Total disc replacement in the lumbar spine: a systematic review of the literature.

The current evidence for total disc replacement was assessed by performing a systematic review of the published literature. This search identified two randomised controlled trials (RCTs), two previous systematic reviews, seven prospective cohort studies, eleven retrospective cohort studies and eight case series. The RCTs involved the use of the Charité artificial disc and the Pro-Disc II total disc replacement. All papers analysed were classified according to their level of evidence as defined by the Centre for Evidence Based Medicine, Oxford, UK (www.cebm). For degenerative disc disease at L4

Postoperative Physical Therapy for Cervical Total Disc Replacement: A Case Report.

BACKGROUND AND PURPOSE: Cervical total disc replacement (CTDR) was developed to decompress nervous structures, preserve spinal biomechanics, and reduce postoperative complications such as nonunion or fracture. There is no published literature regarding the optimal content, timing and duration of postoperative physical therapy (PT) for CTDR. Therefore, the purpose of this case report is to describe the postoperative PT management and one-year outcomes of a subject with CTDR as it relates to pain, function and quality of life. CASE DESCRIPTION: A 37-year-old male and Brazilian Jiu Jitsu athlete

Effect of two-level total disc replacement on cervical spine kinematics.

STUDY DESIGN: Biomechanical study using human cadaver spines. OBJECTIVE: To characterize kinematics of cervical spines implanted with total disc replacement (TDR) at 2-levels referencing the implanted and adjacent levels. SUMMARY OF BACKGROUND DATA: Cervical TDR is an appealing alternative to fusion particularly when treating multilevel disease, where the advantages of maintaining motion and reducing adjacent level stresses with TDR are compelling. To our knowledge there are no biomechanical studies evaluating multilevel cervical TDR. METHODS: Six human cadaveric cervical spine specimens (C

Is Degenerative Spondylolisthesis a Contraindication for Total Disc Replacement? Kineflex Lumbar Disc Replacement in 7 Patients With 24-Month Follow-up.

BACKGROUND: Degenerative spondylolisthesis is associated with a significant segmental kyphosis at the level of the listhesis. We treated 7 disc spaces with Grade 2 listhesis and/or kyphosis of the slipped disc level with Kineflex disc replacement. METHODS: Out of a single-center prospective registry, involving 310 lumbar disc replacement patients, 7 patients underwent a single-level Kineflex disc replacement at the level of a degenerative spondylolisthesis with either segmental kyphosis or a Grade 2 slip. Preoperative and follow-up radiological parameters studied were: pelvic incidence, pelvi

Total disc replacement surgery for symptomatic degenerative lumbar disc disease: a systematic review of the literature.

The objective of this study is to evaluate the effectiveness and safety of total disc replacement surgery compared with spinal fusion in patients with symptomatic lumbar disc degeneration. Low back pain (LBP), a major health problem in Western countries, can be caused by a variety of pathologies, one of which is degenerative disc disease (DDD). When conservative treatment fails, surgery might be considered. For a long time, lumbar fusion has been the "gold standard" of surgical treatment for DDD. Total disc replacement (TDR) has increased in popularity as an alternative for lumbar fusion. A co

Cervical disc arthroplasty (CDA)/total disc replacement (TDR) vs. anterior cervical diskectomy/fusion (ACDF): A review.

BACKGROUND: We performed a focused review to determine the "non-inferiority", potential superiority, and relative safety/efficacy for performing cervical disc arthroplasty (CDA)/total disc replacement (TDR) in carefully selected patients vs. anterior cervical diskectomy/fusion (ACDF). Notably, CDA/TDR were devised to preserve adjacent level range of motion (ROM), reduce the incidence of adjacent segment degeneration (ASD), and the need for secondary ASD surgery. METHODS: We compared the incidence of ASD, reoperations for ASD, safety/efficacy, and outcomes for cervical CDA/TDR vs. ACDF. Indica

Total disc replacement in the treatment of lumbar discogenic pain with disc herniation: a prospective clinical study.

