Using patient self-reports to study heterogeneity of treatment effects in major depressive disorder.

BACKGROUNDS: Clinicians need guidance to address the heterogeneity of treatment responses of patients with major depressive disorder (MDD). While prediction schemes based on symptom clustering and biomarkers have so far not yielded results of sufficient strength to inform clinical decision-making, prediction schemes based on big data predictive analytic models might be more practically useful. METHOD: We review evidence suggesting that prediction equations based on symptoms and other easily-assessed clinical features found in previous research to predict MDD treatment outcomes might provide a

The effect of heterogeneous hyperimmune IgG antibody on prophylaxis and treatment of Pneumocystis carinii infection in rats.

Immunotherapy has been used in support of trimethoprim-sulfamethoxazole treatment for Pneumocystis carinii pneumonia. The present study investigated the therapeutic or preventive effects of heterogeneous hyperimmune IgG antibody (HIA) in experimental rats. Their immunity was suppressed by steroid injection, and they were also injected peritoneally with HIA which reacted with 40-55, 92, 116, and 200 kDa bands of the crude antigen. All rats were infected by P. carinii and the cystic forms on lung impression smears were counted. The count was 20.5-76.5 (mean 52.5 + 19.3) in those which received s

Heterogeneity in MACE Definitions and Its Impact on Treatment Effect Estimates in Coronary Stent Trials: A Meta-Epidemiological Study.

BACKGROUND: Major adverse cardiac events (MACE) are widely used composite endpoints in cardiovascular trials. However, heterogeneity in MACE definitions may influence treatment effect estimates, with potential implications for clinical decision-making. This study explored variability in MACE definitions across coronary stent trials and assessed the influence of individual components on effect estimates. METHODS: We systematically searched PubMed and Embase (via Ovid) from inception to March 5, 2026, for meta-analyses and extracted eligible coronary stent randomized controlled trials (RCTs) fr

Treatment Effect Heterogeneity in Acute Kidney Injury Incidence Following Intravenous Antihypertensive Administration for Severe Blood Pressure Elevation During Hospitalization.

RATIONALE & OBJECTIVE: Severe hypertension (HTN) that develops after hospital admission is prevalent in 10% of patients admitted for reasons other than HTN. Severe HTN is commonly treated with intravenous (IV) antihypertensives and is associated with a greater risk of acute kidney injury (AKI). We explored whether there is heterogeneity in IV antihypertensives' effect on AKI incidence among patients who develop severe HTN during hospitalization. STUDY DESIGN: Heterogeneity of treatment effect analysis. SETTINGS & PARTICIPANTS: Patients who developed severe HTN, defined as systolic blood pres

Machine learning analysis plans for randomised controlled trials: detecting treatment effect heterogeneity with strict control of type I error.

BACKGROUND: Retrospective exploratory analyses of randomised controlled trials (RCTs) seeking to identify treatment effect heterogeneity (TEH) are prone to bias and false positives. Yet the desire to learn all we can from exhaustive data measurements on trial participants motivates the inclusion of such analyses within RCTs. Moreover, widespread advances in machine learning (ML) methods hold potential to utilise such data to identify subjects exhibiting heterogeneous treatment response. METHODS: We present a novel analysis strategy for detecting TEH in randomised data using ML methods, whilst

Metabolic dysfunction-associated steatotic liver disease: heterogeneous pathomechanisms and effectiveness of metabolism-based treatment.

The global epidemic of metabolic dysfunction-associated steatotic liver disease (MASLD) is increasing worldwide. People with MASLD can progress to cirrhosis and hepatocellular carcinoma and are at increased risk of developing type 2 diabetes, cardiovascular disease, chronic kidney disease, and extrahepatic cancers. Most people with MASLD die from cardiac-related causes. This outcome is attributed to the shared pathogenesis of MASLD and cardiometabolic diseases, involving unhealthy dietary habits, dysfunctional adipose tissue, insulin resistance, and subclinical inflammation. In addition, the s

Heterogeneous Antibiotic Resistance Gene Removal Impedes Evaluation of Constructed Wetlands for Effective Greywater Treatment.

