Dental erosion. Definition, classification and links.

An overview of tooth wear, i.e. of non-carious destructive processes affecting the teeth including abrasion, demastication, attrition, abfraction, resorption and erosion is presented. The nomenclature and classification of dental erosion commonly used in the dental literature are summarized. They are based on etiology (extrinsic, intrinsic, idiopathic), on clinical severity (Classes I to III), on pathogenetic activity (manifest, latent) or on localization (perimolysis). Interactions between erosion and abrasion, demastication, attrition, and abfraction as well as caries and low salivary flow r

Surgical extrusion of a cervically root-fractured tooth after apexification treatment.

A case is reported in which an incisor fractured below the alveolar crest 6 months after completion of apexification treatment was surgically extruded for prosthetic coronal restoration. After the surgical procedure, a dowel post was placed in the root canal, a core was built using glass-ionomer cement, and a porcelain veneer crown restoration was completed. The 24-month follow-up examination after surgical, endodontic, and prosthetic treatments showed that the tooth was clinically and radiographically healthy and functioned well.

The cervical wedge-shaped lesion in teeth: a light and electron microscopic study.

BACKGROUND: The cervical non-carious wedged-shaped lesion is controversial in that its aetiology may involve attrition, erosion, abrasion and stress-corrosion (abfraction). This study examined the histopathology of anterior teeth with cervical wedge-shaped lesions by light and electron microscopy to elucidate their pathogenesis. METHODS: Ten undecalcified human teeth with cervical lesions were available for investigation. Patency of the dentine tubules was tested using red dye penetration from the pulp chamber. The morphology of normal and sclerotic dentine adjacent to the cervical wedge-shap

The biomechanics of tooth strength: testing the utility of simple models for predicting fracture in geometrically complex teeth.

Teeth must fracture foods while avoiding being fractured themselves. This study evaluated dome biomechanical models used to describe tooth strength. Finite-element analysis (FEA) tested whether the predictions of the dome models applied to the complex geometry of an actual tooth. A finite-element model was built from microCT scans of a human M3. The FEA included three loading regimes simulating contact between (i) a hard object and a single cusp tip, (ii) a hard object and all major cusp tips and (iii) a soft object and the entire occlusal basin. Our results corroborate the dome models with re

Cervical enamel projections from a periodontal perspective: A scoping review.

Cervical enamel projections (CEPs) represent a unique developmental and anatomical anomaly wherein the enamel structure extends apically beyond the cemento-enamel junction of the tooth. In this scoping review, the existing literature on CEPs was evaluated to delineate their characteristics, prevalence, predilection for specific teeth and surfaces, clinical significance, and management approaches. Searches were conducted on MEDLINE (PubMed), Cochrane Library, and Embase databases using the keywords "enamel projection(s)" or "ectopic enamel." In total, 24 studies meeting inclusion criteria were

Investigation of the presence of Non-carious cervical lesions (NCCLs) in ancient adult skulls: analyzing data from prehistoric and historical samples through a systematic review and meta-analysis.

OBJECTIVES: Historically, the prevalence of caries has undergone significant changes, particularly increasing with the industrialization of sugar consumption. When examining ancient populations, lower caries rates are discovered, attributed in part to dietary factors. These populations consumed abrasive foods, leading to occlusal wear and reduced non-axial occlusal forces, potentially influencing Non-Carious Cervical Lesions (NCCLs). Although some attribute NCCLs to abfraction, the mechanism remains debated. This systematic review aims to evaluate the presence of NCCLs in ancient populations,

Multiple idiopathic cervical root resorption: Case report of an unusual presentation.

Idiopathic cervical root resorption (ICRR) is a rare condition involving the cemento-enamel junction of the teeth. Multiple idiopathic cervical root resorption (MICRR) involves several teeth with an unknown prevalence and aetiology. The resorptive lesions are often detected on routine radiographs or during clinical examination. The defects are often painless, hence they are usually diagnosed late and in an advanced stage when diagnosed clinically. The restoration of MICRR can be demanding with subsequent loss of the teeth affected. In this report, we describe the case of a 16-year-old female w

Prognostication of External Cervical Resorption Based on Heithersay's and Patel et al. Classifications: A Narrative Review.

BACKGROUND: External cervical resorption (ECR) is a relatively rare form of tooth resorption. It is commonly classified either using two-dimensional imaging according to the Heithersay or cone-beam computed tomography using the Patel et al. CLASSIFICATIONS: Effective disease classification systems should provide categories that guide clinical decision-making and support the prediction of management outcomes. This principle should apply to the classification systems for ECR. OBJECTIVES: To assess the association between Heithersay's and Patel's classification systems and the recently proposed

Clinical Remarks on the Significance of Tooth Malposition and Papillae Dimension on the Prediction of Root Coverage.