BACKGROUND: Biomechanical benefits of Total Disc Replacement (TDR) including both the restoration of normal segmental range of motion and the prevention of physiological lumbar lordosis encourage spine surgeons to perform TDR for lumbar disc disease. METHODS: A total of twenty patients (mean age: 39.5) who had degenerative disc disease with unilateral disc herniation were operated on between 2003 and 2006. Microscopic anterior lumbar discectomy with TDR placement via a transperitoneal approach were performed. Each patient was evaluated using a VAS and the Oswestry index. RESULTS: Mean ODI im

Early development and progression of heterotopic ossification in cervical total disc replacement.

OBJECT: The purpose of cervical total disc replacement (TDR) is to decrease the incidence of adjacent segment disease through motion preservation. Heterotopic ossification (HO) is a well-known complication after hip and knee arthroplasties. There are few reports regarding HO in patients undergoing cervical TDR, however; and the occurrence of HO and its effects on cervical motion have rarely been reported. Moreover, temporal progression of HO has not been fully addressed. One goal of this study involved determining the incidence of HO following cervical TDR, as identified from plain radiographs

Long-term clinical outcome of the Charité III total lumbar disc replacement.

PURPOSE: To compare the long-term clinical results and complications of two revision strategies for patients with failed total disc replacements (TDRs). METHODS: In 19 patients, the TDR was removed and the intervertebral defect was filled with a femoral head bone strut graft. In addition, instrumented posterolateral fusion was performed (removal group). In 36 patients, only a posterolateral instrumented fusion was performed (fusion group). Visual analogue scale (VAS) for pain and Oswestry Disability Index (ODI) were completed pre- and post-revision surgery. Intra- and post-operative complicat

Vertebral body split fracture after a single-level cervical total disc replacement.

Cervical total disc replacement (TDR) is a viable option for the surgical treatment of degenerative disc disease. This 67-year-old nonsmoking male patient underwent single-level ProDisc-C cervical TDR at C5-6 without any intraoperative problem. His radicular pain improved and he had no neck pain immediately after the operation. However, on postoperative Day 3, a radiograph demonstrated a vertical split fracture of the C-5 vertebra. This fracture was managed conservatively, and 2 years postoperatively a follow-up CT scan demonstrated stable device position and fusion of the fracture. Although t

Range of motion and adjacent level degeneration after lumbar total disc replacement.

BACKGROUND CONTEXT: There are no published studies on the relationship between total disc replacement (TDR) motion and the development of adjacent level degeneration (ALD). Because prevention of ALD is the underlying justification for TDR, studies investigating the validity of this concept are essential. PURPOSE: To examine the relationship between range of motion (ROM) and ALD 8.7 years after lumbar TDR. STUDY DESIGN/SETTING: Retrospective radiographic and chart review. PATIENT SAMPLE: Forty-two patients 8.7 years after lumbar TDR. OUTCOME MEASURES: Radiographic flexion-extension and ALD.

Total disc replacement using a tissue-engineered intervertebral disc in vivo: new animal model and initial results.

STUDY TYPE: Basic science Introduction:  Chronic back pain due to degenerative disc disease (DDD) is among the most important medical conditions causing morbidity and significant health care costs. Surgical treatment options include disc replacement or fusion surgery, but are associated with significant short- and long-term risks.1 Biological tissue-engineering of human intervertebral discs (IVD) could offer an important alternative.2 Recent in vitro data from our group have shown successful engineering and growth of ovine intervertebral disc composites with circumferentially aligned collagen

An incomplete paraplegia following the dislocation of an artificial cervical total disc replacement.

Replacement of the cervical intervertebral disc by artificial implants, known as cervical total disc replacement (CTDR), is becoming a generally applied method instead of using the gold standard of the anterior cervical discectomy and fusion. Hypothetically, the preserved mobility results in the protection of the neighboring segments. There is growing evidence that results in patients who underwent CTDR were not inferior when compared to results in patients who underwent anterior cervical discectomy and fusion. The authors report a case of a healthy 53-year-old man who suffered an incomplete p