Microorganisms carrying antimicrobial resistance genes are often found in greywater. As the reuse of greywater becomes increasingly needed, it is imperative to determine how greywater treatment impacts antimicrobial resistance genes (ARGs). Using qPCR and SmartChip™ qPCR, we characterized ARG patterns in greywater microbial communities before, during, and after treatment by a recirculating vertical flow constructed wetland. In parallel, we examined the impact of greywater-treated irrigation on soil, including the occurrence of emerging micropollutants and the taxonomic and ARG compositions of

Paracrine effects and heterogeneity of marrow-derived stem/progenitor cells: relevance for the treatment of respiratory diseases.

Stem cell-based treatment may represent a hope for the treatment of acute lung injury and pulmonary fibrosis, and other chronic lung diseases, such as cystic fibrosis, asthma and chronic obstructive pulmonary disease (COPD). It is well established in preclinical models that bone marrow-derived stem and progenitor cells exert beneficial effects on inflammation, immune responses and repairing of damage in virtually all lung-borne diseases. While it was initially thought that the positive outcome was due to a direct engraftment of these cells into the lung as endothelial and epithelial cells, par

Middle meningeal artery embolization as a surgical adjunct for non-acute subdural hematoma: real world outcomes and treatment effect heterogeneity.

BACKGROUND: Middle meningeal artery embolization (MMAE) is a promising treatment for non-acute subdural hematoma (NASDH), but trial evidence is limited by heterogeneity and open label design bias. This study aims to assess the real world effectiveness of MMAE as an adjunct to surgery and to explore sources of treatment effect heterogeneity. METHODS: This was a retrospective cohort study of the Nationwide Readmissions Database (2016-22). Adults who underwent NASDH surgery were included and divided into two groups: surgery with MMAE and surgery only groups. Propensity score matching (PSM) balan

Heterogeneity of treatment effect of interferon-β1b and lopinavir-ritonavir in patients with Middle East respiratory syndrome by cytokine levels.

Animal and human data indicate variable effects of interferons in treating coronavirus infections according to inflammatory status and timing of therapy. In this sub-study of the MIRACLE trial (MERS-CoV Infection Treated with a Combination of Lopinavir-Ritonavir and Interferon β-1b), we evaluated the heterogeneity of treatment effect of interferon-β1b and lopinavir-ritonavir versus placebo among hospitalized patients with MERS on 90-day mortality, according to cytokine levels and timing of therapy. We measured plasma levels of 17 cytokines at enrollment and tested the treatment effect on 90-da

Treatment effect heterogeneity of radiotherapy in localized Ewing sarcoma: A secondary analysis of the EURO-E.W.I.N.G. 99 and Ewing 2008 trial.

INTRODUCTION: Standard Ewing sarcoma treatment includes chemotherapy and local therapy. Previous studies found no average treatment effect of combined local therapy (radiotherapy and surgery) versus surgery alone, leaving it unclear which patients benefit from combined therapy. METHODS: We analysed patients with localized Ewing sarcoma from the EURO-E.W.I.N.G. 99 and Ewing 2008 trials. Using causal survival forests validated through repeated k-fold cross-validation, we predicted individualized treatment effects (ITEs) for 5-year event-free survival (5y EFS) comparing combined therapy to surge

Precision medicine evaluation of heterogeneity of treatment effect for a time-to-event outcome with application in a trial of Initial treatment for people living with HIV.

BackgroundEvaluation of heterogeneity of treatment effect among participants in large randomized clinical trials may provide insights as to the value of individualizing clinical decisions. The effect modeling approach to predictive heterogeneity of treatment effect analysis offers a promising framework for heterogeneity of treatment effect estimation by simultaneously considering multiple patient characteristics and their interactions with treatment to predict differences in outcomes between randomized treatments. However, its implementation in clinical research remains limited and so we provi

Graphical structure learning identifies hypothesized mechanisms for heterogeneous treatment effects in Medicaid population health programs.

