Acute pediatric tracheitis: Distinguishing the disease by tracheostomy status.

OBJECTIVES: Tracheitis is an upper airway infection that often presents in patients with tracheostomies and can potentially cause airway obstruction. This study aims to use a nationwide database to identify a large cohort of pediatric patients admitted with tracheitis to elucidate the management and resource utilization associated with the disease both with and without tracheostomies. METHODS: The Kids' Inpatient Database (KID) 2012 was used to identify 2394 weighted discharges with acute tracheitis, with or without obstruction, as the primary diagnosis. Data on prior tracheostomy status, dem

Extensive cryptococcal tracheitis mimicking lymphoma in an AIDS patient.

We describe a case of a 35-year-old man with human immunodeficiency virus infection/acquired immune deficiency syndrome presenting from an outside hospital with a diagnosis of non-Hodgkin's B-cell lymphoma. Radiologic imaging showed an infiltrative process with significant tracheal involvement and increased uptake of 18-fluoro-2-deoxyglucose on positron emission tomography. These findings were suspicious for non-Hodgkin's B-cell lymphoma. Flexible bronchoscopy and tissue sampling revealed cryptococcal infection of the trachea. This case presentation is of extensive cryptococcal tracheitis, an

Bacterial tracheitis: report of eight new cases and review.

Bacterial tracheitis, previously referred to as nondiphtheritic laryngitis with marked exudate, was commonly discussed in pediatric textbooks before 1940. It seemed to disappear as a clinical entity after that time, but it has been recorded with increasing frequency in the pediatric literature since 1979. We describe eight new cases and review 110 previously described cases. The clinical course consists of a prodromal upper respiratory illness with stridor, fever, and a variable degree of respiratory distress. Unlike patients with croup, patients with bacterial tracheitis do not respond to aer

Post-operative tracheitis in the NICU: incidence and risk factors.

OBJECTIVE: To describe the incidence and risk factors of tracheitis following tracheostomy placement in infants. STUDY DESIGN: We performed a 14-year (2010-2023) retrospective cohort study of infants with a tracheostomy placed in a Level IV Neonatal Intensive Care Unit. The primary outcome was tracheitis, defined by a combination of microbial, laboratory, or clinical evidence. RESULT: We identified 152 infants with a tracheostomy, of which 80 (52.6%) had post-operative tracheitis based on the study definition. Sixty-four (80%) infants with post-operative tracheitis had a positive tracheal co

Bacterial Tracheitis: A New Presentation of a Well-Known Disease.

Bacterial tracheitis (BT) is an uncommon life-threatening condition that results in acute upper airway obstruction. Classical signs include a toxic appearance, stridor, tachypnoea, and fever, often leading to rapid clinical deterioration. Recent studies have shown a shift in BT epidemiology and presentation, where stridor and respiratory distress are now predominant. A poor response to corticosteroids or nebulized epinephrine is also commonly described, along with a need for mechanical ventilation. We present the case of a five-year-old boy admitted to the emergency department with cough, stri

Obstructive, granulomatous tracheitis caused by Onchocerca sp. in a dog.

A 3-year-old spayed female dog was admitted with a history of episodes of dyspnoea and coughing. Severe tracheal stenosis was found on radiography. The dog died during an episode of refractory dyspnoea. Necropsy revealed an obstruction of the thoracic part of the trachea because of a chronic granulomatous inflammation protruding into the tracheal lumen. Histological examination revealed nematodes, which were identified as Onchocerca sp. according to their morphological characteristics. In contrast to the common ocular manifestation in dogs, obstructive tracheitis caused by Onchocerca infection

Herpetic tracheitis and brachial plexus neuropathy in a child with burns.

Herpetic tracheobronchitis is a well-recognized clinical entity that most commonly occurs in immunocompromised patients, including patients with burns. Although the diagnosis of herpetic tracheobronchitis is usually not made until postmortem examination, the presence of the condition can be established when histologic specimens of a patient with upper airway obstruction are studied. In this article, a case is described in which a child developed herpetic tracheitis after undergoing elective intubation after the grafting of burns of the face, neck, and upper extremity. The tracheitis resulted i

Improving management of ventilator associated tracheitis in a level IV NICU.

BACKGROUND: There are no published guidelines regarding the diagnosis and treatment of ventilator-associated tracheitis (VAT) in the neonatal intensive care unit (NICU). VAT is likely over-diagnosed and over-treated, increasing antibiotic burden and cost. LOCAL PROBLEM: Diagnosis and treatment of VAT were entirely NICU provider dependent. METHODS: Retrospective pre- and post-intervention chart reviews were performed. INTERVENTIONS: A VAT diagnosis and treatment algorithm was created for use in the care of intubated patients without tracheostomies. 3 plan-do-study-act (PDSA) cycles were used

Membranous obstructive Candida tracheitis as a complication of endotracheal intubation and tracheostomy.

