Phase II study of topotecan in metastatic hormone-refractory prostate cancer.

Systemic chemotherapy with currently available agents has not improved survival for patients with hormone refractory prostate cancer (HRPC), consequently, the evaluation of new agents is warranted. Topotecan is a specific inhibitor of topoisomerase I with broad antitumor activity in preclinical studies. The purpose of this phase II trial was to determine the objective response rate of topotecan administered as a 30 minute infusion for five consecutive days in men with metastatic HRPC. Thirty-four evaluable patients were treated with topotecan 1.1-1.5 mg/m2 as a 30 minute infusion daily for fiv

Novel ABCG2 Antagonists Reverse Topotecan-Mediated Chemotherapeutic Resistance in Ovarian Carcinoma Xenografts.

Chemotherapeutic resistance remains a challenge in the treatment of ovarian carcinoma, especially in recurrent disease. Despite the fact that most patients with newly diagnosed tumors attain complete remission following cytoreductive surgery and chemotherapy, ovarian carcinoma has a recurrence rate that exceeds 75%. The ATP-binding cassette family G member 2 (ABCG2) efflux protein has been described as one mechanism that confers multiple-drug resistance to solid tumors and contributes to topotecan resistance in ovarian carcinoma. In fact, one clinical trial demonstrated ABCG2 expression in all

Altered phospholipid transfer protein gene expression and serum lipid profile by topotecan.

Camptothecin (CPT) and its structural analogues including topotecan and irinotecan, are inhibitors of topoisomerase I. These drugs are clinically active against a broad spectrum of cancers. To understand the genesis of chemotherapeutic resistance to the CPT family of anticancer drugs, we examined by gene expression profiling the pharmacological response to topotecan in the human hepatoma HepG2 cells and found a striking induction of the phospholipid transfer protein (PLTP) gene expression by topotecan. We showed that activation of PLTP gene expression is specific to CPT and its analogues inclu

Topotecan in the treatment of acquired immunodeficiency syndrome-related progressive multifocal leukoencephalopathy.

Progressive multifocal leukoencephalopathy (PML) affects about 1 in 20 individuals with the acquired immunodeficiency syndrome (AIDS) and has been associated with poor survival. This report describes the results of a phase II clinical trial using the drug topotecan, a semisynthetic analogue of camptothecan, administered to a cohort of subjects with AIDS-related PML. Data were evaluated on 11 of 12 subjects enrolled in the study. Three responded to therapy. Additionally, one patient was treated off-protocol and showed a response to treatment. Progression occurred after the first course; however

Combined effect of topotecan and irradiation on the survival and the induction of chromosome aberrations in vitro.

BACKGROUND: Topotecan demonstrated radiosensitizing effects in vivo and in vitro, which are schedule- and cell line dependent. The underlying mechanisms are not fully understood. Topotecan interferes with DNA replication, transcription, and repair and may thus increase the frequency of radiation-induced lethal chromosome aberrations. MATERIAL AND METHODS: U-373 MG glioblastoma cells, WiDr colon adeno-carcinoma cells, as well as normal human fibroblasts and lymphocytes were irradiated (0-6 Gy) and exposed to varying topotecan doses (0.01-10 microM) always 15 minutes before and either 1 hour af

New agents for the treatment of acute myelogenous leukemia: focus on topotecan and retinoids.

Topotecan and retinoids are among the most promising agents being evaluated for the treatment of acute myelogenous leukemia (AML), refractory anemia with excess blasts (RAEB), and refractory anemia with excess blasts in transformation (RAEB-t). Single-agent topotecan is similar to single-agent ara-C, but may be superior in patients with poor-prognosis chromosome abnormalities (ie, -5,-7). Topotecan plus ara-C is equivalent to topotecan alone in chronic myelomonocytic leukemia (CMML), but significantly more effective in RAEB and RAEB-t. Compared with single-agent ara-C, the complete remission (

Effect of PEG-Induced Liquid-Liquid Phase Separation on DNA-Topotecan Interactions.

Understanding the impact of liquid-liquid phase separation (LLPS) on drug and biological molecules is key to advancing drug delivery and biomolecular engineering. This study investigates how polyethylene glycol (PEG)-induced LLPS modulates the photophysical behavior of topotecan (TPT) and the structural organization of calf thymus DNA (ctDNA) in binary (PEG:TPT and PEG:ctDNA) and ternary (PEG:TPT:ctDNA) systems. In the PEG:TPT system, LLPS induced a shift from the emissive Z-form to the more stable C-TPT form, with enhanced fluorescence and a longer ∼4.0 ns lifetime, suggesting reduced water a

Improved therapeutic index of lower dose topotecan chemotherapy in recurrent ovarian cancer.

OBJECTIVE: Topotecan (1.5 mg/m(2)) administered daily for 5 consecutive days of a 21-day cycle is an established chemotherapeutic regimen in recurrent ovarian cancer. However, noncumulative myelosuppression has limited its use by many clinicians. We sought to determine whether a lower dose of topotecan could provide comparable tumor activity and higher tolerability in pretreated ovarian cancer patients. METHODS: A retrospective chart review was conducted on recurrent ovarian, peritoneal, or fallopian tube cancer patients with measurable disease or elevated cancer antigen 125 levels (evaluable

Nonplatinum topotecan combinations versus topotecan alone for recurrent ovarian cancer: results of a phase III study of the North-Eastern German Society of Gynecological Oncology Ovarian Cancer Study Group.

PURPOSE: The management of recurrent ovarian cancer remains controversial. Single-agent topotecan is an established treatment option, and preliminary evidence suggests improved tumor control by combining topotecan with etoposide or gemcitabine. PATIENTS AND METHODS: Women with relapsed ovarian cancer after primary surgery and platinum-based chemotherapy were randomly assigned to topotecan monotherapy 1.25 mg/m(2)/d, topotecan 1.0 mg/m(2) plus oral etoposide 50 mg/d, or topotecan 0.5 mg/m(2)/d plus gemcitabine 800 mg/m(2) on day 1 and 600 mg/m(2) on day 8 every 3 weeks. Stabilized phospholipid-topotecan prodrug particles increase vascular permeability and ultrasound-enhanced neuroblastoma delivery.

Neuroblastoma (NB) accounts for 15% of pediatric cancer deaths, with high-risk patients having < 50% 5-year survival despite intensive treatment. Topotecan (TPT), a topoisomerase I inhibitor in standard therapy, has limited efficacy due to its short plasma half-life of 30 min. We synthesized a phospholipid-topotecan prodrug (2T-T) and incorporated it into liposomes (L-2TT) to enhance drug loading and delivery. We hypothesized L-2TT would demonstrate superior efficacy versus free topotecan, with further controlled release when combined with sonopermeation (microbubble cavitation driven by spati