Heterogeneous treatment effect models identify Medicaid beneficiaries who benefit most from population health programs but provide limited mechanistic insight. We combined graphical structure learning with regression-based effect estimation to generate hypotheses about intervention-specific mechanisms in a program serving 6396 Medicaid managed care enrollees (2023-2025). The Peter-Clark algorithm and Greedy Equivalence Search identified conditional dependencies between baseline characteristics, intervention exposures (behavioral health therapy, n = 434; clinical pharmacy, n = 632; community he

Heterogeneity in Treatment Effects of Reduced Versus Standard Dose of Cabazitaxel in Metastatic Castration-Resistant Prostate Cancer.

BACKGROUND: In the PROSELICA, a randomized controlled trial (RCT) comparing cabazitaxel 20 mg/m2 (C20) versus 25 mg/m2 (C25) in metastatic castration-resistant prostate cancer (mCRPC), one-variable-at-a-time subgroup analysis suggested possible heterogeneity in treatment effect (HTE) of C25 versus C20 among study participants. Novel predictive HTE analysis approaches may provide an in-depth understanding of such results. METHODS: We analyzed patient-level data from 1200 patients with mCRPC who were randomized in the PROSELICA trial. Outcomes included overall survival (OS) and progression-free

Heterogeneous treatment effects of Cerebrolysin as an early add-on to reperfusion therapy: post hoc analysis of the CEREHETIS trial.

Background: There has been intensive research into enhancing the effects of reperfusion therapy to mitigate hemorrhagic transformation (HT) in stroke patients. Using neuroprotective agents alongside intravenous thrombolysis (IVT) appears a promising approach. Cerebrolysin is one of the candidates since it consists of neuropeptides mimicking the action of neurotrophic factors on brain protection and repair. Objectives: We looked at treatment effects of Cerebrolysin as an early add-on to IVT in stroke patients with varying HT risk. Methods: It was post hoc analysis of the CEREHETIS trial (ISRCTN

Unsupervised clustering approach to assess heterogeneity of treatment effects across patient phenotypes in randomized clinical trials.

BACKGROUND: Primary results from randomized clinical trials (RCT) only inform on the average treatment effect in the studied population, and it is critical to understand how treatment effect varies across subpopulations. In this paper we describe a clustering-based approach for the assessment of Heterogeneity of Treatment Effect (HTE) over patient phenotypes, which maintains the unsupervised nature of classical subgroup analysis while jointly accounting for relevant patient characteristics. METHODS: We applied phenotype-based stratification in the ENGAGE AF-TIMI 48 trial, a non-inferiority tr

Heterogeneous treatment effects of metformin on risk of dementia in patients with type 2 diabetes: A longitudinal observational study.

INTRODUCTION: Little is known about the heterogeneous treatment effects of metformin on dementia risk in people with type 2 diabetes (T2D). METHODS: Participants (≥ 50 years) with T2D and normal cognition at baseline were identified from the National Alzheimer's Coordinating Center database (2005-2021). We applied a doubly robust learning approach to estimate risk differences (RD) with a 95% confidence interval (CI) for dementia risk between metformin use and no use in the overall population and subgroups identified through a decision tree model. RESULTS: Among 1393 participants, 104 develop

Early Antibiotic Therapy in Sepsis Without Shock: A Multimethod Study of Heterogeneous Treatment Effects in ICU Patients.

INTRODUCTION: The survival benefit of early antibiotic administration in septic shock is well established. In contrast, for sepsis without shock, the current Surviving Sepsis Campaign (SSC) guidelines recommend initiating antibiotics according to the likelihood of infection. Given the persistent challenges in accurately diagnosing sepsis, there is a need to identify patient characteristics to guide antimicrobial timing in this population. METHODS: In this cohort study, we included adult intensive care unit (ICU) patients meeting Third International Consensus Definitions for Sepsis (Sepsis-3)
