Failure after closed traction of an unerupted maxillary permanent canine: Diagnosis and treatment planning.

This report describes the treatment of a 13-year-old girl with unerupted maxillary permanent canines. It illustrates how recognizing an unexpected problem influenced the decision-making process. Despite 6 months of closed-eruption traction, the left canine had not erupted. However, the neighboring teeth were intruded, suggesting a diagnosis of canine ankylosis. When the site was surgically reopened, the wire chain used for the orthodontic traction appeared to be osseointegrated. It was renewed, and traction was applied for another 16 months, and the tooth was successfully brought into the arch

Feasibility and Outcomes of Cell-based Regenerative Endodontic Therapy in Postautogenous Transplantation of a Mature Tooth: A Case Report.

Autogenous tooth transplantation (ATT) offers advantages; however, success rates depend on factors like socket formation, donor tooth manipulation, and endodontic treatment. Root canal treatment in a mature post-ATT tooth remains challenging. Cell-based regenerative endodontic therapy (RET) shows promise for regenerating the pulp-dentin complex in mature teeth. However, its application in a post-ATT tooth is unexplored. This case report demonstrates the feasibility and outcomes of cell-based RET in a post-ATT tooth. A 37-year-old male underwent ATT for a missing maxillary left first molar. The

Idiopathic ankylosis-resorption: diagnosis and treatment.

Dental ankylosis involves the fusion of root to bone. The idiopathic form occurs spontaneously before the eruption of the tooth concerned. Etiology is unknown. As the tooth becomes part of the bone-remodeling process, it is gradually resorbed and is replaced by bony tissue. The process is quite rapid and weakens the tooth. Thus, early diagnosis makes it possible to perform a graft in optimal conditions, this being the only means of severing the ankylosis and achieving adequate eruption. In some rare cases, the ankylotic point of fusion can be reached surgically and can be eliminated before ort

Central incisor loss after delayed replantation following avulsion: a contemporary restorative and adjunctive orthodontic management approach.

Delayed replantation of an avulsed tooth may result in rapid root resorption or, more frequently, dental ankylosis with subsequent bone substitution. If this process develop slowly, it is possible to observe that tooth loss is characterized by a well conserved alveolus with regard to bone preservation, particularly in vertical dimension. This clinical case reports a dental trauma of a central incisor in a young boy characterized by tooth avulsion and its delayed replantation. After 10 years, dental ankylosis of the incisor was recorded and the patient underwent a prosthetic-orthodontic rehabil

The etiology of eruption disorders - further evidence of a 'genetic paradigm'.

The clinical spectrum of tooth eruption disorders includes both syndromic and non-syndromic problems ranging from delayed eruption to a complete failure of eruption. A defect in the differential apposition/resorption mechanism in alveolar bone can cause conditions such as tooth ankylosis, primary failure of eruption, failure of eruption due to inadequate arch length and canine impaction. As our knowledge of the molecular events underlying normal tooth eruption has increased, so too has our understanding of clinical eruption disorders. The recent finding that one gene, parathyroid hormone recep

Prognostic Factors for Clinical Outcomes in Autotransplantation of Teeth with Complete Root Formation: Survival Analysis for up to 12 Years.

INTRODUCTION: Tooth autotransplantation is a treatment option that has the potential to restore masticatory function and esthetics to edentulous spaces resulting from extracted teeth. The purpose of this study was to investigate the prognostic factors and clinical outcomes for autotransplanted teeth with complete root formation. METHODS: Patients who had received tooth autotransplantation in the Department of Conservative Dentistry, Yonsei University Dental Hospital, Seoul, Korea, from July 2001 to August 2010 were electronically searched, and a total of 105 cases of autotransplanted teeth me

The Role of Empathy in Professional Quality of Life: a Study on Australian Police Officers Working in Sexual Assault and Child Abuse Investigation.

