Health insurance coverage in Mexico: progress, inequalities and remaining challenges towards UHC2030.

BACKGROUND: Universal health coverage (UHC) requires strong institutional capacity, equity-oriented policies, sustained political and financial commitment and public trust. However, public confidence in many health systems, including Mexico's, has been chronically undermined. This study aims to document Mexico's health coverage trajectory by offering a comprehensive, disaggregated and longitudinal assessment of insurance coverage from 2000 to 2023 - highlighting both achievements and setbacks in the context of UHC2030 goals. METHODS: This study used nationally representative data from Mexico'

The journey to UHC: how well are vertical programmes integrated in the health benefits package? A scoping review.

BACKGROUND: Countries are recommended to progressively work towards universal health coverage (UHC), and to make explicit choices regarding the expansion of priority services. However, there is little guidance on how to manage the inclusion of vertical programmes, funded by external partners, in health benefits packages (HBP) in low and middle-income countries (LMICs). OBJECTIVE: We conducted a scoping review to map the inclusion of six vertical programmes (HIV, tuberculosis, malaria, maternal and child health, contraceptives, immunisation) in 26 LMICs. METHODS: We identified 26 LMICs with a

Healthcare systems and motivation.

Despite the fact that most American physicians, at least until around the 1970s, stood in the way of developing a universal healthcare system, most are generally not happy with the current state of healthcare--or its lack thereof--today. The primary reasons for this general unhappiness are that insurance companies and managed care have successfully conspired to remove much of the physician's autonomy (via imposed time constraints, burdensome paperwork, the time-consuming chore of having to defend going against stringent treatment algorithms that are often inappropriate for some patients) and t

Strengthening financial protection through multitiered health coverage: evidence from 667 055 low-income households in China.

INTRODUCTION: Low-income households face disproportionate out-of-pocket hardship when accessing healthcare, undermining equity and universal health coverage. To address these challenges, China implemented a three-tiered health coverage system-basic medical insurance, catastrophic medical insurance and medical assistance-but its comprehensive impact on the financial protection remains underexplored. METHODS: We analysed Shaanxi Province's Medical Assistance database (2013-2016) to evaluate the impact of China's multitiered health coverage system on financial protection by: (1) estimating the i

Evolution and synergy of healthcare security policies for rare diseases in China: a quantitative content analysis.

BACKGROUND: Managing rare diseases presents substantial challenges to achieving universal health coverage. Despite growing policy interest, limited research has explored the evolution and synergy of healthcare security policies targeting rare diseases. This study addresses this gap through a case study of China. METHODS: An innovative analytical framework was developed, structured around three dimensions of policy synergy: horizontal, vertical, and temporal. A comprehensive dataset of national and provincial policy documents related to healthcare security for rare diseases was systematically

The political economy of universal health coverage reform in Kenya: executive dominance, elite alignment, and populist strategy (2004-2023).

Why do some ambitious social health insurance reforms get enacted, while others stall, even within the same country, and under similar structural constraints? This article compares Kenya's failed 2004 National Social Health Insurance Fund Bill, with the reform enacted in 2023. It combines Kingdon's Multiple Streams Approach (MSA) with political economy and populism theory, drawing on 48 elite interviews, and policy documents. Using outcome-explaining process tracing, the study explains why enactment became possible in 2023 but not 2004. MSA highlights how adoption depends on coupling across pr

Human resources for health development: toward realizing Universal Health Coverage in Japan.

Human resources are an important factor in establishing universal health coverage (UHC). We examined Japan's health policies related to development of human resources for health (HRH) toward establishing UHC, and tried to formulate a model for other countries wanting to introduce UHC through reviewing existing data and documents related to Japan's history in developing HRH. In the results, there were four phases of HRH development in Japan: Phase 1 involved a shortage of HRH; Phase 2 was characterized by rapid production of less-educated HRH; Phase 3 involved introduction of quality improvemen

Understanding pathways from primary health care to universal health coverage outcomes: a realist review.

INTRODUCTION: Primary Health Care (PHC) rooted in community empowerment, integrated service delivery, and multisectoral action is central to efforts toward Universal Health Coverage (UHC), yet little is known about how PHC reforms influence UHC outcomes across contexts. This review examines how PHC approaches affect population coverage, service coverage, and financial protection by identifying what works, for whom, under what circumstances, and why. METHODS: A realist review was conducted using studies from countries with the greatest gains in the UHC Service Coverage Index between 2000 and 2

Universal health coverage and intersectoral action for health: key messages from Disease Control Priorities, 3rd edition.

