Visually guided male urinary catheterization: a feasibility study.

INTRODUCTION: Ten percent to 15% of urinary catheterizations involve complications. New techniques to reduce risks and pain are indicated. This study examines the feasibility and safety of male urinary catheterization by nursing personnel using a visually guided device in a clinical setting. METHODS: The device, a 0.6-mm fiber-optic bundle inside a 14F triple-lumen flexible urinary catheter with a lubricious coating, irrigation port, and angled tip, connects to a camera, allowing real-time viewing of progress on a color monitor. Two emergency nurses were trained to use the device. Male patien

Intraperitoneal rupture of the bladder after urinary catheterization.

Intraperitoneal rupture of the bladder is a rare cause of peritonitis. Intraperitoneal rupture of the bladder was diagnosed during an emergency laparotomy for suspected mesenteric ischemia. The patient had undergone iterative urinary catheterization after a vascular bypass. The perforation was excised and sutured and the patient was catheterized for urinary rest for 15 days. Urinary catheterization is a possible cause of intraperitoneal rupture of the bladder.

Tako-tsubo syndrome following a difficult urinary catheterization.

Tako-tsubo syndrome is a cardiac syndrome triggered by emotional or physical stress and characterized by acute extensive but reversible akinesia of the apex and mid part of the left ventricle (LV) in the absence of obstructive coronary artery disease. It typically presents with chest pain and/or dyspnea and may mimic an acute coronary syndrome (ACS) with ischemic changes in the electrocardiogram and elevated cardiac biomarkers. The precise etiology remains unknown, but prognosis is generally excellent. This is the first reported case of Tako-tsubo syndrome following a difficult outpatient cyst

Urinary elimination: Part 2--retention, incontinence and catheterization.

In part two of this article, the nursing role in relation to the assessment and management of urinary retention, urinary incontinence and catheterization are considered using recent literature and best practice statements. Retention of urine and incontinence have a major detrimental impact on a person's life and nurses have an important role to play in supporting patients. Catheterization is often necessary for acute retention of urine but is the last resort for incontinence. However, when catheter use is appropriate, asepsis technique during insertion and continuing management can help to min

A reminder reduces urinary catheterization in hospitalized patients.

BACKGROUND: Indwelling urinary catheters are placed in up to 25% of hospitalized patients and are a leading cause of hospital-acquired infection. Duration of catheterization is the dominant risk factor for hospital-acquired urinary tract infection. Physicians are often unaware that their patients have a urinary catheter, and these "forgotten" catheters are frequently unnecessary. METHODS: A controlled trial, using a pretest-posttest design, was conducted on four hospital wards at an academic medical center. A simple written reminder was designed to aid the hospitalized patient's team in remem

[Intermittent catheterization of urinary bladder--indications, technique, complications].

The intermittent catheterization technique is considered at present to be one of pillars of the therapy of lower urinary tract dysfunction. The technique gradually became used also in the treatment of some non-neurogenic voiding dysfunctions. There are many ways, how the intermittent catheterization could be performed (methodology, catheter material and type, lubrication type and method, catheterization frequency). Authors provide an overview of present indications to use the treatment; they describe individual techniques and draw attention to the possible complications and their solution. Nex

Prevalence of urinary catheterization in UK nursing homes.

A postal questionnaire survey was undertaken in registered nursing homes in three different health districts in England: Gloucestershire, North Staffordshire and Leeds. Nursing homes may be registered as general nursing or mental health homes. If homes also have provision for residential beds these are defined as dual registered homes. Overall, 9% (438/4900) of residents, with an equal male:female split, had urinary catheters. There was no significant difference in the overall urinary catheterization rate in the three districts (P=0.9). There was a wide range of urinary catheterization prevale

Urinary Bladder Catheterization of Female Mice and Rats.

Catheterization of the urinary bladder of rats and mice is necessary for a variety of clinical and experimental reasons. The anatomy of the female mouse and rat is unique in that the urinary orifice is external and just anterior to the vaginal opening. This characteristic makes the bladder of these species easier to catheterize than that of other species. Methods cited in the literature do not adequately describe techniques that facilitate ease of urinary bladder catheterization (1). We describe a simple technique for urinary catheterization in adult female mice and rats.

Urinary tract catheterization protocols following total joint arthroplasty.

