Pituitary apoplexy, defined by an acute ischemic or hemorrhagic event of the pituitary gland, can lead to neurological deficits. We present the case of a 43-year-old male who initially exhibited symptoms resembling trigeminal autonomic cephalalgia (TAC) as sudden-onset headache, ptosis, and autonomic symptoms. He later developed abducens nerve palsy and magnetic resonance imaging revealed a pituitary macroadenoma with intratumoral hemorrhage. Endocrine evaluation showed multiple hormonal deficiencies. The patient was diagnosed with pituitary apoplexy and referred for surgical intervention. Thi

BACKGROUND: This report describes four cases of cracked tooth syndrome secondary to traumatic occlusion that mimicked trigeminal autonomic cephalalgias. All patients were referred by general practitioners to the Orofacial Pain Clinic at Nihon University Dental School for assessment of atypical facial pain.
CLINICAL PRESENTATION: Case 1: A 51-year-old woman presented with severe pain in the maxillary and mandibular left molars. Case 2: A 47-year-old woman presented with sharp, shooting pain in the maxillary left molars, which radiated to the temple and periorbital region. Case 3: A 49-year-old

We report a 24-year-old man who presented with the sudden onset of unilateral poliosis associated with acute trigeminal autonomic cephalalgia, suggesting a pathophysiology in common and a possible neural hypothesis in the development of segmental vitiligo. Although rare, associations with neurological abnormalities should be considered in cases of focal depigmentation disorders.

Ehlers-Danlos syndrome (EDS) type IV is characterized by thin skin with visible veins, easy bruising, characteristic facial features, arterial and digestive complications, as well as rupture of the gravid uterus. It has never been previously reported that trigeminal autonomic cephalalgias (TACs) could manifest as the only initial symptom of EDS type IV. Here, we report a case of a 27-year-old man who presented atypical headache like TACs stimulated by right internal carotid artery dissection. About one month after his discharge, he suffered dissection of the right renal artery and splenic arte

Patent foramen ovale (PFO), a relatively common abnormality in adults, has been associated with migraine. Few studies also linked PFO with cluster headache (CH). To verify whether right-to-left shunt (RLS) is related to headaches other than migraine and CH, we used transcranial Doppler following microbubbles injection to detect shunts in 24 CH, 7 paroxysmal hemicrania (PH), one SUNCT, two hemicrania continua (HC) patients; and 34 matched controls. RLS was significantly more frequent in CH than in controls (54% vs. 25%, p=0.03), particularly above the age of 50. In the HC+PH+SUNCT group, RLS wa

OBJECTIVE: This narrative review highlights the importance of orofacial pain considerations in the differential diagnosis of trigeminal autonomic cephalalgias (TACs)/trigeminal autonomic orofacial pains (TAOPs). In addition, we provide novel insights into the pathophysiology of the presentation of TACs in the maxillary (V2) and mandibular (V3) trigeminal distributions (TAOPs).
BACKGROUND: TACs are lifelong disorders characterized by severe unilateral head pain accompanied by cranial autonomic phenomena. However, the head pain experienced may refer to the face, be present solely in the orofaci

BACKGROUND: Secondary cases of trigeminal autonomic cephalalgia have been described, most commonly following tumor, trauma, or vascular causes.
CASES: We report a series of 6 patients who experienced ipsilateral headache responding to the International Headache Society criteria for cluster headache, hemicrania continua or probable hemicrania continua. It occurred immediately or in the following days or weeks after cataract or glaucoma surgery. We complete these descriptions with 4 cases from a literature review of trigeminal autonomic cephalalgia attributed to non-destructive eye surgery.

We present a patient with treatment refractory short-lasting unilateral neuralgiform headache attacks with conjunctival injection and tearing (SUNCT) who was found to have low levels of serum testosterone supporting the hypothalamic connection to this trigeminal autonomic cephalalgia. Clomiphene citrate therapy induced a significant elevation of testosterone levels (by its effect on hypothalamic estrogen receptors) and led to a dramatic reduction in SUNCT attacks. Hormonal manipulation may be a treatment strategy for hypothalamic-influenced trigeminal autonomic cephalalgias.

A 51-year-old woman had an attack of severe hemifacial pain with autonomic features as the presenting symptom of a lateral medullary infarction. A bilateral vertebral artery dissection was demonstrated. The existence of secondary cases may lead to a better understanding of the pathophysiology of trigeminal autonomic cephalalgias.