When it comes to complete root coverage of exposed root surfaces, several limiting factors have been suggested. Although tooth malposition and papillae dimension are capable of influencing root coverage, they have not received sufficient emphasis in the literature. Therefore, the aim of the present commentary is to discuss the impact of tooth malposition and papillae dimension on root-coverage outcomes. This commentary combines evidence from the literature with the authors' experience. Limited evidence is available in the literature regarding the influence of tooth malposition on root-coverage

The use of cone beam computed tomography in the management of external cervical resorption lesions.

AIM: To report the use of cone beam computed tomography in the assessment of external cervical resorption lesions. SUMMARY: Asymptomatic external cervical resorption lesions were diagnosed radiographically in two patients. Clinical examination in both cases was unremarkable. Cone beam computed tomography scans revealed the true nature of the lesions in three dimensions. The resorption lesion in case 1 was confined predominantly to the buccal aspect of the root, the lesion had not perforated into the root canal. A mucoperiosteal flap was raised to gain access to the lesion, the resorptive lesi

An updated evidence-based recommendation for the treatment of gingival recession associated with non-carious cervical lesions.

BACKGROUND: Approximately 50% of the gingival recessions (GRs) are associated with non-carious cervical lesions (NCCLs), forming what are known as combined defects (CDs), which often require specific treatment protocols that differ from those used for GRs on teeth with an intact and sound surface. This narrative review aims to evaluate the current and relevant literature on the treatment of CDs and update the latest decision-making process with the new literature. METHODS: Medline/PubMed, Embase, BIREME, and Google Scholar databases were searched. The NCCL portion of the CDs was classified ac

Evidence for the occurrence of gingival recession and non-carious cervical lesions as a consequence of traumatic toothbrushing.

AIM: To identify the best available evidence for the effect of toothbrushing on the initiation and progression of gingival recession and non-carious cervical lesions (NCCLs). METHODS: A protocol was developed for the questions: Does traumatic toothbrushing, compared to normal toothbrushing, lead to an increased prevalence of non-inflammatory gingival recession? [FQ1] and NCCLs? [FQ2]. The search covered four electronic databases. Bibliographies of review articles, relevant texts, World and European Workshops were screened. Hand searches were performed of the Journals of Clinical Periodontolog

Gingival recession. Reappraisal of an enigmatic condition and a new index for monitoring.

Gingival recession (GR) is an intriguing condition. This brief communication, after proposing a definition for GR, considers the possible relevance of anatomical, physiological, pathological and traumatic factors in its etiology. It is probable that no one factor in isolation leads to the development of GR. Because of the possible influence of several factors, not necessarily acting synchronously, the occurrence of GR at a given site may be difficult to explain fully, and any subsequent changes may be hard to predict. The importance of the CEJ (or other fixed point) in assessment of GR severit

BEWE 2.0: Basic erosive tooth wear examination revisited: Introducing an additional level for more severe erosive tooth wear.

The Basic Erosive Wear Examination (BEWE) was proposed in 2008 as a screening tool for tooth wear. The original aim was to create a simple, convenient screening tool to prompt a clinician to record tooth wear. It was not intended to be accurate and was matched to the basic periodontal examination (BPE). Since that time it has been incorporated into dental practice software and has been adopted by Dentists. But the original screening tool created a ceiling effect, particularly at grade 3 which marked a significant progression from slight wear over the surface to almost complete obliteration of

The flaws in tooth root resorption assessment algorithms: the role of source position.

OBJECTIVES: The purpose of this project was to analyse the effects of X-ray source location on mathematical compensations used in the clinical assessment of external apical root resorption (EARR). METHODS: Using geometric constructions, governing equations were derived to express the relationships between actual tooth, root and crown lengths, and their image sizes on "before" and "after" radiographs. RESULTS: Good agreement was found between calculated and published experimental results. More importantly, errors associated with the assessment methods are demonstrated. CONCLUSION: It is esta

Unexpected early apical resorption of primary molars and canines.

AIM: This was to examine the resorption pattern of primary molars and canines in dentitions with advanced apical resorption even though the permanent successor had barely begun root formation and to verify the resorption process histologically. STUDY DESIGN: Panoramic or bite-wing radiographs were selected from a dentition archive of radiographs from 142 children with deviant resorption patterns. METHODS: There were 14 patients (10 boys, 4 girls) aged 6 years 2 months to 8 years 1 month selected. In 11 patients abnormal resorption occurred only in the roots (group I). In 3 patients resorptio

Secondary Dentin Formation Mechanism: The Effect of Attrition.

Human dentin consists of a primary layer produced during tooth formation in early childhood and a second layer which first forms upon tooth eruption and continues throughout life, termed secondary dentin (SD). The effect of attrition on SD formation was considered to be confined to the area subjacent to attrition facets. However, due to a lack of three-dimensional methodologies to demonstrate the structure of the SD, this association could not be determined. Therefore, in the current study, we aimed to explore the thickening pattern of the SD in relation to the amount of occlusal and interprox
