BACKGROUND: Scrub typhus, caused by Orientia tsutsugamushi, is a mite-borne rickettsial infection endemic to the Asia-Pacific region. While its systemic complications are well documented, coronary artery lesions (CALs) associated with scrub typhus in children remain poorly characterized. This study aimed to describe the clinical features, diagnostic challenges, and treatment outcomes of pediatric scrub typhus complicated by CALs.
METHODOLOGY/PRINCIPAL FINDINGS: We retrospectively analyzed seven pediatric patients (aged 11 months to 8 years) with scrub typhus and echocardiographically confirme

Scrub typhus is an important cause of acute febrile illness in Nepal and can rarely present with meningoencephalitis and multiorgan involvement. We report a 32-year-old female farmer from rural Nepal who presented during the monsoon season with high-grade fever, myalgia, vomiting, and an eschar following an unidentified forest bite. She developed meningismus, hypotension, mild acute respiratory distress syndrome, and transaminitis. Cerebrospinal fluid showed lymphocytic pleocytosis and elevated protein, while bacterial culture, acid-fast staining, and GeneXpert were negative. Serum scrub typhu

BACKGROUND: Necrotic flexural eschar is a pathognomonic cutaneous marker of scrub typhus (ST). Relying solely on the classic necrotic flexural eschar can result in missed diagnoses when lesions present atypically. This study evaluated the clinical profile and predictors of atypical eschars (ATEs) in children with ST.
METHODS: In this 5-year prospective observational study from South India, children (< 12 years) with serologically confirmed ST (IgM ELISA) were systematically examined for eschars using a standardized whole-body assessment. Eschars were classified as typical eschars (TEs: necrot

BACKGROUND: Acute motor and sensory axonal neuropathy is a rare, severe variant of Guillain-Barré syndrome marked by rapid progression, autonomic dysfunction, and delayed recovery. The patient in this report was treated in a resource-limited intensive care unit and had coexisting tropical infections that complicated diagnosis and treatment.
CLINICAL FINDINGS: A 40-year-old man presented with acute quadriparesis, respiratory distress, and autonomic instability following a febrile illness. Laboratory tests revealed anemia, electrolyte imbalance, and methicillin-resistant Staphylococcus aureus b

OBJECTIVE: To investigate the dynamic changes of five major peripheral blood immune cell lineages during acute scrub typhus in children and their association with disease severity.
METHODS: A total of 141 children with clinically diagnosed scrub typhus were enrolled, including 100 mild and 41 severe cases. All children provided pretreatment immune samples, and 110 children with repeated post-treatment sampling contributed 299 observations to longitudinal immune analyses. Peripheral blood was collected at multiple time points, and the proportions and phenotypes of dendritic cells (DCs), monocy

BACKGROUND: Substantial outbreaks of scrub typhus, coupled with the discovery of this vector-borne disease in new areas, suggest that the disease remains remarkably neglected. The objectives of this study were to map the contemporary and potential transmission risk zones of the disease and to provide novel insights into the health burden imposed by scrub typhus in southern China.
METHODS: Based on the assembled data sets of annual scrub typhus cases and maps of environmental and socioeconomic correlates, a boosted regression tree modeling procedure was used to identify the environmental niche

Scrub typhus, caused by Orientia tsutsugamushi, is a re-emerging zoonotic infection with significant diagnostic reliance on its cutaneous manifestations. The disease, transmitted by larval trombiculid mites, is endemic to the 'Tsutsugamushi Triangle' and extends to other regions globally. Dermatological features are pivotal in early recognition, with eschar formation being pathognomonic. Eschars, resembling 'cigarette burns', develop as necrotic lesions surrounded by erythema and often resolve with minimal scarring. Maculopapular rashes, although variably reported, predominantly affect the tru

Tropical infections continue to cause significant morbidity and mortality in India. While mixed tropical infections are relatively common in the general population, they are rare during pregnancy. We report a 24-year-old pregnant woman with a history of previous lower segment caesarean section and preeclampsia, who developed dengue and scrub typhus infection near-term. The illness was complicated by multiple organ dysfunction syndrome, yet with timely multidisciplinary management, both maternal and neonatal outcomes were favourable. Literature on such mixed tropical infections in pregnancy rem

INTRODUCTION: Acute lymphoblastic leukemia (ALL) in adult patients usually poses a treatment challenge with intensive chemotherapeutic agents. Prognosis is worse in comparison to pediatric patients owing to disease biology and associated comorbidities. Spontaneous remission of ALL is rare and warrants close follow-up for progression.
CASE PRESENTATION: Herein, we report the case of a male in his 40s initially admitted for febrile illness with pancytopenia; later, his serology was positive for scrub typhus infection, and he was managed for the same with antimicrobials. Due to the persistence o

Orientia tsutsugamushi, the causative agent of scrub typhus, is endemic to the Asia-Pacific region. In South Korea, the Boryong strain is considered dominant; however, nationwide phylogeographic distribution and genetic diversity based on clinical isolates remain incompletely characterized. In this study, 121 O. tsutsugamushi clinical isolates were collected from scrub typhus patients at 11 hospitals across South Korea between 2015 and 2024. Isolates were genotyped using 56-kDa gene sequencing and multilocus sequence typing (MLST) of seven housekeeping genes. Sequence analysis and phylogenetic

Scrub typhus, caused by Orientia tsutsugamushi , is an emerging zoonotic infection that typically presents as an acute febrile illness. While neurological manifestations are increasingly recognized, cerebellar involvement remains exceedingly rare. We report the case of a 42-year-old man from Uttar Pradesh who presented with high-grade fever, followed by slurred speech and gait ataxia. Clinical examination revealed cerebellar signs, including dysmetria and dysdiadochokinesia. Laboratory testing confirmed scrub typhus via a positive immunoglobulin M enzyme-linked immunosorbent assay, and other i

Scrub typhus is a zoonotic infectious disease caused by Orientia tsutsugamushi infection. It has a high fatality rate if not treated in time. Currently, there are still limited reliable biomarkers for predicting the disease progression of scrub typhus. A Retrospective Analysis of the Clinical Features of Scrub Typhus and the Exploration of Early Indicators for Progression to Moderate-Severe Disease. Using the Luminex method, we quantified ten cytokines in the serum of patients with moderate-severe scrub typhus (n = 51), mild scrub typhus (n = 32), and healthy controls (n = 62). We compared ser