Neuromyelitis Optica (NMO) can have atypical presentations like hiccups, vomiting, etc. which is classically described as the area postrema syndrome. Here we report a case of a 39 year old male patient who presented with features of Trigeminal Autonomic Cephalalgia (TAC). MRI spine showed long segment myelitis. Diagnosis of NMO was confirmed by a positive Anti aquaporin 4 antibody assay. TACs are a rare group of headache disorders characterized by severe unilateral headache in the V1 distribution of the trigeminal nerve and autonomic symptoms. This presentation in NMO is hitherto unreported in

BACKGROUND: Trigeminal neuralgia (TN) has been described in association with various primary headache disorders. So far, no case of TN has been reported in association with hemicrania continua (HC).
CASE REPORT: Here, we report two patients of hemicrania continua associated with TN (HC-tic syndrome). These patients had both headaches concurrently. Both patients responded to a combination of carbamazepine and indomethacin. The skipping or tapering of carbamazepine led to the recurrence of the neuralgic pain of TN. In the same way, the skipping of indomethacin resulted in the relapse of the pai

BACKGROUND: The trigeminal autonomic cephalalgias are a group of primary headache syndromes marked by severe head pain and associated cranial autonomic symptoms which can include a full or partial Horner's syndrome. Rarely, the eye-related symptoms will become fixed even between headache attacks. There is minimal documentation that the Horner's syndrome can be reversed if successful treatment of the underlying headache disorder is initiated.
CASE REPORTS: Two cases are presented of trigeminal autonomic cephalalgia subtypes with chronic persistent Horner's syndromes that alleviated with treatm

Raeder's paratrigeminal neuralgia is a rare clinical syndrome characterized by ipsilateral periorbital pain, partial Horner syndrome (ptosis and miosis without anhidrosis), and sensory disturbances in the ophthalmic division (V1) of the trigeminal nerve1. The syndrome results from involvement of sympathetic fibers traveling along the internal carotid artery (ICA), often in association with vascular or parasellar pathologies2. Among its etiologies, ICA dissection is particularly important, as it may present with unilateral headache and cranial autonomic features that closely mimic trigeminal au

BACKGROUND: The study of the trigeminal autonomic cephalalgias (TAC) has been limited by difficulty aggregating sufficient numbers of patients. We used the Epic Cosmos electronic health record research platform to harness nationwide data from health care systems across the United States using the Epic electronic health record to analyze the prevalence, demographics, comorbid conditions and treatments for the TACs.
METHODS: We queried the Epic Cosmos electronic health record database for patients with diagnoses of hemicrania continua, cluster headache, paroxysmal hemicrania, and SUNCT. Prevale

The objective of this study is to describe the therapeutic effect and the technical and surgical problems of deep brain stimulation (DBS) of the posterior hypothalamus over seven years, for treatment of chronic trigeminal autonomic cephalalgias and atypical facial pain. We report a surgical series of 20 patients that underwent DBS of the posterior hypothalamus. This series includes 16 patients with chronic cluster headache (CH), one patient with short-lasting unilateral neuralgiform headache attacks with conjunctival injection and tearing (SUNCT) and three patients with atypical facial pain. PURPOSE OF REVIEW: Visual disturbances and ocular symptoms are common manifestations of two primary headache disorders, migraine and trigeminal autonomic cephalalgias, and many secondary headache disorders.
RECENT FINDINGS: Structural lesions have been described with trigeminal autonomic cephalalgias. A systematic diagnostic evaluation including neuroimaging with assessment of intracranial and cervical vasculature, and the sellar and paranasal regions is recommended in every patient presenting with symptoms indicative of trigeminal autonomic cephalalgias for the first time.

OBJECTIVE: This article reviews the epidemiology, clinical features, differential diagnosis, pathophysiology, and management of three types of trigeminal autonomic cephalalgias: cluster headache (the most common), short-lasting unilateral neuralgiform headache attacks with conjunctival injection and tearing (SUNCT), and short-lasting unilateral neuralgiform headache attacks with cranial autonomic symptoms (SUNA).
LATEST DEVELOPMENTS: The first-line treatments for trigeminal autonomic cephalalgias have not changed in recent years: cluster headache is managed with oxygen, triptans, and verapami

SUNA is a trigeminal autonomic cephalalgia (TAC) characterized by short unilateral attacks centered on the ophthalmic trigeminal distribution, and accompanied by at least one of a number of cranial autonomic symptoms that can include lacrimation, redness of the ipsilateral eye, nasal congestion, rhinorrhea, and eyelid edema. It exists in episodic and chronic form. We have described an atypical case of episodic SUNA with an exclusive seasonal pattern as previously reported in other trigeminal autonomic cephalalgia, commonly known as TACs.
