New dorsal capsulotomy for the surgical exposure of the wrist.

A reliable, safe approach to the wrist through fiber-splitting dorsal capsulotomies has been developed. The dorsal wrist capsule is exposed by subperiosteally elevating the fourth and fifth extensor compartments ulnarly and translocating the extensor pollicis longus tendon with the radial wrist extensor tendons radially. The midcarpal joint and the radial half of the radiocarpal joint are exposed by longitudinally splitting the dorsal radiocarpal and dorsal intercarpal ligaments, with the apex at the triquetrum. The flap created is elevated radially, detaching the dorsal capsule from the radiu

Anatomic study of the pisotriquetral joint: ligament anatomy and cartilagenous change.

Eighty embalmed cadaver wrists (40 right and 40 left, 55 male and 25 female, aged 36 to 93 years [average age, 72.5 years]) were dissected to assess the anatomy of the pisotriquetral joint and the location of degenerative changes. The patterns of degenerative changes were classified into five types: type 1, central (8.8%); type 2, peripheral (38.8%); type 3, fan-shaped (13.8%); type 4, mixed (combination of > or =1 of types 1, 2, and/or 3; 10.0%); and type 5, total (12.5%). There was no cartilagenous change in 16.2% of the pisoform and 13.7% of the triquetrum. Degenerative changes (exposed sub

Three-Dimensional In Vivo Kinematic Analysis of Kienböck Disease Treated with Arthroscopic Lunate Excision.

PURPOSE: Treatment options for advanced stage Kienböck disease primarily involve lunate excision, either in the context of proximal row carpectomy or in association with partial wrist fusion. This study aimed to investigate the in vivo three-dimensional carpal kinematics following arthroscopic lunate excision. METHODS: We analyzed carpal translation and rotation of the affected wrist in five patients who underwent arthroscopic lunate excision for stage IIIC Kienböck disease. Our dynamic computed tomography image-based analysis focused on determining the centroid position of the scaphoid, triq

Pisotriquetral instability causing an unusual flexor tendon rupture.

We present a case of a closed rupture of the flexor digitorum profundus tendon of the small finger. It is our hypothesis that because of pisotriquetral instability as a result of a lesion of the pisotriquetral ligaments the flexor digitorum profundus tendon was entrapped repeatedly between the pisiform and triquetrum. This resulted in an attritional lesion of the tendon. The pisotriquetral joint showed no bone abnormalities.

Imaging Report With Radiologic Criteria for Differentiation: Avulsion Fracture or Os Triquetrum Secundarium?

Accessory ossicles of the wrist are uncommon bone variants that can be mistaken for avulsion fractures, potentially leading to unnecessary tests, procedures, and radiation exposure. We report a case of a 19-year-old male who presented after a motor vehicle collision with bilateral wrist pain and swelling. Radiographs of the right wrist were normal. Imaging of the left wrist demonstrated a small, oval ossicle located between the triquetrum and the ulnar styloid process. The ossicle displayed smooth cortical margins, a preserved cortical-medullary ratio, and a distinct radiolucent gap from adjac

[Bilateral congenital fusion of the scaphoid and the trapezium. A case report].

Congenital synostosis has been described between nearly all of the carpal bones. It can occur in an isolated form or most commonly in relation between the lunatum and the triquetrum. It can also occur in a multiple form and be associated with other malformations. We report a rare case of scaphoid-trapezium fusion observed bilaterally in a nearly asymptomatic 63-year-old patient.

Long-standing Ulnar Wrist Pain Caused by an Anatomic Variant of the Ulnar Nerve: A Case Report.

CASE: We present a 28-year-old woman with long-standing left ulnar wrist pain. Physical examination revealed severe tenderness over the pisiform; imaging findings were normal. Surgical exploration revealed compression of an aberrant branch, likely Kaplan's accessory branch, around the pisiform. Complete symptom relief was achieved after nerve decompression. CONCLUSION: Clinical symptoms associated with Kaplan's accessory branch are rare, indicating that many branches may be asymptomatic or overlooked despite the presence of symptoms. Kaplan's accessory branch should be considered as a potenti

Diagnosis and Treatment of Acute Lunotriquetral Ligament Injuries.

Isolated acute lunotriquetral (LT) injuries are an uncommon diagnosis in hand surgery. Diagnosis is aided by a high index of suspicion when pain is localized over the LT joint. Standard radiographs show typically normal findings, leading to advanced diagnostic investigations, including MRI and wrist arthroscopy. Standard treatment options for acute LT injuries include immobilization, arthroscopy, and direct open LT repair.

[Blood supply and function of the scaphoid bone].

The proximal row of carpal bones, including Os scaphoideum, Os lunatum and Os triquetrum, should be considered as a functional unit. The arterial supply stems from branches of the rete carpi dorsale et palmare, which pass through the Ligamentum radiocarpeum dorsale et palmare, respectively. In two-thirds of all cases, the scaphoid bone, the most frequently fractured of the wrist bones, is uniformly vascularized, so that by fracture both fragments retain their blood supply. In the remaining third of all cases, only one end of the scaphoid is supplied arterially. Thus the poorly vascularized fra

A rare case of an isolated triquetrum body fracture in a 14-year-old boy.

Isolated carpal fractures are uncommon injuries usually caused by a fall on the outstretched hand. The patient might present with non-specific clinical signs and X-ray diagnosis might be difficult due to bone overlay. An isolated triquetrum body fracture is an absolute rarity. The treatment is easy and the outcome excellent but if missed, degenerative changes with chronic pain and impaired movement might be the consequence.

Bilateral ulnar "thumbs".

