Paget-von Schrötter syndrome: upper extremity deep vein thrombosis after heavy exercise.

We present a 52-year-old man with swelling, tightness, heaviness, and redness in the right half of his neck, face, upper arm, and right precordial area after repetitive exercises. No etiological factors such as trauma, venous catheter, malignancy, hematologic disorder history, or any chronic disease were noted. A Doppler ultrasound of the upper extremities was performed, which showed a complete thrombotic occlusion in the right internal jugular vein, partial occlusion in the right brachiocephalic and subclavian veins with thrombosis, and chronic thrombosis occluding the left brachial vein. Upper extremity deep vein thrombosis after suspension of progesterone-only oral treatment.

The intake of steroid hormone contraceptives is a strong and independent risk factor for venous thromboembolism. Several studies have assessed an increased risk of venous thromboembolism in women using oral contraceptives who are carriers of the G20210A mutation in the prothrombin gene. Most trials evaluating the thrombotic risk of oral contraceptives are based on combined oral preparations, but only a few focus on progestogen-only oral preparations. Results from such studies are conflicting and globally assess the thrombotic risk, ranging from modest to slightly increased. Furthermore, little

Exercise-induced deep vein thrombosis of the upper extremity. 1. Literature review.

Paget-Schroetter syndrome or effort-related upper extremity deep vein thrombosis is a rare condition that usually afflicts young healthy individuals, most commonly males. The cause is multifactorial but almost always involves extrinsic compression of the subclavian vein at the thoracic inlet, causing venous stenosis from repetitive trauma. The diagnosis of this condition may be difficult, and its delay may contribute to potential complications including thrombosis progression, pulmonary embolism, thrombosis recurrence, and post-thrombotic syndrome. Similarly, the best therapeutic option has no

[Deep vein thrombosis in the upper extremities focusing on Paget-Schrötter syndrome].

Among the three main forms of upper extremity deep vein thrombosis (UE-DVT), catheter-related UE-DVT has the highest incidence, cancer-associated UE-DVT the highest mortality, and primary UE-DVT (Paget-Schrötter syndrome) the most varied treatment proposals. This review finds that nticoagulant treatment (AC) for 3-6 months leads to symptom freedom or mild postthrombotic syndrome in most patients. Still, several studies show better results after surgical treatment if performed less-than 6 weeks after symptom onset. A randomized study of AC alone versus AC + surgery less-than 6 weeks in patients

A STROBE cohort study of 755 deep and superficial upper-extremity vein thrombosis.

Upper extremity vein thrombosis (UE-VT) are more and more frequent pathologies and yet little studied. The aim is to describe the clinical and ultrasound features, UE-VT-related diseases, and the prevalence of pulmonary embolism (PE) and associated deaths. All UE-VT patients diagnosed by Doppler-ultrasound in Nantes University Hospital, from January 2015 to December 2017, were included retrospectively. UE-VT suspicion patterns, clinical features, UE-VT topography, and prevalence of PE and death were analyzed.Seven hundred and fifty-five UE-VT were analyzed, including 427 deep thrombosis (UE-DVT

Upper Extremity Deep Vein Thrombosis: The Oft-forgotten Cousin of Venous Thromboembolic Disease.

Upper extremity deep venous thrombosis (UEDVT) accounts for between 4% and 10% of all deep venous thromboses, and may be due to primary or secondary causes. The incidence of UEDVT is increasing, partly due to the exponential growth in the use of central venous catheters and the increasing placement of permanent cardiac pacemaker or defibrillator devices. UEDVT not only compromises future vascular access but results in significant morbidity and mortality, with symptomatic pulmonary embolus occurring in approximately 12%, post-thrombotic syndrome in 13%, and mortality ranging from 15-50%. Treatm

Upper Extremity Deep Vein Thrombosis in Cancer Patients: Clinical Course by Cancer Site.

BACKGROUND: Upper extremity deep vein thrombosis (UEDVT) is common in patients with cancer. Data on recurrence and bleeding after cancer-associated UEDVT are limited. OBJECTIVES: The objectives were to identify factors associated with venous thromboembolism (VTE) recurrence and bleeding in patients with cancer and UEDVT, overall and by cancer site, during and after anticoagulation. METHODS: We analyzed patients from the Registro Informatizado de Pacientes con Enfermedad TromboEmbólica (RIETE), an international, prospective, observational registry of objectively confirmed VTE. We estimated cu

Breaking the blockage by advancing treatment strategies for upper extremity deep vein thrombosis.

BACKGROUND: Catheter-directed thrombolysis (CDT) and percutaneous mechanical thrombectomy (PMT) are established endovascular strategies for the treatment of deep vein thrombosis (DVT). However, comparative outcomes data specific to upper extremity DVT (UEDVT) remain limited. Given that PMT is associated with improved outcomes in lower extremity DVT, we hypothesized that PMT would be associated with improved mortality and morbidity compared with CDT in treating UEDVT. This study evaluates outcomes associated with CDT vs PMT for UEDVT and Paget-Schroetter syndrome (PSS). METHODS: A retrospectiv

Incidence of upper extremity deep vein thrombosis in the retrosternal reconstruction after esophagectomy.

BACKGROUND: Upper extremity deep vein thrombosis (UEDVT) is relatively rare but cannot be negligible because it can cause fatal complications. Although it is reported that the occurrence rate of UEDVT has increased due to central venous catheter (CVC), cancer, and surgical invasion, there is still limited information for esophagectomy. The aim of this study was to evaluate the clinical factors, including CVC placement and thromboprophylaxis approach, as well as retrosternal space's width as a predictive factor for UEDVT in patients receiving esophagectomy. METHODS: This study included 66 pati

A case report on 2 unique presentations of upper extremity deep vein thrombosis.