The World Bank is publishing nine volumes of Disease Control Priorities, 3rd edition (DCP3) between 2015 and 2018. Volume 9, Improving Health and Reducing Poverty, summarises the main messages from all the volumes and contains cross-cutting analyses. This Review draws on all nine volumes to convey conclusions. The analysis in DCP3 is built around 21 essential packages that were developed in the nine volumes. Each essential package addresses the concerns of a major professional community (eg, child health or surgery) and contains a mix of intersectoral policies and health-sector interventions.

Informing policy through evidence: A scoping review of factors that influence enrolment in community-based health insurance in East Africa.

BACKGROUND: Community-based health insurance (CBHI) has emerged as a promising option to enhance the attainment of Universal Health Coverage in Low- and middle-income countries and Sub-Saharan Africa. However, CBHI schemes in Africa particularly in the eastern regions, grapple with chronic low enrolment, jeopardizing their sustainability and intended impact. Understanding the determinants of enrolment in these schemes is critical for designing effective strategies to boost participation. In this scoping review, we categorized and discussed these determinants across socio-ecological levels. ME

Administrative perspectives on the implementation and sustainability of state-supported health insurance schemes in Nigeria: a descriptive qualitative study.

BACKGROUND: Since the state-supported health insurance schemes (SSHIS) began in Nigeria, the perspectives of implementers and other administrative actors have been under-documented in the program evaluations. Bridging this information gap is crucial to addressing the challenges impeding the scheme's impact. This study, therefore, investigated the administrative stakeholders' perspective on the implementation and sustainability of the SSHIS in Nigeria. METHODS: This study adopted a descriptive qualitative case study design and was conducted in six Nigerian states with functional SSHIS, each re

Estimating the threshold of health workforce densities towards universal health coverage in Africa.

BACKGROUND: There have been past efforts to develop benchmarks for health workforce (HWF) needs across countries which have been helpful for advocacy and planning. Still, they have neither been country-specific nor disaggregated by cadre-primarily due to data inadequacies. This paper presents an analysis to estimate a threshold of 13 cadres of HWF density to support the progressive realisation of universal health coverage (UHC). METHOD: Using UHC service coverage as the outcome measure, a two-level structural equation model was specified and analysed in STATA V.16. In the first level of struc

Evidences supporting the inclusion of immigrants in the universal healthcare coverage.

In 2012, the Spanish government enforced a healthcare exclusion policy against undocumented immigrants. The newly elected government has recently derogated this policy. To analyze how this decree could have affected population health, we looked at primary health patients who would have been excluded and compared with a matched sample of non-excluded patients. Potentially excluded patients had decreased odds of: depression, chronic obstructive pulmonary disease, dyslipidaemia, heart failure and hypertension while diabetes mellitus rates were similar to non-excluded. Infectious diseases were mor

Primary Health Care Systems and Their Contribution to Universal Health Coverage and Improved Health Status in Seven Countries: An Explanatory Mixed-Methods Review.

BACKGROUND: Primary health care (PHC) systems and their successes and challenges vary between and within countries. We elucidate the role of PHC on health status and universal health coverage (UHC) by describing the achievements and challenges of PHC systems in seven countries representing the three economic levels: high-income (Belgium, Australia), middle-income (South Africa, Thailand), and low-income countries (Cambodia, Ethiopia, and Nepal). METHODS: We adopted a mixed-methods approach and (a) extracted quantitative data on the key health and universal health coverage index of countries a

Out-of-pocket health expenditures and financial protection in Türkiye: pre-pandemic (2019) and COVID-19 era (2022) microdata.

BACKGROUND: In alignment with the UN Sustainable Development Goal 3.8 on Universal Health Coverage, this study assesses the extent to which Türkiye’s health insurance system protected households from out-of-pocket (OOP) health expenditures before and during the Covid-19 pandemic. Despite the implementation of the Health Transformation Program and Universal Health Insurance, concerns remain about financial protection, particularly amid economic downturns and the rapid expansion of private healthcare. METHODS: Using nationally representative Household Budget Survey data from 2019 (pre-pandemic)

Universal health coverage and incarceration.

Global progress towards universal coverage of essential health services, a component of UN Sustainable Development Goal (SDG) 3.8, is measured at the country level using the WHO Service Coverage Index. However, data collection for this crucial metric excludes prisons and youth detention centres, despite the health needs in these settings, chronic underinvestment in custodial health care, and poor health outcomes for people released from custody in most countries. Particularly in countries with high incarceration rates, failure to include custodial settings in calculations of the service covera
