Mandibular osteoma as a cause of ankylosis and progressive trismus.

Osteomas are benign tumours of bone tissue restricted to the craniofacial skeleton. The aim of this article is to present and discuss the demographic and clinical aspects and the management of craniomaxillofacial osteomas. When the patient was submitted from primary care to our hospital, he was 68 years old, and he had ankylosis of the temporomandibular joint for the previos 4 years. A CT scan was performed, finding a giant mandibular osteoma. Conservative treatment and radiological follow-up were carried out with clinical stability. Osteomas more often are seen in the paranasal sinuses and in

[Diagnosis of trismus, excluding tetanus and local causes].

The most frequent causes of trismus are tetanus and local lesions. This report discusses other etiologies. Trismus is usually only a secondary sign of a rich clinical picture, but may be of diagnostic value in some cases. Emphasis is placed on the frequency of involvement of neuroleptic intoxication, the early onset of trismus in the malignant hyperthermia syndrome, and on the useful role of trismus in the localization of neurological lesions.

Spontaneous Nosocomial Pseudomonas aeruginosa Meningitis Presenting as Trismus.

We describe the case of a 78-year-old female receiving adjuvant postsurgical chemotherapy for colon adenocarcinoma who spontaneously developed nosocomial Pseudomonas meningitis causing severe trismus. The patient was initially admitted for ileus, developing neck stiffness and trismus on the thirteenth day of admission. Cerebrospinal fluid grew pansensitive Pseudomonas aeruginosa. Magnetic resonance imaging of the brain was consistent with bilateral subacute infarcts secondary to meningitis. The patient responded well to 21 days of broad spectrum antimicrobial therapy modified to ceftazidime al

Trismus-pseudocamptodactyly syndrome: case report ten years after.

BACKGROUND: In 1969, Hecht and Beals described for the first time a rare dominant autosomal syndrome characterised by reduced mouth opening, pseudocamptodactyly, short stature, and foot deformities. Recent studies have confirmed that TPS is caused by a mutation of MYH8 that is common to another disease called Carney syndrome. CASE REPORT: The authors describe the long term follow-up of a case presented in 2003, ten years after the first surgical procedure: a 14-year-old girl, affected by this rare syndrome, had underwent an early (at 4 years) surgical treatment of bilateral coronoidotomies to

Using the Trismus Checklist to identify potential malignancy.

The primary dental care team commonly encounters patients presenting with trismus as the result of various benign causes. Common among these causes are the range of temporomandibular disorder diagnoses. In a rare but significant number of cases the cause may be an underlying malignancy in which case urgent referral is mandatory. This article describes the implementation of the Trismus Checklist, a nine-item screening tool designed to improve patient safety by structuring the diagnostic process for trismus. Objective measurement techniques, including recording the intra-incisal distance, while

Strategies for obviation and management of trismus in oral cancer.

PURPOSE OF REVIEW: Trismus (decreased mouth opening) is frequently associated with oral premalignant/malignant lesions, and also occurs in upto 40% of head neck cancer patients post radiation therapy. Trismus is associated with adverse functional and oncologic outcomes through impact on quality of life and posttreatment disease surveillance. Relevant literature is mostly retrospective and single author/institution- a review of the same would aid management of this complex multifactorial disorder. RECENT FINDINGS: Newer innovations aim at obviation of trismus during treatment planning for head

A case of conversion disorder presenting as chronic trismus.

Trismus can be the symptom of several diseases. For the most part, the inability to open the mouth is due to an articular disorder but occasionally, the cause may be extra-articular. In this case, being reported, non-articular hysterical trismus caused the jaw to lock in an 11-year-old boy for 3 months. During this period the jaw was completely locked and the locking was associated with moderate to severe pain. After three sessions of therapy the patient was able to open his mouth 33 mm and his eating was back to normal. Conversion disorders often present with dramatic physical presentation in

Retrospective study on the predictive factors in chronic trismus.

