Philemon and Baucis Death? Two cases of double deaths of married couples.

When two human corpses are found in temporal and locational context the assumption of a non-natural cause of death is close at hand. Suicide, extended suicide, homicide or accident (particularly with carbon monoxide) come to mind. When, however, a natural cause of death emerges for both after autopsy and further examinations, this provokes the question whether this incidence is coincidental. In our two presented cases, a married couple each was found dead together. In both cases, the wife needed nursing and was being cared for by a husband who died of cardiac disease shortly before the wife di

Chronic myeloid leukemia treatment intolerance imposes additional resource and economic burden on oncology practices in the United States.

OBJECTIVE: Tyrosine kinase inhibitors (TKIs) have transformed the prognosis of chronic myeloid leukemia (CML) into a manageable chronic condition, but TKI intolerance remains a significant issue. This study aimed to quantify the economic burden incurred by oncology practices to manage a CML treatment intolerance episode. METHODS: This mixed-methods study leveraged interviews with US oncology practice managers (e.g. physicians, nurse leaders, advanced practice providers) and cost estimation analysis to estimate average time and cost per CML patient treatment intolerance episode, defined as an

Healthcare for migrants and for marginalized individuals: The Marienambulanz in Graz, Austria.

The Marienambulanz in Graz, which was founded in 1999, is an outpatient clinic aiming to provide free medical services for marginalized groups. Medical and socioeconomic data of patients at the outpatient clinic have been collected electronically since 2003. The purpose of this study was to find out which persons were treated at the Marienambulanz in the last seven years (2003-2009) and to find out the most frequent reasons for attending the Marienambulanz. We analysed existing medical data descriptively in order to find out the four most frequent reasons for going to the outpatient clinic, th

[Biographies following release from mental hospital, Glasgow 1875-1921].

For a long time mental asylums were seen as hermetically sealed units for the long-term confinement of patients. The broad and excluding nature of these establishments was their most prominent feature. From the end of the last century socio-historical and patient-oriented research has questioned and revised these properties. The present essay is based on that research. Using the example of a pauper asylum in Glasgow between 1875 and 1921 the essay analyses the number of released patients, how they were released and how they lived after being released. The sources used were individual patient f

Successful management of latent tuberculosis infection in an underserved community by a student-run free clinic.

The management of latent tuberculosis infection (LTBI) most commonly consists of a nine-month course of isoniazid (INH) therapy and is complicated by low adherence and completion rates. The Latent Tuberculosis Initiative at the HAVEN Free Clinic was developed to provide LTBI treatment to an underserved, high-risk, foreign-born population. We conducted a retrospective chart review to evaluate the program. Of 39 patients enrolled, 26 (67%) successfully completed nine months of INH, eight (21%) discontinued, and five (12%) were lost to follow-up. Patients had a median of nine encounters during th

The association between tobacco, alcohol, and drug use, stress, and depression among uninsured free clinic patients: U.S.-born English speakers, non-U.S.-born English speakers, and Spanish speakers.

The abuse of substances is a significant public health issue. Perceived stress and depression have been found to be related to the abuse of substances. The purpose of this study is to examine the prevalence of substance use (i.e., alcohol problems, smoking, and drug use) and the association between substance use, perceived stress, and depression among free clinic patients. Patients completed a self-administered survey in 2015 (N = 504). The overall prevalence of substance use among free clinic patients was not high compared to the U.S. general population. U.S.-born English speakers reported a

A novel patient support program to address isotretinoin adherence: proof-of-concept analysis.

The iPLEDGE protocol for isotretinoin treatment requires multiple steps to be completed within strict timing windows, resulting in many interruptions or discontinuations of treatment. The US Food and Drug Administration has indicated that approximately 40% of isotretinoin prescriptions written over the course of one year of the iPLEDGE program were denied due to failure to comply with iPLEDGE. Insurance restrictions add to the likelihood of prescriptions not being filled. Here, we describe a novel program implemented specifically to assist patients and providers with improving isotretinoin the

A patient navigation intervention for drug-involved former prison inmates.

BACKGROUND: Former prison inmates experience high rates of hospitalizations and death during the transition from prison to the community, particularly from drug-related causes and early after release. The authors designed a randomized controlled trial (RCT) of patient navigation to reduce barriers to health care and hospitalizations for former prison inmates. METHODS: Forty former prison inmates with a history of drug involvement were recruited and randomized within 15 days after prison release. Participants were randomized to receive 3 months of patient navigation (PN) with facilitated enrol

Poor compliance with postmolar surveillance and treatment protocols by indigent women.

OBJECTIVE: To estimate compliance by indigent women with surveillance protocols after molar pregnancy. METHODS: Women whose molar pregnancies were evacuated at an urban, public hospital were advised to return weekly either until hCG levels decreased below 5 mIU/mL, then monthly for 6 months, or until diagnosis and treatment of gestational trophoblastic disease, then monthly for 12 months. Hormone testing was by enzyme-linked immunosorbent assay. Statistical analysis was by chi(2) tests. RESULTS: Of 51 women identified, 11 (22%) developed trophoblastic disease. All achieved remission after ch

Professional monopoly, social covenant, and access to oral health care in the United States.

