Use of umbilical vein graft as an arterial substitute.

Seventy reconstructions, 14 to the popliteal artery above the knee, 40 to the popliteal artery below the knee, and 16 to the tibioperoneal arteries were performed with the Dardik umbilical vein graft tanned in glutaraldehyde. Indications for use included absence of or inadequate saphenous veins, or for expediency during emergency or complex procedures. The cumulative patency rate (calculated by the life table method) for the total group was 77% at 18 months. The failures are further analyzed according to the type of procedure, the grade of disease, and the distal runoff vessel. Early failures

Four-Vessel Umbilical Cord: Supernumerary Right Umbilical Vein With No Associated Congenital Anomalies.

A four-vessel umbilical cord is a rare anomaly that can occur with abnormal persistence of the caudal portion of the vessel. Although supernumerary vessels can present as an isolated finding, they are known to be associated with multiple significant congenital anomalies. Ectopia cordis, pulmonary stenosis, cleft lip, cleft palate, situs inversus, tetralogy of Fallot, and gastroschisis are some anomalies associated with four-vessel cords. This is a case of a 22-year-old multigravida with a four-vessel umbilical cord initially found on sonography. The patient was sent to Maternal Fetal Medicine

Does umbilical vein catheterization to exchange transfusion lead to portal vein thrombosis?

UNLABELLED: The aetiopathology of extrahepatic portal vein obstruction is unknown. In retrospective studies, umbilical vein cannulation and sepsis have been alleged to cause portal thrombosis. This prospective study was undertaken to detect whether thrombosis and consequent obstruction of the splenoportal venous system develops after umbilical vein catheterization for exchange transfusion in newborns using Doppler ultrasound. Forty children (M = 24; F = 16) who had undergone exchange transfusion for hyperbilirubinaemia were studied at school age. Maximal duration of the venous umbilical cannul

Coronary vein flowing into the umbilical portion of the left portal vein: angiographic appearance.

The coronary vein has few variations. We describe a case of coronary vein with an unusual course. In this case, the coronary vein entered the umbilical portion of the left portal vein directly, whereas it usually flows into the portal trunk or splenic vein. The angiographic appearance is presented.

Two cases of umbilical vein thrombosis, one with associated portal vein thrombosis.

BACKGROUND: Thrombosis of the umbilical vessels is extremely rare. It is unpredictable and is associated with high perinatal mortality. Few cases with neonatal survival have been reported. CASES: We identified two instances of umbilical vein thrombosis occurring two weeks apart in the same unit. Each case was identified at the time of delivery and each had a good outcome. In the first case, a 22-year-old primigravid woman had severe fetal bradycardia develop in the second stage of labour. Delivery was expedited with vacuum extraction, and a large thrombus in the umbilical vein was noted. In t

Increased oxidative stress and endothelial activation in umbilical veins from pregnancies diagnosed with preeclampsia.

Vascular dysfunction is a hallmark of cardiovascular disease (CVD). Offspring from preeclamptic pregnancies are at risk of CVD later in life. Whether fetal vasculature from preeclamptic pregnancies displays signs of vascular dysfunction (i.e., oxidative/nitrosative stress, endothelial activation) associated with increased expression of lectin-like oxidized LDL receptor-1 (LOX-1) and angiotensin-II type-1 receptor (AT1) is unknown. We demonstrated increased superoxide, nitrotyrosine and ICAM-1 levels in umbilical vein tissues of preeclamptic vs. normal pregnancies; without changes in LOX-1 and

Effect of ethanol on prostacyclin, thromboxane, and prostaglandin E production in human umbilical veins.

The purpose of this study was to determine the effects of ethanol on prostacyclin (PGI2), prostaglandin E (PGE), and thromboxane (TXA2) production in perfused human umbilical veins. PGI2, PGE, and TXA2 levels were measured from human umbilical veins perfused with either 25, 50, or 100 mM ethanol by radioimmunoassay of their stable metabolites. Alcohol content was measured by an enzymatic spectrophotometric assay. Data were analyzed by ANOVA and Fisher's Protected Least Significant Difference Test. Ethanol decreased PGI2 production in a concentration-dependent manner (p < 0.05). In a concentrat

Characterization of epitheloid cells from human omentum: comparison with endothelial cells from umbilical veins.

Capillary cells represent 95% of the vascular bed, and cells from large and micro-vessels do not express identical functions. In order to study the hormonal regulation of plasminogen activator inhibitor 1 (PAI-1) secretion by human capillary cells we used epithelial cells from omental tissue (HOTMEC). As their endothelial origin is subject to controversy, we attempted to determine their characteristics by comparing them to human umbilical vein endothelial cells (HUVEC). Morphological and biological criteria were studied. By phase contrast microscopy HOTMEC elicited a cobblestone pattern simila

Placental size and umbilical vein volume blood flow.

