BACKGROUND: De Vega type suturing dosage plasty is the intervention most widely used for relative insufficiency of the tricuspid valve (TV). In fact, this operation may be considered as an equivalent of prosthetic reconstruction of the fibrotic ring, shaping a fixed orifice for passage. Intrapulmonary pressure normalized as early as during the first months after surgery in patients with the mitral-tricuspid cardiac valvular disease. Under such conditions a rigid suture (prosthesis of the anulus) at the interface of the right atrium and ventricle is not physiological.

We present 2 cases of tricuspid insufficiency following blunt chest trauma: 1 was diagnosed 5 months after the trauma and the other, 20 years after the trauma. In both patients, the tricuspid valve was replaced with a porcine bioprosthesis, because valve repair was not considered feasible. These cases emphasize the variability of clinical presentation of post-traumatic tricuspid valve insufficiency and indicate the need for close follow-up of patients after major thoracic trauma.

Forty-seven patients who underwent tricuspid valve replacement (TVR) or tricuspid annuloplasty (TA) combined with aortic (AVR) or mitral valve replacement (MVR) at Henry Ford Hospital, from 1971 to 1977 are reviewed. Thirty-three patients underwent TVR with a Hancock valve combined with MVR or AVR; hospital mortality was 30% (10 to 33) and was higher at the beginning of the series when smaller valves were used.