This report describes a rare case of acute life-threatening stridor that was due to membranous obstructive Candida tracheitis, and this condition occurred after long-term endotracheal intubation and tracheostomy. An obstructive membrane was found 2 cm above the carina by bronchofiberscopy. The removal of the membrane resulted in the complete relief of the airway obstruction.

Experimental Porcine Astrovirus 4-Associated Tracheitis and Bronchitis in Pigs.

Porcine astrovirus 4 (PoAstV4) is an emerging pathogen that has previously been associated with respiratory disease in piglets. The objective of this study was to inoculate naïve piglets with PoAstV4 and characterize the host-pathogen interaction and pathological lesions. Cesarean-derived colostrum-deprived (CDCD) piglets were inoculated intratracheally and intranasally with PoAstV4 PCR-positive tissue homogenate previously screened for the presence of other primary pathogens with next-generation sequencing (NGS). Nasal and fecal swabs for PCR identification of PoAstV4 were collected pre and p

When tuberculosis travels: granulomatous tracheitis leading to critical airway stenosis.

Tuberculous tracheitis is an uncommon yet serious complication of tuberculosis (TB) infection. The underlying granulomatous inflammation typically results in tracheal stenosis within six months of diagnosis. This report details a case of a woman from Southeast Asia in her mid-40s who presented with chronic weight loss and cough. Imaging demonstrated pulmonary nodules as well as tracheal and oesophageal thickening. Bronchoscopy showed friable, ulcerated tracheal mucosa. Mycobacterium TB was confirmed on microbiological testing. Despite initiating anti-tuberculosis therapy with a standard 4-drug

A case of pseudomembranous tracheitis caused by Mycoplasma pneumoniae in an immunocompetent patient.

Pseudomembranous tracheitis (PMT) is a rare condition characterized by pseudomembrane formation in the tracheobronchial tree that may be associated with infectious and noninfectious processes. However, PMT attributed to Mycoplasma pneumoniae (M. pneumoniae), a common atypical respiratory infectious pathogen, has not been reported till date. Here, we report about a 29-year-old woman with complaints of severe persistent cough and radiographic deterioration despite antibiotics administration for pneumonia at an outside facility. She was finally diagnosed as having PMT with bilateral diffuse bronc

Herpes simplex virus tracheitis in a patient with the acquired immunodeficiency syndrome.

Herpetic tracheobronchitis and pneumonia occur basically in immunodepressed patients, but have rarely been reported in patients with the acquired immunodeficiency syndrome (AIDS). Some large reviews on pulmonary manifestations in AIDS report a small number of herpetic pulmonary infections, without determining any prevalence of this particular viral involvement. Predisposing factors are alteration of cell-mediated immunity and invasive procedures (such as endotracheal tube use) in debilitated patients. The case we report illustrates the occurrence of a herpetic tracheitis in an HIV-infected pat

Necrotizing tracheitis secondary to corynebacterium species presenting with central airway obstruction.

Pseudomembranous necrotizing tracheitis is a rare cause of life-threatening central airway obstruction. Patients present with hoarseness, cough, dyspnea, wheezing, stridor, and occasionally with respiratory failure. Diagnosis requires a high index of suspicion, and should be confirmed with a comprehensive bronchoscopy and biopsy. For patients with signs and symptoms of airway obstruction, a therapeutic rigid bronchoscopy with mechanical debridement is recommended. We present a rare case of necrotizing tracheitis secondary to Corynebacterium species, which caused life-threatening airway obstruc

Characterization of a herpesvirus associated with tracheitis in Gouldian finches (Erythrura [Chloebia] gouldiae).

Severe tracheitis and bronchitis were identified in two fatal cases of respiratory disease affecting a flock of Gouldian finches (Erythrura [Chloebia] gouldiae). Intranuclear inclusion bodies in epithelial cells of the upper respiratory tract were identified in samples from two birds. Electron microscopic examination showed that the inclusions consisted of viral particles consistent in appearance with Herpesviridae. Degenerate PCR primers targeting a conserved region of the herpesviral-DNA-dependent DNA polymerase were used to amplify a region of DNA isolated from tissues with lesions from eac

Fourteen juvenile (15- to 20-month-old) green sea turtles (Chelonia mydas), representative of a group of sea turtles with clinical signs of respiratory tract disease, were euthanatized and submitted for necropsy. Macroscopically, lesions included periglottal necrosis, tracheitis with intraluminal caseous and laminated necrotic debris, and severe pneumonia. Several turtles had caseous conjunctival exudate covering the eyes. Microscopically, the turtles had fibrinonecrotic inflammation around the glottal opening, tracheitis, and severe bronchopneumonia and interstitial pneumonia. In multifocal a