INTRODUCTION: The fetus receives oxygen and nutrition from the placenta through the umbilical vein. There is a large variation in placental size and placental size relative to fetal size (placental-fetal ratio). We studied whether placental size or placental-fetal ratio is related to blood flow in the umbilical vein. MATERIAL AND METHODS: We performed a prospective study at Akershus University Hospital in Norway and a total of 73 women were examined in gestational weeks 27 and 37. Fifty-six women were examined at both time points. B-mode ultrasound and Doppler ultrasonography were used to mea

Umbilical vein remodeling is associated with pregestational maternal overweight.

INTRODUCTION: Excess weight during pregnancy is a condition that can affect both mother and fetus, through the maternal-fetal interface, which is constituted by the placenta and umbilical cord. The umbilical vein is responsible for transporting oxygen and nutrients to the fetus, and its proper functioning depends on the integrity of its structure. The remodeling of the umbilical vein represents one of the causes of inadequate transport of nutrients to the fetus, being potentially harmful. This study aims to evaluate whether maternal overweight alters the structural characteristics of the umbil

Retrieval of a retained umbilical vein catheter at 6 months.

Umbilical venous catheterisation is carried out for the care of neonates who have very low birth weight and are critically ill. It is uncommon for catheter fragments to become fractured and retained. Here, we describe a case of a retained fractured umbilical vein catheter in an infant. An endovascular procedure to retrieve a catheter is the usual management option when the catheter has been accidentally cut and has moved proximally into a major vessel or inside the heart. It is rare to need open exploration and retrieval.

Is umbilical vein varix associated with changes in cerebroplacental ratio?

Objective: We aimed to review a single-center experience in follow-up and management of fetuses with umbilical vein varix (UVV) and to assess the effect of UVV on fetal Doppler parameters. Methods: We reviewed retrospectively maternal antenatal records, delivery records, and newborn records to identify cases of UVV. Further, we retrospectively compared 25 fetuses with isolated UVV and available cerebroplacental ratio (CPR) analysis with 75 matched controls. Results: We identified 67 cases of UVV. The median gestational age (GA) at diagnosis was 34 weeks (range: 26-41 weeks). The average diameter

Supernumerary umbilical vein in a hydropic neonate with hypertrophic cardiomyopathy.

The anomalies of the umbilical vessels are uncommon, with the exception of a single umbilical artery. We report a term female infant with fetal hydrops, hypertrophic cardiomyopathy, and a four-vessel umbilical cord consisting of two umbilical arteries and two umbilical veins. The presence of two veins in the umbilical cord has been attributed to persistence of both the normal left umbilical vein and the caudal part of the right umbilical vein. This fetal vascular pathology has been reported very rarely and may be associated with increased risk of congenital malformations and adverse perinatal

Umbilical vein catheter malposition: report of one case.

A male premature newborn was diagnosed as respiratory distress syndrome shortly after birth. Umbilical arterial and venous catheters were inserted. The position of the umbilical venous catheter was erroneous roentgenographically, but return of blood was noted when the catheter was pull back to below the diaphragm. After fluid infusion via the catheter, swelling of buttock, scrotum and bilateral thighs with pitting edema over the abdominal wall and lower back, and increased body weight were noted the next day. The abdomen was distended without bluish discoloration. Laboratory studies showed nei

The effects of sex steroids on human umbilical artery and vein.

OBJECTIVE: To define the vascular actions of selected sex steroids on human umbilical artery and vein and to determine whether there are any gender-specific differences in vasorelaxation between umbilical cord vessels of male and female fetuses. METHODS: Segments of umbilical artery (n = 12) and umbilical vein (n = 8) from male and female fetuses were suspended in organ baths of Krebs solution for isometric tension recording. The vessels were contracted using 60 mM potassium chloride followed by exposure to increasing concentrations (10(-7) to 10(-4) M) of sex steroid hormones (estradiol-17 b

Modified Blalock-Taussig shunt with an umbilical vein graft.

We describe a modification of the Blalock Taussig anastomosis, with the interposition of a glutaraldehyde-tanned umbilical vein graft between the subclavian and pulmonary arteries. This operation was performed in 64 children: 11 were less than 1 month of age (17.2%), and 23 were between 1 and 6 months of age (34.9%). Hospital deaths occurred in six patients-all less than 6 months of age (9.4%). There was no instance of shunt occlusion noted. The clinical course was uneventful among survivors, except for one patient who died of endocarditis in the late postoperative follow-up. The shunt procedu