Trismus is one of the most debilitating and treatment-resistant complications resulting from head and neck oncological treatments. The objective of this study was to assess how primary tumour variables could assist in predicting chronic trismus. From a (retrospective) oncological database (Department of Oral and Maxillofacial Surgery, University Hospitals Leuven), tumour-related, surgical, and oral functional variables were reviewed. Contributing factors for chronic trismus, defined as a mouth opening of less than 35 mm, at least one year after treatment for oral squamous cell cancer, were ass

A Case of Cephalic Tetanus in an Elderly Patient with Trismus.

We report a case of a 77-year-old woman who presented to the Emergency Room with a three-day history of oral lesions and jaw tightness. Her physical examination was remarkable for the presence of trismus and white ulcers on the visible portion of the tongue. CT head and neck was unremarkable, and she was discharged with empiric treatment for oral candidiasis. She returned two days later with worsening symptoms and subsequently developed tonic-clonic seizures. MRI of the brain and temporomandibular joints were noncontributory. Psychiatry was consulted, and the patient was prescribed olanzapine

Surgery for extra-articular trismus: a systematic review.

The aim of this systematic review was to identify operations that are used to improve mouth opening in patients with extra-articular trismus (caused by cancer and its treatment, oral submucous fibrosis, or noma) and to find out if they work. We searched the electronic databases PubMed, Embase, Cinahl, and the Cochrane collaboration, and then systematically selected papers before we assessed their quality, extracted the data, and did a meta-analysis. We analysed 32 studies that included 651 patients, the median (IQR) size of which was 11 (7-26). The quality of the methods used and of reporting

Restricted mouth opening and trismus in oral oncology.

Restricted mouth opening (RMO) and trismus are terms commonly used in oral oncology in instances where there is difficulty in mouth opening. The term trismus in oral oncology is mainly used to indicate the radiation-induced fibrosis of the muscles of mastication. The treatment given for RMO as reported in the literature is given for muscular dysfunction trismus, whereas RMO in oral oncology can occur owing to various reasons other than muscular dysfunction. RMO occurs in various conditions of the oral cavity; in posterior pharyngeal infection, where it is termed reflectory trismus; in oral sub

Neonatal paroxysmal trismus and camptodactyly: the Crisponi syndrome.

The Crisponi syndrome is an infrequently described syndrome characterized by extensive muscular contractions in the face after even minimal stimuli, hypertonia, camptodactyly, and typical facial features (chubby cheeks, broad nose with anteverted nares, and long philtrum). Most patients have died in the first months of life due to hyperthermia. The syndrome has been described in Italians only; the inheritance pattern is most probably autosomal recessive. Here we describe a 4-year-old boy of Portuguese descent with this entity. Polysomnography during a paroxysmal muscle contraction showed sever

Radiologic examination of trismus as a complication of cancrum oris.

In the radiologic examination of trismus complicating cancrum oris, abnormalities can be detected in the soft tissues, osseous tissues, and temporomandibular joint. In the soft tissue, scar formation may show as a depression in the normal smooth, convex contour of the lateral aspect of the face. There may be a myositis ossificans in the soft tissue, producing bony bars that lead to extra-articular ankylosis. By far the most important changes are in the temporomandibular joint, where there can be varying degrees of joint narrowing, sclerosis of the articular cortex, flattening of the mandibular

Postictal Trismus Complicating Electroconvulsive Therapy: A Case Report.

We report a case of a 72-year-old woman who experienced postictal episodes of trismus lasting several minutes on 6 occasions during a series of 18 episodes of electroconvulsive therapy (ECT). There was no clear relationship between the development of trismus and any medications used during the treatments. The patient had no adverse outcomes or discomfort, but the development of trismus can put patients at significant risk.

Trismus Following Inferior Alveolar Nerve Block: A Case Report.

Trismus, characterized by a restricted mouth opening due to involuntary muscle spasms, poses significant challenges to eating, speaking, and other oral functions. In fact, this condition often results from various factors including post-third molar surgery complications, temporalis and medial pterygoid muscle involvement, repeated or incorrectly administered intramuscular injections, and complications from local anesthesia usage. Despite the high safety and efficacy of local anesthetics in oral surgery, side effects including trismus warrant careful consideration. Management strategies for mus