RATIONALE: Thoracic outlet syndrome (TOS) is a rare cause of upper extremity deep vein thrombosis (UEDVT). The treatment usually involves catheter directed thrombolysis followed by systemic anticoagulation. Surgical decompression is frequently recommended after anticoagulation for definitive therapy. PATIENT CONCERNS: We report two cases of UEDVT secondary to venous TOS with important clinical presentations. DIAGNOSES: Venous TOS. INTERVENTIONS: One patient was initially treated conservatively but had a recurrent UEDVT. The second patient had a residual stump from a prior rib resection that

How I treat central venous access device-related upper extremity deep vein thrombosis.

Central venous access device (CVAD)-related thrombosis (CRT) is a common complication among patients requiring central venous access as part of their medical care. Complications of CRT include pulmonary embolism, recurrent deep venous thrombosis, loss of central venous access, and postthrombotic syndrome. Patient-, device-, and treatment-related factors can influence the risk of CRT. Despite numerous randomized controlled trials, the clinical benefit of pharmacologic thromboprophylaxis for the prevention of CRT remains to be established. Therefore, minimizing patient exposure to known risk fac

Upper extremity deep vein thrombosis (Paget-Schroetter syndrome) after surfing: a case report.

This case report summarises the presentation of a 28-year-old female with signs and symptoms characteristic of thoracic outlet syndrome, but who was later found to have an effort-induced Upper Extremity Deep Vein Thrombosis (UEDVT), otherwise known as Paget-Schroetter syndrome. Effort-induced UEDVT is rare, but the similarity between the signs and symptoms of thoracic outlet syndrome and this type of thrombosis can result in patients with this condition presenting to musculoskeletal therapists. The key features of the case are described, followed by an overview of UEDVT and the importance of r

Permanent pacemaker-related upper extremity deep vein thrombosis: a series of 20 cases.

BACKGROUND: Upper extremity deep vein thrombosis (DVT) has become a common occurrence nowadays due to emergence of multiple newer risk factors, such as implantation of permanent pacemaker (PPM) and implantable cardioverter-defibrillators. We are reporting 20 cases of upper extremity DVT related to PPM implantation. METHODS AND RESULT: All the patients presented within 6 months, with unilateral upper extremity swelling (90%), pain (45%), erythema (15%), and other less frequent features. Venography established the diagnosis in all the cases. Considering the distribution of possible risk factors

A case of upper extremity deep vein thrombosis due to Kikuchi Fujimoto disease.

Kikuchi Fujimoto disease (KFD) is a rare form of lymphadeopathy with systemic symptoms. We present a case of a 31-year-old female farmer who was admitted to the emergency ward because of swelling of the left arm. Upper extremity venous thrombosis due to local compression of lymph nodes was diagnosed. The histological workup of lymph node biopsy showed histiocytic necrotizing lymphadenitis which is a typical sign of KFD. Anticoagulant treatment for the venous thrombosis was initiated. As KFD is often associated with a transition to systemic lupus erythematosus follow up visits are scheduled in

Upper extremity deep vein thrombosis in a 25 year old apparently healthy man.

This case of upper extremity deep vein thrombosis is selected for case report as it is a rare form of deep vein thrombosis without a very well established treatment modality and prognosis. The objective of this study was to report the outcome of a 25 years old male patient with idiopathic upper extremity deep vein thrombosis treated conservatively with low molecular weight heparin (LMWH) and oral warfarin. The data sources used were patient interview, laboratory and radiology investigation results and patient charts. The patient had no apparent recurrence or complication for 3 years except the

Case Report: Upper extremity deep vein thrombosis revealing an occult invasive ductal breast carcinoma.

UNLABELLED: Deep vein thrombosis of the upper limb is a rare but clinically significant condition, as it may represent the first manifestation of an underlying malignancy and often poses diagnostic and therapeutic challenges. We present the case of a 66-year-old woman with no significant medical history, admitted for extensive deep vein thrombosis of the left upper limb that developed shortly after surgical decompression of the carpal tunnel. Postoperatively, she presented with marked swelling of the left arm, and Doppler ultrasonography confirmed thrombosis of the deep venous system. Thoracic

Spontaneous upper extremity deep vein thrombosis as initial manifestation of a malignant mediastinal teratoma.

Upper extremity deep vein thrombosis (UEDVT) is an uncommon clinical condition, accounting for fewer than 10% of all deep vein thromboses. While catheter-associated thrombosis and anatomical abnormalities are well-recognized causes, spontaneous UEDVT may occasionally serve as the first manifestation of an underlying malignancy. We report the case of a 43-year-old previously healthy male who developed spontaneous thrombosis of the left subclavian vein. He tested heterozygous for the Factor V Leiden mutation. Imaging revealed a large anterior mediastinal mass compressing the brachiocephalic vein

Outcome of central venous catheter associated upper extremity deep vein thrombosis in cancer patients.

INTRODUCTION: Data on efficacy and safety of using low molecular weight heparin in cancer patients with catheter-related upper extremity deep vein thrombosis is scarce and the risk of recurrent venous thromboembolism after discontinuation of anticoagulation is unknown. MATERIAL AND METHODS: We conducted a retrospective cohort study including consecutive cancer outpatients assessed for the management of symptomatic central venous catheter-associated proximal upper extremity deep vein thrombosis. RESULTS: Of 99 included patients, 89 were treated with one month of full therapeutic weight-adjust
