Severe craniocerebral injury by an axe with good outcome: case report.

We report a young patient who was operated on for a penetrating slow impact craniocerebral injury in the left frontal region caused by an axe. The patient was admitted comatose, with right hemiplegia. The blade of the axe was embedded deeply into his head. A craniectomy was carried out around the axe blade and it was removed easily. The cerebral wound was 6 cm long in horizontal plane and about 7 cm deep. Significant amount of contused and necrotic brain tissue was aspirated. The patient showed an uneventful recovery.

Shock index combined with age and the Glasgow Coma Scale during the initial care of polytraumatized patients as a predictor of mortality.

OBJECTIVES: To study whether combining age and the Glasgow Coma Scale (GCS) with the shock index (SI) - SIA/G - during the initial care of polytraumatized patients can improve the ability of the SI alone to predict mortality. To compare the predictive performance of the SIA/G combination to other prognostic scales: the addition of points for the GCS, age and systolic blood pressure (GAP); the Revised Trauma Score (RTS); and the Injury Severity Score (ISS). MATERIAL AND METHODS: Observational cohort study of patients with severe trauma admitted to the intensive care unit of a tertiary care hos

Crush syndrome in a case of severe infant physical abuse: a case report.

Crush syndrome, also known as traumatic rhabdomyolysis, is the result of the disruption of skeletal muscle fibers with the release of intracellular contents into the bloodstream. Although trauma is the main trigger for rhabdomyolysis in adults, in the pediatric population viral infections and inherited disorders seem to be the most frequent causes. Only a few reports in the literature mention rhabdomyolysis secondary to non-accidental pediatric trauma. We herein report an unusual case of traumatic rhabdomyolysis, following significant physical abuse in an infant. Rhabdomyolysis should be suspe

[Severe combined injury of the rectum and bladder in a child].

Traumatic anorectal injuries are rare in pediatric surgical practice. Only several similar cases are described in the world literature. This causes no generally accepted algorithms and tactics for these patients. We demonstrate successful surgical treatment of combined trauma of the rectum and bladder in a child. A 13-year-old boy was hospitalized after the child sat on the leg of an overturned chair. No evidence of penetrating abdominal injury was revealed. The boy underwent sigmoidoscopy under general anesthesia. We found a lacerated wound of anterior wall of the rectum measuring 1/3 of its

Bilateral Shoulder Dislocation Presenting as a Unilateral Shoulder Dislocation: Case Report.

Shoulder dislocations are a common condition seen by orthopedic surgeons and emergency medicine physicians. Shoulder dislocations typically present unilaterally and are anterior in nature. Bilateral simultaneous posterior shoulder dislocations are typically associated with seizure or electrocution and are more common than bilateral simultaneous anterior dislocations, which occur because of trauma or fall. In a traumatic setting, a shoulder dislocation may be accompanied by a distracting injury, and a thorough examination must be conducted to avoid overlooking concomitant diagnosis. The authors

Predicting the potential for recovery within 24 hours of a head injury.

A linear discriminant model is applied to clinical and radiological signs from 492 consecutive head injuries transferred to a neurosurgical unit over a period of four years. Unstable discriminators may be eliminated and those remaining transformed to interval scales in order to release their full prognostic potential. Patient selection criteria included being in coma, judged in the accident and emergency (A & E) department to be susceptible to developing a complicating secondary event, having a depressed fracture and presenting evidence of leakage of cerebrospinal fluid. These criteria are con

The use of symptom severity measured just before termination to predict child treatment dropout.

The current study examined indices of trauma-related symptom severity as predictors of dropout from exposure-based cognitive behavioral therapy in a sample of 99 child and adolescent trauma victims. The investigation incorporated measures of symptom severity at two time points: pretreatment and just before termination. The results indicated that a model with symptom severity measured just before termination was significantly associated with the number of attended sessions; however, a model with the symptom-severity indices measured at pretreatment was nonsignificant. In addition, a significant

Injury scaling at autopsy: the comparison with premortem clinical data.

The difference between injury scaling performed in the same patients on the basis of clinical information only and postmortem examination only is largely unknown. We compared scores in all 279 trauma patients who died in the Department of Critical Care Medicine at Auckland Hospital from 1982 through 1987 (93% blunt trauma, 4% penetrating trauma, 3% burns; median time until death--2 days) using both the 1980 and 1985 revisions of the Abbreviated Injury Scale (AIS-80, AIS-85) and derived Injury Severity Scores (ISS-80, ISS-85) where such scoring was based on clinical information only (CLAIS, CLI

How Well Do Ocular Trauma Scores Predict Vision after Open-Globe Injury?: A Systematic Review and Meta-Analysis.

TOPIC: This systematic review and meta-analysis evaluate the predictive accuracy of ocular trauma scoring systems, particularly the Ocular Trauma Score (OTS) and Pediatric OTS (POTS), for estimating final visual outcomes after open-globe injuries (OGIs). CLINICAL RELEVANCE: Open-globe injuries are vision-threatening injuries with significant global morbidity. In children, amblyopia risk and age-related limitations in visual assessment complicate prognostication. Although OTS and POTS are commonly used for counseling and triage, their accuracy and applicability in pediatric patients remain unc

Diagnosis and treatment of triceps tendon injuries: a review of the literature.

OBJECTIVE: To elucidate mechanism of injury, nonoperative protocols, surgical techniques, rehabilitation schedules, and return to sports guidelines for partial and complete triceps tendon injuries. DATA SOURCES: The PubMed and OVID databases were searched in 2010 and peer-reviewed English language articles in 2011. MAIN RESULTS: After a fall on an outstretched hand, direct trauma on the elbow, or lifting against resistance, patients often present with pain and weakness of extension. Examination may reveal a palpable tendon gap, and radiographs may reveal a Flake sign. Acute partial injuries

An analysis of head impact severity in simulations of collisions between pedestrians and SUVs/work utility vehicles, and sedans.

OBJECTIVE: To describe the determinants of the severity of the head kinematics of a pedestrian when struck by common sport utility vehicles (SUV) and work utility vehicles (WUVs) to assess how effective assessment protocols are in assessing injury risk for SUVs and work utilities. METHODS: Three hundred twenty-four simulations of pedestrian collisions with SUVs, work utility vehicles, and sedans were performed using several vehicle geometries, pedestrian orientations, speeds, and braking levels. Contact stiffnesses in the models were based on impact test results with exemplar vehicle structur

The role of the radiologist in the management of politrauma patients.

The management of trauma patients has become a very relevant issue and one of the major challenges in the western countries. In the assessment of politrauma patients, since the last 2 decades, radiological imaging methods have been increasingly used in order to provide a quick and thorough survey of craniocerebral, cervical, abdominal, pelvic and limb traumatic injuries. Among imaging methods, conventional radiographs (CR) have precise but limited indications. In most European hospitals, UltraSonography (US) represents the method of choice for patients referred following blunt abdominal trauma

Appraising current methods for preclinical calculation of burn size - A pre-hospital perspective.

BACKGROUND: Calculation of the percentage of total body surface area burnt is a vital tool in the assessment and management of patients sustaining burns. Guiding both treatment and management protocols. Currently there is debate as to which method of estimation is the most appropriate for pre-hospital use. METHODS: A literature review was undertaken to appraise current literature and determine the most appropriate methods for the pre-hospital setting. The review utilised MEDLINE and structured hand searching of Science Direct, OpenAthens, COCHRANE and Google Scholar. RESULTS: Fourteen studie