Six hundred one consecutive total joint arthroplasty patients were divided into three groups using three different catheterization protocols. Each group was evaluated for preoperative and postoperative urinary tract infections, number of catheterizations, and number of subsequent urinary tract manipulations. Group 1 (165 patients) was treated with "as needed" intermittent catheterization. One hundred six of these patients (64%) required a minimum of one catheterization with a total of 265 (mean 2.4 per patient) catheterizations. One post-operative urinary tract infection was recorded. Group 2

Current trends in the management of difficult urinary catheterizations.

Routine urinary catheter placement may cause trauma and poses a risk of infection. Male catheterization, in particular, can be difficult, especially in patients with enlarged prostate glands or other potentially obstructive conditions in the lower urinary tract. Solutions to problematic urinary catheterization are not well known and when difficult catheterization occurs, the risk of failed catheterization and concomitant complications increase. Repeated and unsuccessful attempts at urinary catheterization induce stress and pain for the patient, injury to the urethra, potential urethral strictu

Caring for catheterized patients: urinary catheter maintenance.

This article brings together the evidence and opinions surrounding the ongoing problem of catheter blockage and how to help maintain catheter life. The reasons behind blockage are examined as well as the roles that pH testing and building a comprehensive catheter history, including all aspects of patients' general health, play in caring for the catheterized patient. The use of catheter-maintenance solutions is addressed with discussion on smaller volumes, the use of double instillations and gentle technique for a shorter time of instillation and improvement in catheter-intervention techniques.

Elimination of bleeding associated with urinary catheterization in neonates.

Three cases of hematuria associated with urinary catheterization in infant males in this NICU over a year prompted an investigation by the health care team. The team tracked catheterizations in the unit, conducted a national survey, and did a literature review. Based on their findings, the team revised the unit policy and procedure and evaluated new products. After changes were implemented, the incidence of hematuria--originally at 20.7 percent--dropped to 5.5 percent during the first evaluation period and to zero during a second follow-up period.

Reducing urinary tract infections in catheterized patients.

To compare the effectiveness of two drainage systems in controlling urinary tract infections (UTIs), 65 elderly home care patients with indwelling urinary catheters participated in a retrospective intervention study. The patients first used a Foley drainable bag (DB) system, followed by a nondrainable one (NDB). Both systems used a Foley catheter. Data were obtained from physicians, nurses, caregivers, and patients regarding the number of UTIs and hospitalizations that occurred when using each system. Using the DB, 65 patients had 1,395 UTIs, 27 of which required hospitalization. Using the NDB

Longer Duration of Urinary Catheterization Increases Catheter-Associated Urinary Tract Infection in PICU.

OBJECTIVES: Catheter-associated urinary tract infections account for 30% of healthcare-associated infections. To date, few studies have addressed pediatric catheter-associated urinary tract infection in PICUs. The aim of our study was to assess the risk of catheter-associated urinary tract infection in relation to the duration of catheterization in the PICU. DESIGN: Retrospective cohort study. SETTING: PICU at a tertiary children's hospital. PATIENTS: Our study was conducted between April 2012 and June 2015 at Tokyo Metropolitan Children's Medical Center in Japan. Children in the PICU with

Impact of urinary catheterization on geriatric inpatients with community-acquired urinary tract infections.

BACKGROUND: Urinary tract infections commonly cause hospitalizations in community-dwelling geriatric populations. Our aim was to understand the impact of urinary catheterization on geriatric inpatients with community-acquired urinary tract infections (CAUTIs). METHODS: Retrospective analyses were performed using electronic discharge summaries in a rural community hospital of northeastern Taiwan in 2004. We screened data with ICD-9-CM codes and performed chart reviews on inpatients aged >or= 65 years with CAUTIs. RESULTS: A total of 294 subjects who experienced CAUTIs were enrolled; 114 subje

[Effectiveness of cleaning or disinfecting the urinary meatus before urinary catheterization: a systematic review].

The urinary tract infections associated with catheterization are very common in hospital and home care contexts. Currently there are several recommendations for its prevention, however, when approaching the kind of solute used in the urinary meatus prior to catheterization doubts continue to persist. Thus this study aimed at determining the effectiveness of cleaning the urinary meatus with water or saline comparing to its sterilization through a systematic review and meta-analysis. In order to do so, the principles proposed by the Cochrane Handbook were followed, a critical analysis was conduc
