[Bone conduction in tympanosclerosis patients and its changes after surgery for tympanosclerosis].

Objective:To investigate the bone conduction (BC) in tympanosclerosis patients and its changes after surgery for tympanosclerosis, as well as its possible causes.Method:Retrospectively analyzed 30 cases of tympanosclerosis patients.Result:Before surgery, the average BC of the operated ear was (21.06±8.88) dBHL, the average BC of the healthy side was (16.78±8.97) dBHL, the difference had a statistical significance (t= 3.072, P< 0.01). The BC threshold, air conduction (AC) and ABG were significantly lower after operation. Preoperative BC at 2 000 Hz was the highest threshold (25.17±10.95) dBHL, …

Stapes Release in Tympanosclerosis.

: Tympanosclerosis in the middle ear space is common in Ethiopia and often involves the ossicles and particularly the stapes. Ear operations in Ethiopia are relatively expensive in this country of limited medical resources and a low average living wage. In this setting, 2-stage operations using prostheses become prohibitively expensive. Therefore, the recommended 2-stage approach for tympanic membrane perforation with tympanosclerosis and stapes fixation is impractical for Ethiopia. We present a series of 67 patients who had a single stage tympanoplasty, removal of tympanosclerosis from the sta…

Malleostapedotomy in tympanosclerosis patients.

OBJECTIVES: To present our personal experience of a series of 10 patients suffering from tympanosclerosis with functional blocking of the stapes or footplate, who underwent malleostapedotomy surgery. The criteria for patient selection for this type of operation, and its results and complications, are discussed. METHODS: Prospective study. RESULTS: Incus and malleus dysfunction was observed in 70 per cent of cases, either alone or combined with fixation of the stapes. The post-operative hearing results were considered to be satisfactory (i.e. within 20 dB) in 80 per cent of cases. Only one pa…

[Surgical treatment of the tympanosclerosis].

Sixty-seven patients of tympanosclerosis were reported in this paper. All patients were underwent tympanoplasty. The results showed that the overall successful rate of hearing improvement following operation was 72.9 percent, there was no significant difference in postoperational hearing improvement rate between type I and II, III tympanoplasty patients, the results were better in the ears of patients without a sclerotic region around the stapes than in the ears of patients with a sclerotic region at the some site. Type II, III tympanoplasty was of benefit to patients with sclerotic regions ar…

The structure of the tympanosclerotic plaques of the tympanic membrane was examined by using the scanning electron microscope. Studies were … ons. The results showed that the role of a degenerative process was dominant in the pathogenesis of tympanosclerosis.

The mini-grommet and tympanosclerosis.

One hundred and sixteen children with otitis media with effusion underwent myringotomy and insertion of a conventional pattern of Shah grommet in one ear and the much smaller Mini Shah grommet in the other. Close observation post-operatively determined the comparative rate of extrusion, recurrence of effusion, and of onset and degree of tympanosclerosis. At one year review, the Mini Shah shows a significantly earlier extrusion and a greater tendency to recurrence of otitis media with effusion. However, this is compensated by a decreased incidence of tympanosclerosis and reduced severity in tho…

Reconstructive techniques for tympanosclerosis.

Tympanosclerosis is found in more than 30% of patients with chronic ear disease. Clinically significant tympanosclerosis is defined as that affecting the surgical procedure by requiring removal to effect a hearing improvement. This type is seen in less than half of all patients with tympanosclerosis. The incidence and patterns of involvement are documented for 352 of the author's surgical patients subdivided into two groups: one operated on in the early 1960s and another observed more than 15 years later. The surgical techniques employed for hearing restoration are described, as are the hearin…

[Tympanosclerosis etiology and treatment].

Tympanosclerosis is the middle ear tissue hyalinization and calcification caused by chronic middle ear inflammation, which mainly results in conductive deafness with unobvious clinical symptom. Etiology is unclear. The treatment is given priority to surgical treatment at present, while long-term effect reported mostly poor. This article analyzed etiology and treatment of the tympanic cavity sclerosis.

[Clinical features of tympanosclerosis].

The analysis of 412 case histories of patients operated for chronic otitis media was made. Tympanosclerosis (TS) was detected in 26(6.3%) patients. 19 patients had bilateral TS, 7 patients--unilateral. TS exacerbations observed up to the age of 13 frequently changed for long-term remission. All the patients had conductive and mixed hypoacusis. A total of 36 standard hearing-improving operations were made.

Surgical treatment of tympanosclerosis.

OBJECTIVE: To report the hearing results of the surgical treatment of tympanosclerosis. STUDY DESIGN: A retrospective review of surgically treated cases of tympanosclerosis. SETTING: A tertiary referral center. PATIENTS: One hundred fifteen patients with middle ear tympanosclerosis operated on between 1987 and 1996, with an average age of 36 years (range 18-59 years). Cases were classified into four groups according to Wielinga and Kerr. Those with an associated cholesteatoma were excluded. INTERVENTION: Depending on the ossicular status, either mobilization of the major ossicles or epitym…

Surgical treatment of tympanosclerosis.

Tympanosclerosis as a cause of conductive hearing loss may require corrective surgery in carefully selected cases, although results of treatment have been controversial and must be viewed with caution. Currently available surgical techniques are reviewed and outcomes discussed.

Clinicopathologic studies in tympanosclerosis.

Light-and electron-microscopic studies of the tissue from middle ears diagnosed as having tympanosclerosis, adhesive otitis media, postinflammatory ossicular fixation, and chronic otitis media revealed more or less similar findings of inflammatory response and healing. The histopathologic findings of "whitish sclerotic masses," especially around the ossicular chains, were due to fibrosis of submucosal connective tissue with hyalinization and calcification and/or metaplasia of mucosal epithelium. Longstanding and repeated undrained suppurative or nonsuppurative effusions would promote progressi…

Surgical treatment of tympanosclerosis.

During the 5-year period (1971--1976), 45 patients at the Shea Clinic underwent reconstructive surgery for tympanosclerosis. The most common ossicular chain problem was found to be malleus and incus fixation (33%); malleus, incus, and stapes fixation (22%); and stapes fixation (13%). The most successful reconstrucitve approaches included removal of tympanosclerosis from tympanic membrane or isolated plaque from ossicular chain, the use of the Partial Ossicular Replacement Prosthesis (PORP), and the Total Ossicular Replacement Prosthesis (TORP).
