized graft to improve thumb length and stability. This technique avoids the donor site morbidity associated with toe phalangea

[Isolated macrodactyly or extremely circumscribed Proteus syndrome?].

We report on two unrelated female patients with unilateral macrodactyly of the 1st and 2nd toes and of the 3rd, 4th, and 5th toes, respectively. In addition, they had a local plantar soft tissue lump and radiographically abnormally broad phalanges of the affected toes. These clinical manifestations may represent either an isolated macrodactyly or an extremely localized form of Proteus syndrome.

Macroscopic and histological analysis of the medial and lateral collateral ligaments of the distal phalanges of the foot.

BACKGROUND: Given the conflicting reports in the literature regarding the presence of ligaments in the distal phalanx of the foot, this study aimed to verify the existence of ligamentous structures in this region. Confirmation of these structures is clinically important because numerous pathological lesions have been reported in the lateral regions of the distal phalanges, such as exostoses and osteophytes, which may cause discomfort to patients. MATERIALS AND METHODS: The study was conducted on 10 feet (5 from male and 5 from female donors) obtained from fresh-frozen human cadavers. The dono

Undergrowth: brachydactyly.

Brachydactyly simply means "short finger", but it has a clinical spectrum ranging from minor digital hypoplasia to complete aplasia. The index and small fingers are the most commonly affected digits, and the middle phalanx is the most commonly affected bone. Presentation of brachydactyly is highly variable-a digit may be just slightly shorter because of reduced growth of one phalanx or the metacarpal, or there may be complete absence of one or more phalanges, or there may be complete aplasia of the digit or multiple digits. This article discusses the results of conventional reconstruction usin

Duplicated distal phalanx of thumb or hallux in trisomy 13: A recurrent feature in a series of 42 fetuses.

Trisomy 13 or Patau syndrome (PS) is a well-known aneuploidy characterized by a polymalformative syndrome. We described a large series of fetuses with PS and compared them with cases described in the literature, most of which were live-born. In all, 42 fetuses, aged from 14 to 41 gestational weeks (GW), were examined. The main defects observed were similar to those described in live-born patients: congenital heart defects (76%), holoprosencephaly spectrum anomalies including arhinencephaly and hypotelorism (74%), urinary tract anomalies (71%), ear anomalies (69%), postaxial polydactyly (67%),

Phalangeal joints kinematics in ostrich (Struthio camelus) locomotion on sand.

In ostriches, the toes are the only body parts that contact loose sand surfaces. Thus, toe interphalangeal joint motions may play vital biomechanical roles. However, there is little research on ostrich phalangeal joint movements while walking or running on sand. The results from the three-dimensional motion track analysis system Simi Motion show that gait pattern has no significant effect on the key indicators (angles at touch-down, mid-stance, lift-off and range of motion) of the toe joint angles. The motion of the toe phalanges when walking and running on sand is basically the same. The grou
