Transition of Care in Celiac Disease.

Celiac disease (CD) is a gluten related disorder which affects all age-groups and occurs in genetically susceptible population after introduction of gluten in diet. The worldwide prevalence of CD is ~1% and it is higher in certain "at-risk groups". The clinical features are variable, ranging from classical diarrhea to an asymptomatic state. Diagnosis requires serology and duodenal histology although a non-biopsy diagnosis is recommended by European Society of Pediatric Gastroenterology, Hepatology and Nutrition (ESPGHAN) for a select group of children. Treatment of CD is with a life-long stric

Interhospital Care Transitions: Coordinating Care Across Institutions.

Interhospital transfers (IHT) are indispensable for providing timely access to specialized services, particularly for rural populations and high-acuity conditions such as trauma, stroke, and myocardial infarction. However, IHTs introduce substantial risks, including transport complications, care discontinuity, increased costs, prolonged hospitalization, and elevated mortality. Although Emergency Medical Treatment and Labor Act and Centers for Medicare & Medicaid Services regulations establish baseline safeguards, coordination deficiencies persist. Centralized transfer centers, standardized doc

A Risk-Informed Transitional Care Framework for Patients with Glomerular Disease.

Patients with glomerular disease (glomerulonephritis, GN) experience complex care transitions that increase the risk of fragmented care, delayed decision-making, and inadequate preparedness. Within the Mid-Atlantic Glome rulonephritis Network (MAGNET), an interdisciplinary nephrology care team identified the absence of a standardized approach to support patients across key transition points, including transplant referral, dialysis planning, and pediatric-to-adult care. In response, the PATHWAY GN protocol was developed as an interdisciplinary transitional care model embedded within routine nep

Transitional Care in Pediatric Brain Tumor Patients: A Systematic Literature Review.

BACKGROUND: Due to advances in the treatment of pediatric brain tumors (PBT), an increasing number of patients are experiencing the transition from the pediatric to the adult health care system. This requires efficient transitional models. METHODS: We systematically reviewed the literature regarding PBT concerning different transitional models and aspects of the transitional period. For this purpose, PubMed, Medline, and Embase databases were searched systematically through January 2022. RESULTS: We reviewed a total of 304 studies, of which 15 were ultimately included. We identified five tra

Are we there yet? An update on transitional care in rheumatology.

Significant progress has been made in the understanding of transitional care in rheumatology over the last few decades, yet universal implementation has not been realised and unmet needs continue to be reported. Possible explanations for this include lack of evidence as to which model is most effective; lack of attention to the multiple dimensions, stakeholders and systems involved in health transitions; and lack of consideration of the developmental appropriateness of transition interventions and the services/organisations/systems where such interventions are delivered.Successful transition h

Transitional health care for patients with Hirschsprung disease and anorectal malformations.

BACKGROUND: Hirschsprung disease (HD) and anorectal malformations (ARM) are congenital disorders with potentially lifelong consequences. Although follow-up is performed in most pediatric patients, transfer to adult health care is often problematic. This study assesses transitional care with the help of questionnaires in consultation with adult patients. METHODS: This study was conducted in an outpatient clinic of a pediatric surgical center in the Netherlands. All patients born and treated for ARM or HD before 1992 were invited to visit our clinic. Patients completed questionnaires concerning

Transitional care challenges of rehospitalized veterans: listening to patients and providers.

Readmissions to the hospital are common and costly, often resulting from poor care coordination. Despite increased attention given to improving the quality and safety of care transitions, little is known about patient and provider perspectives of the transitional care needs of rehospitalized Veterans. As part of a larger quality improvement initiative to reduce hospital readmissions, the authors conducted semi-structured interviews with 25 patients and 14 of their interdisciplinary health care providers to better understand their perspectives of the transitional care needs and challenges faced

Rural Patients' Transitional Care Experiences: A Qualitative Analysis of Provider Notes.

PURPOSE: Rural patients face an increased risk of 30-day mortality after hospitalization, compared to their urban counterparts. The Department of Veterans Affairs (VA) Geriatric Learning Health System's operational partners sought to identify care-related and contextual factors associated with mortality among rural patients being discharged from hospital to home. METHODS: We examined clinical notes from medical records of 200 (N = 100 rural; N = 100 urban) Veterans ≥65 years-old who were discharged alive to home after a medical or neurological index admission but who died within 30 days of di

Which transitions count? A systematic hermeneutic review of the literature on transitions of care.

BACKGROUND: Transitions of care are defined as critical moments along the care continuum for patients as they navigate changes in their health status or level of care. Researchers suggest that the existing research on transitions of care is largely descriptive and lacking in theoretical depth, which limits its application to different clinical contexts. AIM: We therefore aim to review the existing literature and theoretical frameworks on transitions of care, with a focus on primary care settings. METHOD: We undertook a systematic hermeneutic review - an approach that emphasises analytical re

[Transition to adult care for patients with epilepsy].

The transition of children and adolescents with epilepsy to the adult healthcare system presents many challenges. The disease is frequently accompanied by cognitive and developmental impairments that make it difficult to achieve self-management of the disease. Seizures are often associated with a loss of consciousness; therefore, conversations regarding medical history often take place only between the physician and the parents. The children and adolescents then usually have a very little knowledge about their disease and do not learn to talk about their seizures and other disease-related prob

Transition to adult care in sleep medicine.

More children with chronic and complex care needs are transitioned to adulthood due to advancements in medical technology including the use of non-invasive ventilation [NIV] at home and innovative medical therapies. Sleep medicine is becoming a common and at times vital component of the management plan. Various challenges are experienced in transitioning sleep patients depending on the underlying condition. These include the direct conflict between the desires of a young person for independence and their declining ability to provide self-care in neuromuscular patients, the behavioural challeng