Lack of access to oral care is a severe problem in the United States with over one-third of the population lacking dental insurance. In this group, 32 million people lack dental insurance and access to public dental services (Medicaid or Medicare), and 7 million of them need dental care. In some high-risk populations, such as Native Americans, two-thirds have unmet dental needs. Only 1 percent of Medicaid-eligible babies have a dental examination before twelve months of age. In this paper the social covenant of the dental profession is examined and suggestions made for improving access to care

Identification of High-Performing Safety Net Hospitals for Radical Cystectomy: Implications for National Cancer Institute Cancer Center Partnership.

PURPOSE: For patients with bladder cancer (BCa) undergoing radical cystectomy (RC), National Cancer Institute-designated (NCI-D) cancer centers are considered the gold standard treatment hospital. Underserved populations face disparities in access to NCI-D and receive care at safety net hospitals (SNHs). Variation in quality of care for the underserved at safety-nets remains understudied. We hypothesized that safety-nets with NCI-D partnerships (affiliated safety-nets) would have superior RC outcomes compared with unaffiliated safety-nets. METHODS: We compared outcomes for BCa undergoing RC b

Indications to LDLT: a changing pathway?

Liver transplantation (LT) revolutionized the outlook for cirrhotic patients, offering a potential cure with over 80% life expectancy after 5 years. Cirrhosis, with or without hepatocellular carcinoma (HCC), is the primary LT indication. Living donor LT (LDLT) initially explored as an alternative, declined due to poorer outcomes. Studies on LDLT improved outcomes through precise recipient selection, emphasizing the importance of careful donor/recipient matching. Emerging concepts like left lobe preference and minimally invasive donor approaches enhance LDLT outcomes. The RAPID technique shows

Nucleated red blood cells as a marker of acidemia in term neonates.

OBJECTIVES: Nucleated red blood cells are produced in increased numbers under hypoxic conditions. We sought to examine the relationship between nucleated red blood cell count in the circulations of term neonates and other possible markers of fetal hypoxia. STUDY DESIGN: We prospectively collected umbilical blood from all live-born neonates delivered at our institution. Arterial blood was analyzed for pH and blood gas values. Venous blood was analyzed for nucleated red blood cell count. We reviewed the medical records for maternal data and neonatal outcomes of gestations of >/=37 weeks' durati

Engaging Interprofessional Students to Develop and Evaluate an Educational Booklet for Cancer Caregivers.

Spouses, adult children, or friends who provide uncompensated, comprehensive care to people living with a serious illness are often referred to as caregivers. Caregivers for patients with cancer spend considerable time and effort providing intense care and dealing with numerous challenges. The project aimed to engage learners from various health professions to develop and evaluate printed educational material addressing the multidimensional needs of cancer caregivers. Nine students from nursing, social work, and pharmacy attended weekly sessions from September 2023 to July 2024, developing con

Variation, Overlap, and Stability in Defining Safety Net Hospitals.

IMPORTANCE: The lack of universally accepted definitions for safety net hospitals (SNHs) has made it difficult to effectively design policies to support these hospitals and the populations they serve. OBJECTIVE: To evaluate the overlap, variation, and consistency across different definitions for SNH status. DESIGN, SETTING, AND PARTICIPANTS: This retrospective cohort study used a hospital year-level dataset on short-term acute care US hospitals from 2014 to 2022. Hospital-level and area-level measures were used to define SNHs. Hospital characteristics under each definition, overlap across de

The health care safety net: money matters but savvy leadership counts.

The nation's health care safety net--heavily reliant on external funding and support--is uniquely vulnerable to shifting and often adverse market and policy conditions. While adequate funding is essential to ensuring safety net providers can care for low-income people, the Center for Studying Health System Change (HSC) has identified a number of other factors key to building and maintaining viable community safety nets. Throughout the four rounds of HSC's Community Tracking Study (CTS) site visits, researchers have found that strong political and organizational leadership, community support, c

The Institutional Effects of Incarceration: Spillovers From Criminal Justice to Health Care.

POLICY POINTS: The steady increase in incarceration is related to the quality and functioning of the health care system. US states that incarcerate a larger number of people show declines in overall access to and quality of care, rooted in high levels of uninsurance and relatively poor health of former inmates. Providing health care to former inmates would ease the difficulties of inmates and their families. It might also prevent broader adverse spillovers to the health care system. The health care system and the criminal justice system are related in real but underappreciated ways. CONTEXT:

[Cost analysis of contrast-enhanced cranial MRI at a German university hospital].

PURPOSE: Detailed evaluation and cost analysis of a cranial contrast-enhanced MRI (c-ceMRI) in outpatients, inpatients, patients in an intensive care unit and children under anesthesia. MATERIALS AND METHODS: Based on a detailed process-oriented model, we calculated the cost of a cranial MRI for the four situations mentioned above. A comprehensive evaluation of the overhead and personnel costs was performed. RESULTS: We performed 5108 MRI examinations on 2 scanners in the year 2008. 2150 examinations (42 %) were identified as c-ceMRI. For inpatients we calculated a total cost of € 242.46 per