Extant and extinct reptiles exhibit numerous combinations of tooth implantation and attachment. Tooth implantation ranges from those possessing roots and lying within a socket (thecodonty), to teeth lying against the lingual wall of the jawbone (pleurodonty), to teeth without roots or sockets that are attached to the apex of the marginal jawbones (acrodonty). Attachment may be ligamentous (gomphosis) or via fusion (ankylosis). Generally speaking, adaptative reasonings are proposed as an underlying driver for evolutionary changes in some forms of tooth implantation and attachment. However, a su

Caiman periodontium as an intermediate between basal vertebrate ankylosis-type attachment and mammalian "true" periodontium.

The teeth of many fish, amphibia, and reptiles are attached to the alveolar bone via ankylosis. In contrast, mammalian periodontia are characterized by a gomphosis, an attachment of the tooth root in the alveolar bone socket via periodontal ligament fibers. Among the reptiles, the crocodilians are the only group featuring a gomphosis-type connection between tooth root and alveolar bone, while in other reptiles tooth-root and jawbone are connected via ankylosis. The purpose of the present study was to compare several key features of the crocodilian periodontium with those of the mammalian and n

The effect of orthodontic extrusion on traumatically intruded teeth.

The management of traumatically intruded permanent incisors is controversial. Some authors suggest a decreased incidence of ankylosis in cases treated with orthodontic extrusion. The purpose of this study was to examine two common management techniques for traumatic intrusion, orthodontic extrusion, and observation for re-eruption. The four first premolars of three shepherd dogs were traumatically intruded with a mallet while a holding device was used to prevent tooth fracture. Five to 7 days following the injury, orthodontic force was applied unilaterally while the contralateral tooth served

Bone morphogenic protein-2 induces apoptosis and cytotoxicity in periodontal ligament cells.

BACKGROUND: Periodontal ligament (PDL) expresses endogenous growth factors, such as bone morphogenic proteins (BMPs), which facilitate maintenance of tissue homeostasis. Inflammatory conditions, such as chronic periodontitis, could disrupt this homeostasis, and physiologic levels of growth factors may be insufficient to maintain tissue homeostasis. BMPs facilitate periodontal bone regeneration but also are implicated in causing tooth ankylosis and root resorption. The underlying mechanism of tooth ankylosis is unclear. However, there is evidence that BMPs induce apoptosis in progenitor cells.

[Diagnosis of ankylotic deciduous molars using intraoral scans in oligodontic patients].

In this retrospective study, 2 intraoral scans with an interval of at least 1 year were superimposed in 25 oligodontic patients. The differences in vertical eruption (mm) were measured and the orthopantomograms were analyzed for the presence of ankylotic deciduous molars with no successor. The mean eruption of deciduous molars with and without successor was significantly lower than the mean eruption of permanent molars. The eruption of permanent molars was a predictive variable for the eruption of deciduous molars. The number of agenetic elements and the presence of a successor were strongly a

Treatment of replacement resorption with Emdogain--preliminary results after 10 months.

Ankylosis of traumatized teeth in children and adolescents may inhibit further development and growth of the corresponding jawbone. Therefore, ankylosed teeth should be removed. As an alternative treatment option to autotransplantation of a premolar, intentional replantation using Emdogain may be considered, provided the ankylosis is detected at an early stage or has only affected a small area of the root. Eleven ankylosed teeth presenting with replacement resorption were treated as follows: after tooth extraction, the root canal was treated extraorally and obturated by retrograde insertion of

Ankylosis of primary molar along with congenitally missing first permanent molar.

Ankylosis of primary mandibular molars has been routinely found to be associated with various developmental disturbances in permanent dentition such as aplasia of the succedaneous tooth, ectopic eruption of the premolar, infraoclusion of the ankylosed tooth leading to tipping of the first permanent molar etc. This article describes a rare case where there was ankylosis of a mandibular second primary molar along with congenitally missing first permanent molar which resulted in the transposition of second premolar. Treatment options and prognosis of the case are discussed.

Algorithm for the differential diagnosis of posterior open bites: two illustrative cases.