Congenital fusion of the carpal bones is fairly common and may be associated with other congenital deformities in the hand. This report deals with an unusual congenital anomaly of the little fingers associated with fusion of the triquetrum and lunate. Both little fingers were adducted and rotated giving crude resemblance to thumbs on the ulnar borders of the hands. The deformity was easily corrected by osteotomy through the base of the fifth metacarpals.

Volarly displaced transscaphoid, translunate, transtriquetrum fracture of the carpus: case report.

A 16-year-old girl sustained a transscaphoid, translunate, transtriquetrum volarly displaced fracture due to shear stress in the coronal plane. Treatment involved open reduction and internal fixation of the fractures followed by immobilization. At 15 months' follow-up, grip strength and motion were diminished compared with the uninjured wrist. Radiographs demonstrated fully healed fractures with narrowing of the cartilage space between the capitate and the lunate.

Diagnosis and Treatment of Chronic Lunotriquetral Ligament Injuries.

Chronic lunotriquetral (LT) injuries are less common than scapholunate ligament injuries and difficult to diagnose. They may be associated with positive ulnar variance. Clinical diagnostic tests elicit pain at the LT interval. Although radiographs are typically normal, MRI and wrist arthroscopy can help confirm the diagnosis. When conservative treatments fail, surgical options include LT ligament reconstruction, LT arthrodesis, and ulnar-shortening osteotomy (in patients with positive ulnar variance).

Trans-carpal injuries associated with distal radial fractures in children: a series of three cases.

Isolated trans-carpal injuries in children are rare. This paper presents three cases of trans-carpal injury associated with an ipsilateral distal radial fracture. Two cases involved simultaneous fractures of the scaphoid and the capitate. The third case involved the scaphoid and the triquetrum. The full extent of these injuries was not recognized on the initial X-rays. If a child presents with a displaced fracture of one carpal bone it may not be an isolated injury.

[Chronic lunotriquetral instability of the wrist. Presentation of our method of treatment].

PURPOSE OF THE STUDY: One of the causes of pain on the ulnar side of the wrist is post-traumatic lunotriquetral (LTq) instability, which is difficult to detect on radiographs. For diagnosis, arthroscopic examination is most reliable. The methods for treatment of LTq instability include mere immobilization, ligament reconstructions and LTq joint stabilization with Kirschner's wires in acute conditions, and stabilization of the LTq joint by tenodesis or arthrodesis. In this study our method of treating chronic isolated injury to the lunotriquetral ligament is described. MATERIAL: The group comp

Osteoid osteoma of the triquetrum: A rare cause of ulnar-sided wrist pain in an adolescent female.

Osteoid osteoma is a common benign osteoblastic tumor typically affecting males in their second or third decade of life, with a predilection for the long bones of the lower extremities. Its occurrence in atypical anatomical sites, such as the carpal bones, is rare. Specifically, triquetral involvement is exceedingly infrequent, often leading to diagnostic challenges as the lesion can mimic more common inflammatory or infectious pathologies. We present the case of an 11-year-old girl with a 9-month history of persistent, localized left wrist pain. Initial magnetic resonance imaging demonstrated

Variations in Strain Distribution at Distal Radius under Different Loading Conditions.

Distal radial fractures exhibit various fracture patterns. By assuming that the strain distribution at the distal radius affects the diversification of the fracture pattern, a parameter study using the finite element model of a wrist developed from computed tomography (CT) images was performed under different loading conditions. The finite element model of the wrist consisted of the radius, ulna, scaphoid, lunate, triquetrum, and major carpal ligaments. The material properties of the bone models were assigned on the basis of the Hounsfield Unit (HU) values of the CT images. An impact load was

Multiple Intraosseous Cysts of the Carpal Bones Presenting as Unilateral Carpal Tunnel Syndrome.

Intraosseous ganglion cysts (IGC) of the carpal bones are frequently reported in the literature, involving at most two carpal bones of the same wrist. Only one case recently described the presence of multiple intraosseous ganglion lesions in the capitate, lunate, and triquetrum, resulting in chronic wrist pain. The following study reports the first case of multiple IGCs causing a unilateral carpal tunnel syndrome (CTS), in a 56-year-old woman, with no previous history of trauma. Failure of conservative management prompted carpal tunnel release and the surgical excision of the ICGs, followed by

Trapezial Fractures and Associated Fractures of the Ulnar Carpus: A Ring-Bone Model.

BACKGROUND: Fractures of the trapezium are rare; however, the incidence may be under-reported in the literature. The incidence of ulnar-sided carpal body fractures as a concomitant injury has not been reported. Our study aimed to evaluate the incidence of trapezium fractures in conjunction with ulnar-sided carpal body fractures. METHODS: Over a five-year period, our electronic records were queried and charts reporting carpal bone fractures were reviewed. All cases of trapezium fracture were evaluated further and presented. RESULTS: Eight trapezial fractures were identified, representing 8% o

Triquetrum Excision in Radioscapholunate Arthrodesis for Posttraumatic Radiocarpal Osteoarthritis With Ulnar Impaction Syndrome.

Dependent on fracture type, concomitant carpal injuries, and operative treatment, distal radius fractures can lead to symptomatic radiocarpal osteoarthritis. In addition, radial shortening can cause ulnar impaction syndrome. Radiocarpal arthrodesis and ulnar shortening osteotomy are known and frequently applied surgical procedures for each of those pathologies. There are limited data concerning treatment options for a combined disorder. The presented technique in this article demonstrates that radioscapholunate arthrodesis with distal pole scaphoidectomy and total triquetrum excision successfu