PURPOSE: Posterior open bites can result from several causes: trauma, degenerative changes, tongue thrust habits, single-tooth ankylosis, multiple-tooth ankylosis, and/or condylar hyperplasia. Occasionally, posterior open bites are secondary to a combination of condylar hyperplasia and dental ankylosis, which can be difficult to diagnose and treat because of the large array of causative problems. MATERIALS AND METHODS: This article presents 2 unusual cases of posterior open bite secondary to multiple etiologies in adolescent male patients. A useful method of algorithmic diagnosis, treatment,

Challenges in treating traumatically intruded and ankylosed permanent incisors: a case report with a multidisciplinary approach.

This report discusses the challenges, complications and management of traumatic intrusion injuries affecting the permanent dentition. A case is described where trimming of the incisal edge of a severely intruded and ankylosed upper lateral incisor resulted in an unexpected re-eruption of the tooth. It is suggested that the vibrations from drilling may have disrupted the ankylosis, initiating spontaneous re-eruption and this approach could provide a minimally invasive and conservative pathway to treating ankylosed teeth. Clinical Relevance: To date no effective treatment has been described to r

Autotransplantation of maxillary second premolars to mandibular recipient sites where the primary second molars were impacted, predisposes for complications.

OBJECTIVE: The aim of the study was to elucidate whether ankylosis of a transplanted permanent tooth can be predicted based on an evaluation of the primary and permanent dentitions. MATERIALS AND METHODS: The study comprised 162 maxillary second premolars and 49 third molars transplanted to the mandibular second premolar region in 157 patients treated in the period 1980-2003. SELECTION CRITERIA: panoramic radiograph available; agenesis of the mandibular permanent premolar/s; persistence of the primary molar at the recipient site. The 211 transplanted teeth were observed for a period of betwe

Effectiveness of decoronation technique in the treatment of ankylosis: A systematic review.

BACKGROUND: Dentoalveolar ankylosis in growing patients is complex leading to continuing root replacement resorption, tooth infra-position, or may even affect the development of alveolar ridge and adjacent teeth. While extraction of ankylosed teeth might be associated with bone loss, decoronation of the offending tooth (removal of crown portion and instrumentation of pulp canal to stimulate bleeding) has been suggested as a more conservative approach of bone preservation until definitive implant placement is planned. OBJECTIVE: To primarily assess the efficacy of bone width and height preserv

Development of ankylosis in permanent incisors following delayed replantation and severe intrusion.

Ankylosis is a predictable outcome for replanted incisors and for most severely intruded incisors. There is no treatment to arrest or reverse this periodontal ligament complication. Ankylosis of the incisors of preadolescents alters local alveolar growth and eventually produces tooth loss subsequent to resorption. Currently, clinical methods used to diagnose ankylosis in the early postinjury period include subjective assessments of percussion sound and mobility and quantitative devices such as the Periotest. This paper describes the progression of ankylosis in two preadolescent patients that s

[Decoronation: indications and treatment timing].

Dental trauma in children is a frequent event and may involve functional, esthetic and orthodontic complications. Replacement resorption and ankylosis are frequently diagnosed following avulsion and replantation, severe intrusive luxation, and less frequently as a consequence of lateral luxations and root fractures. The optimal treatment for an ankylotic permanent incisor is yet to be found. The treatment of ankylosis and replacement resorption of a permanent incisor following dental trauma is further complicated when it involves a growing child. It is often associated with increasing infrapos

An eleven-year case report of an avulsed maxillary central incisor after delayed replantation.

A 12 year-old girl presented with an avulsed maxillary central incisor for a trauma occurred two days earlier. The avulsed tooth was endodontically treated and replanted. Regardless of the ankylosis and replacement root resorption the tooth was retained for a very favourable nine-year period after trauma. In addition no severe infraposition occurred. When the extraction was necessary, despite the slight atrophy, as a result of compromised alveolar bone development due to ankylosis, an implant-supported crown was placed as definitive rehabilitation without recourse to bone augmentation procedur
